I always assumed the weird mutations like tentacles and extra eyes/mouths for nurgle was to represent the nurgle boiz being more like actual horrible little microbes than multi cellular organisms

@@Sercotani eyes, mouths, limbs ... basically any body part. Tzeentchs thing is change, i.e. mutation.
To me, at least, extra mouths don't really seem "Nurgle-y"

With Nurgle: A) You worship because you've been coerced. "Worship me and I will take away the pain from the diseases I have given you", and B) You spread an immense amount of pain,once you become a vector for his plagues, not all his victims are allowed into his grace.
At least with the other Gods, you usually choose to succumb and there are TINY paths to doing semi-noble things, with Khorne you can try to be honorable, with Slaanesh you can strive for excellence, with Tzeentch you can try to change things for the better.

See, the issue with giving Khaos a win is that you end up with what happened to the "old world". I don't think any of us want that to happen again.
