Massage Tutorial: Ankle pain techniques and strategy

Got massage clients with ankle pain? Consider all of the structures that might be contributing to that pain: the dorsi- and plantarflexors, and the inverters and everters. Realize that, while the pain may "live" in the ankle, the soft tissue criss-crossing the joint is likely playing a role in that pain sticking around.
With this particular client, I found it necessary to use lighter-pressure massage. Her sensitivity has been ramping up over the last few years, especially as she's transitioned from steady dance training to other activities. I have no intention of trying to break up adhesions, bust up "knots," or coax her fascia toward a particular direction. Instead, I'd like to give her nervous system the signal to calm down and reduce its sensitivity. While some clients might seek painful massage to treat painful conditions, I think that this is often counterproductive.

I felt everything you were doing. I wish I were her. I broke my ankle 6 weeks ago and although the bone is healed I have the exact same pain that she does. No doubt from being in a boot for a month. I tried to follow along and do what you were doing. Thank you.
I was in a boot for 6 months and lost my entire calf muscle and all strength from knee down. Still experience pain years later

+Marco Polo It's called professionalism, guys. Put your tongues back in your mouths. A heavy-set older lady, or a fit young woman, or a handsome athletic man, or an old frail gentleman with paper-thin skin and varicose veins...all just a body on a table, a person with pain, a collection of muscles, nerves, joints and emotions. When a professional looks at the body, we see an opportunity to lift a burden of pain. That's all.

I have had a lot of massage-work done in my life, and I have never received anything approaching the deep and careful work that can be seen in videos like this one. A quick, half-arsed rub of the neck and upper back and that's your lot is what seems to be the norm.

If you haven't already done so, please let me encourage you to go see a doctor for an evaluation. Sudden and severe pain is worth having checked out - and if it ends up being just muscle tension, then working with a physiotherapist can be a big help. Best of luck!

22:25 I tend to relate direction to their own body. This reduces (but does not eliminate) confusion. So, in this case, I would grab the heel and say "Rachel point the toes of this foot up towards your shin (or knee)."
You are relating to the client in their primary positional awareness (distil/proximal/medial/lateral) rather than in spacial and planar directions which they are disoriented to while on the table.
What do you do when you had a swollen ankle for over a year
Do you recommend stretches as home care? Or refer out for stretch and strengthening home care? Thank you for the great tutorial!
I do offer stretches that I'm able to demonstrate quickly and simply, like in this video: czcams.com/video/Ascz2whmczs/video.html For anything more complex, like a full stretching and strengthening regimen, I'll direct my clients to see a personal trainer or (in the case of pain) a physical therapist.

what oils do you use besides the jojoba?
Just the jojoba. If I were smart I'd keep some thick cream around for the feet (jojoba sometimes doesn't interact well with the texture of the plantar foot), but in those cases I just do less gliding. Have I mentioned how lazy I am? :)

Would you use cross frictions on the tendons to loosen them
+Adelia Chamberlain I don't put a lot of stock in our ability to loosen/lengthen connective tissue, or to break up collagen/scar tissue. Dense connective tissue is far stronger than the surrounding tissue, so any attempt to dig into it to change its structure is much more likely to cause microtrauma in the surrounding tissue (bruising). While we can likely promote connective tissue change over time by stimulating the activity of fibroclasts and fibroclasts, that would be over the course of many sessions, and with the assistance of the client making habit changes.
That said, I would want to work directly with the tendons and ligaments once they're able to tolerate contact. It's not to cause any structural chance, but rather to stimulate the embedded nervous tissue and, hopefully, further reduce sensitivity. Good question!
