  • when to start physical therapy after shoulder surgery and why. This is my 4 months post shoulder surgery experience and update after i had a labral tear arthroscopy repair ( tear in the labrum). so many viewers have been asking why i started physical therapy at 6 weeks post shoulder surgery, so in this video i will address why and give a demonstration as to what can happen if you start physio therapy too early.

    Thank you for your videos. I am now 11 weeks out of a massive rotator cuff tear repair. My rehab is slow and steady. Patience is key. I was told by my surgeon not to rush. He would rather see me go slow than re-tear my repair. The odds are less than 50% success with the size of tear and retraction. I agree, sleep is so so important. I took pain pills at night to sleep until about 5 weeks. Now I can take Advil PM or cannabis. The pain is better at night finally, about 9 weeks I slept well with little pain. I’ve had to talk myself off the proverbial ledge a few times because I am an active person and struggle with the limits and find myself having small panic attacks, but that passes and I move on. I really appreciate these videos. They are very helpful.

    Thanks for doing these videos. I'm doing this on Tuesday! Oh man. I wasn't sure if I should do it but in the weeks since I decided to do it I've realized my shoulder has actually gotten worse so I think it is good for me to go ahead and do it. Fortunately I do the kind of work where I should be able to get back to it pretty quickly. All I need to be able to do is make phone calls and type. Still... I'm nervous about it all. I hope I managed to end up on the four week sling plan, but who knows?

    Hi I got my surgery we went with the nerve block. It feels weird but icing really help numb my entire arm. Now the next step is getting healthy to be able to go back to training and playing sports.

      brent Hayden I had arthroscopic so I don’t know how the healing process is for open surgery. Even though I feel great my surgeon says I can’t lift until three more months

      brent Hayden I’m guessing that open shoulder surgery will take longer because right now I have three, fully healed, small incisions. And like I said Full range of motion and no pain. The only problem is the strength that you lose by just a few weeks out of the gym.

      Just so everyone knows this is not a common result from surgery. Happy for you Michael because your one of the lucky ones I'd say 7 out of 10 people I've talked to say they still struggle with range of motion and strength/fatigue/pain

    Hey bud I am really surprised that they didn’t start you on therapy right away. My therapy started a week and a half after my surgery. I had a torn labrum which they had to perform a tenodesis because of. I also had a rotator cuff repair, and they removed bone spurs as well. I have four holes in my shoulder and a 2 inch incision down my left bicep. My stitches were removed a week after surgery, and three days later I started therapy. My therapy started with complete assisted mobility exercises with no assistance from me at all. After two months, they started strength training exercises and I am at three months now with full mobility and my strength is coming back more and more every week. I do want to say that this whole process has been extremely painful at every therapy session, and the sessions that I perform at home as well. I saw my doctor about a week ago, and he explained that I shouldn’t have to see him again and that my shoulder should be fully recovered in another three months. I asked him why I feel so much pain during therapy and he said no pain no gain LOL. I want to be clear about the pain level. I believe that I feel the same amount of pain at every therapy session because we keep adding more and more weight to the movements. As I said before my strength is coming back gradually and my range of motion is back to normal and my day-to-day activities are handled more easily. I think that if I had a better diet, my progression would have been better and I am also a 48-year-old man that don’t heal as fast as a younger man would. At first I thought the therapy was too much too soon but now I don’t regret it because I feel really good.

      I was also supposed to be in the sling for six weeks but I stopped using it at home after four weeks because my elbow and wrist and hand were too stiff and it was more comfortable to be out of the sling and let the blood flow. Again, the process for me has been very painful but therapy very early after surgery is very very important. That is my experience, some may have different views and yes he is right, everybody is different.

      Yes I wake up in severe pain, Monday the 24th will only be my 3rd week post-op. Sleeping is very hard to do until my meds get to work then I’m only getting about 3 hours sometimes up to 4 hours of relief so maybe I’m rushing it but I have to start physical therapy on the 27th Im so scared it’s too early but my surgeon doesn’t want my shoulder to freeze up and he wants my mobility back ASAP Y’all wish me luck with the physical therapy session im sure it will set me up with more pain but I’ve gotta do this. My shoulder surgery was supposed to be for bone spur in there but turned out my pain was more so from a torn in half rotator cuff and the bone spur plus he had to make a tendon because it was completely gone. So surprise to my wonderful surgeon and myself. I’ve been doubting this since Christmas 2020 and just now had surgery on May 3rd 2021. I’ve toughened it out thinking it was nothing bad. Well it was and way more than I ever thought 😩 I appreciate your videos Bill I’d appreciate it if you keep us all informed of your journey thank you so much 😊 get stronger and stay well bill

    I'm 12 weeks out of SLAP type 2 and rotator cuff post op, my recovery is very slow with 60 % range of motion and my PT started right after surgery day, the good things is I can sleep well with no pain after 10 weeks and feelings so good after wakeup with a little of "Frozen shoulder"

    I had my surgery 3 months ago and my doc wanted that sling off the next day.... He wanted me to start making small movements with my arm. Physical therapy started immediately 4 days after.... the first visit with physical therapist they had me stretching my arm all the way up using a pulley. They said that you have so much weeks after the surgery to gain back your range of motion before it is gone. I had both my shoulders done with my left, 5 years ago and this time my right. My left shoulder took almost a year before it was 100% healed and back to lifting normally.... My Right arm was a little worse as it had a spur which they had to grind down and also put in anchors, so I'm expecting the right to take longer, but at this point the pain is almost gone and I have pretty much full range of motion besides reaching around my back with some pain. They said that is the last motion to come along so hopefully it will be gone in the next couple months. Sorry to hear about your experience but I do believe not starting physical therapy right away had a big role in your shoulder freezing. I know how much it sucks not being able to workout especially when you want to lift weights instead of doing cardio. good videos, keep it up man!

    Loving your vids! unfortunately I have had 2 surgeries on my dominant shoulder within a space of 6 months and I wish my surgeon explained it like you did! I am otherwise young fit and healthy, My advice would be is to also listen to your surgeon and ask questions.. Keep up the good work!

      I was ok after the second. I unfortunately went on to a further two - 4 in total. However shoulder is doing great! I am 19 months post the last one!

    I’m 17 years old I’m still in school and going through surgery tomorrow morning and in the earlier videos you said you weren’t able to put a t shirt and shoes on since you were in the sling and my problem is that I would have to put a polo shirt on since it’s my uniform and i don’t know how would that work and i don’t really want to push myself to put the shirt on

    You seem to be having a very slow recovery. You must’ve had a full tear. For all the people reading this that are planning on having the surgery: he’s mentioned in many videos that everyone’s recovery is unique to their injury and their body. He’s absolutely right. I’ve watched so many recovery videos and found very little consistency in progress as well as the feedback that people have received from their doctors. My surgeon urged me to remove the sling and start PT as soon as I could. I had partial tears in my labrum and supraspinatus. Had arthroscopic surgery 2 weeks ago. I’ve started PT and can already raise my arm over my head (with pain obviously). I couldn’t imagine using a sling for 6 weeks.

      @@Ryujinn999 My shoulder feels great. I'm just over 3 months since my surgery. The aggressive approach to PT that my surgeon recommended has been very effective. I'm back to weight lifting and shadow boxing. I'm limited in how much I can do but it feels great to be active again. I still have a bit of trouble sleeping on my side though

    Bro sounds like you have had a tough run through your recovery which is shit. I am 5 weeks now from my labrum repair surgery in my left shoulder. Fuck me mate the first two weeks was pretty painful and uncomfortable. But I am pretty happy where I am now at 5 weeks. I can lift my arm up further then you both in front and horizontal. Another thing i wanted to add was my symptoms for the past 18 months prior to surgery was similar to you. I had ongoing shoulder and neck pain on the left side. My MRI came back with larbum tear, hence why i had the labrum repair surgery. But during my surgery my surgeon seen that my bursa was seriously inflamed and red. So he went ahead and performed a Subacromial Decompression (SAD) procedure in addition to my labrum repair (which required 2 anchors). The SAD is basically where they remove the inflamed bursa entirely and if required shave some bone in the subacromial space to create more space as this is where the impingement is occurring and most likely the cause of my pain. I immediately noticed after surgery that this has fixed my impingement pain which was causing me grief in my shoulder and neck. You might want to discuss this with your surgeon. Anyways yes mate you probably should have started your physio sooner then 6 weeks... I was instructed to wear my sling for 4 weeks. But at the 3 week mark i decided to start weening myself off it as i realized it was getting stiff as fuck. Up until 3 weeks i was only doing the pendulum exercise which no doubt helped. but at 3 weeks i emailed my physio and told him i'm gonna start the exercises (passive elevation, stretches, shoulder shrugs, passive external rotation etc.. very very gently mind you) as soon as i started doing these although painful i noticed drastic improvement each day. Each physio appointment he has been giving me a good 20 minute release deep tissue massage which helps a ton also with loosening. I'm sure your having that done. anyways take home message is look into SAD procedure if you haven't had it done and also yes start and progress exercises asap. I should probably make a video also to help the ppl. I'm 5 weeks now so if i continue the way im gong im hoping to have full range of movement in the next 5 weeks (10 week mark). im keen to get back in the gym and playing basketball again. if you have any questions mate hit me up.     I was doing dumbbell incline and destroyed my left shoulder 3 years ago. Will be having this surgery done in a month or so. The left shoulder is very much unstable and just messed up. If I drag a bag of clothes my left arm and bicep become very tight. Elbow pain as well. Was this something you had, the elbow pain?

    Hey man would love to see an update. I'm almost 7 months post op and feel like I had a leap in progress around 5 months then just leveled out. I can't reach all the way up I still have scapula pain and get very fatigued in my shoulder quickly. Hope your recovery is going well.

      @@abhishekguptasarma2060 a little I'm able to reach up going forward almost all the way but if I have my hands to my sides and go outward like doing a jumping jack the shoulder locks level so I lost about 50% range of motion. I got released back to work because I didn't want to get retrained and then covid hit so last 10 months no work or gym it's sore all the time it's never gonna be the same

      @@abhishekguptasarma2060 the first two weeks is the worst. Don't go into with a bad mind set. Baby it when physical therapy tells you to but when they say start going hard follow it. You will know if you have a good physical therapist if your telling at them from pushing you to hard. I had two separate tears in mine which were fairly small but a lot of bursitis and then come to find out I pulled the cartlidge completely up off the bone in one spot so they had to reattach I am better then I was before the surgery just don't expect to be 100% again.

      ​ @Spencer Jones I actually had a problem of recurrent shoulder dislocation... but apart from that, there wasn't any issue in moving my arm or lifting heavy. I don't have any issues with pain... I have a pretty high pain tolerance(probably the reason my shoulder got so messed in the first place as many times it dislocated and I relocated it back randomly by myself) and didn't even need the pain meds they suggested to me. However, I am really worried about my range of motion and strength not getting back to normal. Even if it's not 100%, I want it as close to 100% as possible.
      After the surgery, the surgeon told me that they were surprised to see that my labrum cartilage was missing from 3 to 6 'o'clock... and he played some tricks by extending the ring before 3 'o'clock to fill that but there may be some void before 3 which he mentioned isn't required by us much. Do you happen to know anything about it?
      Initially, before the surgery, they predicted that I'll have to wear a sling for 3-4 weeks... but afterward they extended it to 6 as the damage was more than they expected. I needed only 3 anchors though. Also, my surgeon suggested to start physiotherapy at the 6th-week mark... so I am not there yet. But I will keep your suggestions in mind... thanks for your advice.

    I am also post surgery rehabilitation of 3 months.. Recovery is very hard.. I am facing one major issue that my surgical shoulder is riding up while taking shoulder up.. Uneven shoulders are there.. The shoulder is not moving smoothly, taking entire portion up

    Hi Bill! Thanks for this. I have been diagnosied with a SLAP II tear. If I can kindly ask: I have been told that I need to get the stiches (same as you). But I have also been said that (in order to prevent it from ripping apart) I need to get bicep tenodesis (this is, getting the bicep cut and pasting it back lower in the arm so it stops from pulling the shoulder). Have you heard of this? I assume from your videos you did not get the bicep cut. Could you kindly advise if you had been told this? Could you ask your doctor please? I would like to avoid the cutting of the bicep!

      Santiago García I had a tenodesis performed on my shoulder 13 weeks ago. My surgeon told me that I would never have problems with my biceps ever again after this procedure. I have not felt any pain related to that part of the procedure I had performed. The pain I am feeling is from the labral repair, and the bone spur removal and rotator cuff repair. My surgeon did a really good job of relocating my biceps tendon into my humorous bone. My biceps still look the same and feel great.

      Let me clarify that by saying that I haven’t felt any pain at the location where the biceps tendon was relocated to but I have felt a lot of pain under the deltoid where the tendon was removed from underneath it.

    I’m 14 weeks out and back to working out everyday! Gained 2 inches back to my arm in a month. Make sure to do ur PT everyday no matter what.

      Justin Ortiz I can probably throw a football like 80% but it doesn’t hurt if we are like playing catch. My PT said I’m the fastest he’s ever seen recover from a labrum tear so goodluck don’t compare ur self to anyone.

    I'm 4 weeks post op with type 2 Slap tear. The pain is constant, and doesnt feel like its getting any better. I can't sleep for longer than an hour without waking, its torture. 2 weeks left in sling. Helpful vids, how did you manage your pain?

    I’m having a surgery soon (slap tear, decompression, maybe tenodesis, etc) and what worries me is my age (40) i haven’t ask my doc how good or bad my chances are
    So my question is how old are you?
    And how long did you wait for surgery?

    I'm 13 weeks post op today and right about where your at. Scares me though at 16 weeks though your not further a long. I'm trying to figure out what the average best it is gonna get time frame. I am tired of the pain and not being able to use my right (dominant) arm. I also have been having the pain in my left shoulder for the over compensation. In just ready to get back to work I work in construction and I'm getting nervous that I'm not gonna be able to go back. That's my livelihood. Good luck to everyone going through this and would love to hesr everyone's stories and progress.

      I’m definitely getting better it’s just really slow. With returning to work that’s the hard part especially if your job is physically demanding. That’s why I said in my last video don’t jump into surgery without thinking about what’s really involved . Only time will tell. Hopefully everyone is getting some benefit from the videos and comments section

      @@abhishekguptasarma2060 I have more mobility to a certain point but I could not move my arm past level with my shoulder outwards. Going straight up infront of me I can get almost all the way there. As far as strength I'd say I've lost a lot. Carrying anything these days I can atleast power through it like my 30 pound son but eventually it gives out and I pay the price for a day or so. Covid happened so as soon as I got signed off for work they laid everyone off. So I've been out of work for about 9 months. I'm honestly scared to go back to work cause I'm no where near normal. Everyone is different though and all I can tell you is make sure you have a great surgeon and a great physical therapist. You will personally hate your physical therapist if you have a good one. They will push your shoulder to come back to as close as normal as possible. I moved right after my surgery and had to switch my physical therapist. They were to gentle on me but I also feel like my surgery was not a complete success.

    I had my surgery 4 months and a week ago and I started physical therapy at 6 weeks also. I am a baseball pitcher and have full range of motion and am able to start light tossing in 2 weeks. I am worried though because my shoulder has been getting fatigued and achey very easily. Any thoughts?

    I’m on the waiting list for total shoulder replacement on my right, dominant arm. I wrecked both shoulders by pushing too hard in the gym. Anyone out there there wanting to describe their shoulder replacement experiences?

      I had a complete rotator cuff tear, or they call it the rupture of the shoulder, it’s been two months since my surgery where they had to use Arthur Croppie I’m not sure on the word there I don’t even know what it means. I hurt my shoulder at work , I had to wait a month and a half because of Workmen’s Comp. to get an arm MRI, the doctor said my muscles were atrophy. Been going to physical therapy, the therapist said I’m not doing very well I have an appointment on Tuesday. We’ll see what the doctor says

    dude if youre having pain and limited rom 4 months post op somethings definitely wrong. immobilizing your shoulder for six weeks is not good.

      Make sure you both take your pain medication every 4 hours if you wait till you feel the pain its a night mare to chase it. When doc says don't do shit dont do shit. Good luck just prepare mentally for the recovery. Surgery was the easy part.

      Leah Lawrence that’s weird I haven’t really had pain at all. I rarely take any meds after the first two or three days. Might be from how severe our tears were initially
