Before. Part of the reason he tried to kill himself (as seen in Saw II) was because he felt he had no options left. The man was basically dead: his unborn son was miscarried, he and Jill had divorced, and he had an inoperable tumor. Finding out that nobody gave enough of a damn to help him probably sent him over the edge. It's why he can keep himself personally distant from his victims.
I'm confuse, is this scene before he became Jigsaw or after? If this is after why did William not notice he is a serial killer?
