How I Treat Acne: An Overview of Treatment Options

  • I chose to break up the topic of Acne into multiple parts. Dermatologists do more than "pimple popping" but we do pride ourselves on being the ultimate experts at treating moderate to severe acne in kids/adolescents, teens and adults (adult acne or acne rosacea). Here I will go over the treatment options depending on the type of acne, the severity and the location of acne (jawline and chin distribution hints at hormonal acne). At the end of the video, I review over the counter options that are okay to consider before seeing a dermatologist. But remember that if you are having scarring from your acne, do not delay seeing a skin doctor to get your acne treated as scarring can be permanent and disfiguring.
    We also discuss acne scars and scar treatments as well as chemical peels by Skinceuticals (Micropeel 30 plus and Advanced Corrective Peel)!
    This is a very important topic as acne can affect self esteem and even cause depression. I love seeing my patients improve immensely from treatment and can regain their confidence after achieving their skin care goals!

    Good information on available options for acne treatment. Your suggestion on showing your dermatologist your list of medications and for how long is a good idea. I’ve used otc stuff that caused me to break out but didn’t know which ingredients caused the bad reaction

    Great video, thanks doc!
    But I have a question: I'm on Adapalene (0.1%) since 3 months against my acne. After this (long) time my acne gets better, but it feels like it's not enough. I still get pimples in areas where I usually get them and they appear again and again. My skin is sensitive/dry so I use twice a day the LRP Gentle Cleanser, sometimes Vichy Mineral 89 HA and always moisturizing cream (and yes now I want to add a sunscreen ^^). So I'll see my doctor soon, but does my situation sounds like a case where Tretinoin could be the better option? Thanks for sharing your opinion!

    Great video doc. How important is a toner in the skin care routine? I just started using Retin-A for acne and I'm worried that using a toner will irritate my skin.

      khonelihle Z thanks so much for watching! Great question. I think toner is not super important but it can help to help remove excess dirt during this time while we use more face masks. But you’re right that certain toners with a lot of AHA can irritate the skin further especially with a retinoid. Take care!

    Thank you for such an informative and clear video! I wish I lived in Seattle so I can get an appointment with you, but I’m from Canada.
    I am wondering, Is it better to use benzoyl peroxide as a leave on treatment or in wash form to prevent new breakouts? Also, is it okay to use glycolic acid lotion after benzoyl peroxide? What is a good drugstore sunscreen for acne prone skin? Thank you for your time!

      Sil R hello and thank you so much for watching! I like benzoyl peroxide wash for maintenance and the leave on spot treatment for active breakouts. I usually tell people to separate glycolic acid and bpo use to avoid irritation and rash. I like niacinamide in sunscreens: Cerave face lotion spf 50 and EltaMD uv clear are the best

    Hello! amazing video--do you think you could address moisturizers free of silicone? Every moisturizer seems to have it in it.
    Also, what is the main difference between Elta MD Am and Pm therapy? I see so many more recommendations about the am formulation.

I think your acne regiment with tret and dapsone shows that you are fairly optimized with some of the best topicals. I agree that almost all big name moisturizers have some form of silicone. Many of the silicone free moisturizers I am not familiar with their brands but I believe eltamd PM therapy does not have silicone whereas the AM does. I like that AM and PM are both niacinamide containing but AM has some additional theoretical benefits (in the real world, I’m not sure if they are doing what they’re supposed to): caffeine for helping with redness and maybe tightens skin (?) and its willow bark extract which supposedly exfoliates. If both of these ingredients do what they’re supposed to, I can see why AM is more appealing than PM therapy. Take care!

    Could you suggest makeup removers for dry but acne prone skin? Is the makeup eraser microfibre cloth ok to use, or oil cleanser followed by one of the washes you recommended?

    I had lots of acne growing up since 11 yo. Thank goodness to my acne doc I turned out okay now. I likely have trace rosacea.

    Thanks for this informative video! I think my skin might be sensitive to Niacinamide (amongst lots of other ingredients). Do you think it will be ok for me to use a cleanser with Niacinamide in it since it will be washed off? Or should I avoid it all together? My sensitivity presents as itchiness and small bumps (looks like pimples that never come to head). Thanks!

      kloujiko thanks so much for watching! Why do you say niacinamide specifically causes irritation? Just to throw it out there, I have been seeing quite a few patients getting irritation from The Ordinary niacinamide serum. Even if you have irritation from that product (could be a non active ingredient responsible for irritation), I would still keep an open mind to gentle cleansers with vitamin b3 in it.

      Dr. Daniel Sugai thanks for your quick reply! I was using cerave cream and found it a bit heavy so switched to cerave PM. It was amazing the first time I used it - woke up to really soft skin, and my t zone wasn’t oily like normal. Then I put it on for the day and my skin started getting itchy and started getting little bumps :( my skin still felt itchy for a few days when I switched back to cerave cream. It was also the first time I got the bumps. Usually I get a couple of hives when my skin is sensitive to a product.

      kloujiko definitely see people getting irritation from Cerave. I wouldn’t blame the niacinamide still and would try a cleanser with it. You could see your derm for a sample size before committing to purchasing it. Lmk how it goes! Take care 🤙

    So would you recommend using my gentle cleanser from vanicream once a day and then a foaming one like the cetaphil the other time? What if I workout and sweat a good bit?

    Aloha from the North Shore of Oahu,
    I'm wondering if medium chain triglycerides or caprylic/capric triglycerides can clog pores.
    I breakout from nearly every oil or heavy moisturizer that I have tried, so I'm wondering this these triglycerides would also clog me?
    I am just starting tazorac cream and MCTs are one of the ingredients.

    Hey, I have a question about retinol, if Vitamin A is fat soluble, would not wetting the face before applying any retinol serum be more effective at absorbing it into the skin? My thinking is that water would block any penetration into the skin. If the reason for use helps more, it's only to see if it reduces some fine lines, so purely cosmetic.

    Dr Sugai, cerave moisturise broke my skin. Its not purging due to niacianamide in it because i have been using the ordinary niacianimide from long time which has done great on my acne prone skin. Please recommend moisturiser for SENSITIVE DRY ACNE PRONE SKIN 😞😞we dont have elta in Australia. 😔

      Hi! I'm not Dr Sugai but had a similar issue. I find the La Roche Posay Toleriane Sensitive Fluide moisturizer to be the only one that really doesn't cause me any kind of issues, both short and long term, really recommend it. They do have a thicker formula too if you find you need it.

      JV Lamsen hello! Good q- never pop a pimple on your own! Your dermatologist can “pop” or extract those that are safe to do so. Many types of acne will cause permanent scarring when traumatized especially if it’s inflamed. Take care and thanks for watching! 🤙🙏🏼

      JV Lamsen need to see your derm to get anything extracted or injected and we assess it by different factors besides color (location, swelling/inflammation, type of acneiform lesion, surrounding scarring, etc). Take care.
