I looked back at the release dates of the episodes and the cards and at least Dimension Fusion and Painful Choice debuted in the anime at least a year after the ocg release. I think those were examples of the anime using pre-existing cards. I doubt that's the case for Card of Safe Return so I didn't bother but yeah. Also this doesn't prove that Konami invented those 2 cards and that the anime added them to the show because I don't know about how far back the scripts, concepts and ideas dated. Also to note, we can't reference the manga because these appeared in anime filler arcs.

I grew up watching the anime and getting into the TCG at the same time. The first time you notice the shift in the anime really pushing existing cards (particularly newly released sets) was during the Noah saga, which at the time was the release of Legacy of Darkness. Noah's duel was basically an advertisement for spirit monsters lol. Ever since then the anime continued that trend of pushing the new cards instead of the reverse where the earlier cards were based off the anime.

The concept of the card “Last Turn” just befuddles me. I get that you want alternate win conditions, and it’s limited to being a trap card, but it still just promotes a strategy of not caring about the game, just about literally who has the highest atk monster in their deck. It doesn’t even need to be otherwise playable, just strength is all that matters.

With Last Turn the worst was the forcing of draws, the ruling surrounding this "Special battle Phase" that was to occur with its effect and how it interacted with cards on the field during and after the resolution of that card. Also, the monster selected by the opponent does still have to be playable under regular special summoned circumstances. Meaning you can't select monsters that have a special requirement to summon them.

Yeah that were it gets into the bad sportsmanship. As in OCG you can only give up during your turn. Which I guess they could make the TCG have that rule.
But that would create a questionable problem with slow playing. Doing combos to slow play. Because your able to do that. And letting a duel be over before the next turn or risk losing a match to an extremely hard to summon monster that only effect is it can make you not play an Extra game.
I think for Konami to see if it possible they require more judges so it reduce player abuse with slow play. And to have test effects of things that work on outside the current game being played. Since match winning cards are they only ones that do that. So LP or additional summons might be the starting and testing point.

Dimension fusion ahh what a memorie... i remember when i played the yugioh GX tag force 2 game i had a dragon deck with a particular combo (don't know if it was meta or now, i really don't care either) which consist in running future fusion, dragon mirror, and dimension fusion; so i had the chance, on turn 1, to use future fusion, to bring un 1 FGD to the field, then use dragon mirror, bring ANOTHER FGD to the field, and then use dimension fusion to bring back other 3 dragons from out of the game, and know what's better? light and darkness dragon already was a thing, which is one of the best disruptors in the game, i just made sure to use him first on the dragon mirror effect and there you go, turn 1, 1 FGDs 1 DnL dragon, and 3 randoms dragons (the another FGD never made it out because the board was full and future fusion triggers the second turn after it's activation)
Of course this only happend like 3% of the time but was really funny... shame that was a PBE game i would love to see my opponent expression when that sort of thing happend.

Was a fun combo, along with my FJ, Serpentine Princess, and Injection Fairy Lilly deck. Both annoyed my friends and others - the sign of the best decks out there! I never really played a deck to always win, my main focus - other than constantly trying new combinations and strategies for the heck of it - was to always play a deck that was different from the current meta and had the added benefit of annoying my opponents as much as possible. If the deck won, so much the better, and if it trashed the popular meta decks, better yet. Always hated the idea that was so prevalent at the time that you had to be playing this or that deck to be any good or to win the tourney.
Ran into so many 'players' that belittled and derided those that didn't play the most recent popular decks, and then ranted and raved when they played those decks and lost to the oddball strategy of someone who didn't play those decks, then rage quit. It was like watching 3 year olds having a fit because they didn't get their favorite toy (and sometimes, the 3-year olds had better self control than the older people that I played). After watching that for a couple of years, along with the direction that Konami was taking the game, I decided to quit the 'game' altogether. The enjoyment was no longer there, as there was no longer any 'dueling', it all began to show the signs of play everything you have on turn 1 or two, otherwise the deck is too slow and thus useless. There is no more true back and forth, just who goes first, then wins on the first or second turn.
No enjoyment in that kind of game. At least with something like MtG, there is a resource cost, thus forcing the game to slow down to where both players actually face each other.

I like how card of safe return was even more broken in the anime as it let you draw 3 cards, hence why it has three cards on the card art

I think it's funny that chicken game takes the number 1 spot here considering it's the only one of these that's banned in the tcg but still at 3 in the ocg. Yes it still gets played in decks there but Konami clearly never saw it as a problem there

Card of Safe Return kind of had to exist though, because it appeared in an important manga duel. (One can say the same about Graceful Charity, for that matter.) And if Konami doesn’t know how to properly balance a card game, Kazuki Takahashi DEFINITELY doesn’t know.
I agree with you about the other four, though, and would also like to nominate the Chaos envoys on the grounds that they powercrept every other card, practically. There is no way during play testing that Konami couldn’t have noticed this, so it had to have been on purpose.

I have a decent fix for Chicken Game!:
The controller of this card cannot activate any of these effects until after their next End Phase.
Worded it like that to prevent Metaverse during the opponent's End Phase.

If Card of Safe Return was erated to be a once per turn similar to Supply Squad, then it might get off the ban list.

When it was banned, Painful choice was mainly banned due to it setting up Chaos decks. Pick 2 light and 2 dark monsters and one card that can return to your hand if it's sent to the grave. No matter which card they picked, you can often guarantee they're gonna use the set up to special summon Black Luster Soldier Envoy of the beginning or Chaos Emperor Dragon Envoy of the end and just sweep the field.

Tournament question: what occurs (in terms of who is eliminated/moves on) when there is a triple draw? Do both players get eliminated, do both move on? How does that work?

I really think that Yu-Gi-Oh! is no different than MTG when it comes to this topic. When a tcg is in it's early stages, it's impossible to tell how the game is going to evolve and propel into the future. And a lot of times, there a too many cards that have been printed to take into account all the mechanics and individual cards that could potentially make a new card or mechanic broken.

And the cost is considerable. 1000-5000LP with no battle phase is steep. But then again it's one of those "high risk-high reward" cards.

This video brings me back, but also reminds me why I am glad I switched to MTG. I can play with these types of cards but they are more balanced because I have to spend resources upon the card that limits what as player can do.
