i don’t think shaking hand is right as she didn’t even perform hand hygiene.

Nope! she will most likely wash her hands before she even starts vital signs. Besides patient ID, hand hygiene is second on the list of vital things to do before every "physical" patient interaction to prevent contamination or the spread of any other germs :)
