Installing permanent grab bars in a shower stall can be a nightmare.

Can the suction cup grab bars b used on a 4" tile in tub/ shower

Not unless the suction cups on the grab bar are small. If any part of the suction cup is going over grout, or a curved area, it isn't safe.
