    I don’t think there is a comparison between your in-office, laser therapy, and at-home microneedling. But this is apples and oranges. Most of us don’t have the ability to travel to the West Coast for your practice. And in previous videos, you’ve made it clear that some providers have dubious competence, leading us to wonder if the practitioner in our area even knows what he or she is doing.
    Sometimes the best we can do is to research what we can get done on our own at home, and for me that is microneedling.
    My Dr. Pen cost me $129 on Amazon and it is still going strong after three years of regular use. The individual cartridges cost me three dollars .
    Would I prefer to fly from Massachusetts to California several times per year for your laser treatments? Sure! But my wallet has other ideas.

    I am 45 and had microneedeling treatments quite often. Not only got my acne scars invisible, but my skin always looked fresh and way better, even weeks afterwards. Also, I never had skin loss problems. I store my collagen by supplying as well. For me, who can't afford expensive laser treatments, it's a perfect alternative. 🤷‍♀️

      Microneedling can be expensive as well. Im glad you had a positive experience and thank i for sharing. For women over 50 or post menopause unfortunately the results are underwhelming

    I’ve seen and read so many good experiences from microneedling. There are many bad laser stories too like all treatments. I wouldn’t use the RF microneedling as I wouldn’t want to risk fat loss. But regular microneedling is a yes for me.

    I was going to get microneedling! Which I desperately need for acne scars, I watched a video where you insisted microneedling was not worth it. I based my decision off your recommendation * My Fault* needless to say I elected for another procedure and was very unhappy with the results! And still have my acne scars.. I am now saving up for microneedling but I feel some type of way about people who have access to unlimited cosmetic procedures, trying to convince the general public against something that could be helpful for there skin and confidence! Keep pushing those lasers babe, I know they bring better business!

      I have done microneedling, and chemical peels for the last year 1/2, and it totally transformed my skin for the better. I totally recommend! Do your research on the doctor.

    I love all your videos! In this particular case, I happen to disagree with you. From my anecdotal experience, Microneedling has absolutely transformed my hands, thighs and stomach. I did a lot of at home Microneedling (every 4 to 6 weeks) on my stomach after my baby girl, and it truly helped tremendously with the laxity problems I had.

      This also changed my life. My stomach and inner thighs had tons of stretch marks and hanging skin. Microneedling transformed those areas.

      @@milenaandonov9291 I use a Dr. Pen for my face and a stamper for my body (stamper is faster for large surface areas and goes to 3.0 mm deep, which I do go that deep on body skin only)

      I’m African American and microneedling has been the ONLY mechanical procedure to clear my hyperpigmentation (from acne). It is also reversing a hypotrophic (indented) scar I have on my face due to shingles outbreak aftermath. I must admit I haven’t tried laser devices due to insane costs. Microneedling may work better for more melanin producing people perhaps?? Medical grade tretinoin is the OG for my pigmentation but I cycle on and off for safety so the microneedling helps me keep my skin clear in between.

    Thoughts on what Dr. Des Fernandes says about micro needling?? “The instigation of collagen from the fractionated lasers is from the death of tissue. With needling you get regeneration through the release of platelets from the bleeding. It’s the deposition of the elastin that seals the fact that this is regeneration and not simple collagen formation.” (He says there is proof on histology with the presence of elastin after micro needling and to my understanding that lasers do not feature this effect)
    Personally normal plain Jane (no rf) micro needling has been amazing for me. The thick, pitted stretch marks I had from puberty have drastically reduced. There is no more pitting, the skin is smooth and with more sessions I think I can get that skin to look nicer than the rest of the skin on my body 😂 I do micro needling safely at home. I live in a small town and don’t have good providers around for lasers. The only issue I had was when I did get some small areas of hyperpigmentation when I micro needled then put .01 tret on directly after. I wonder how many people had issues with mn due to application of other skin care products too soon after. Never got the hyperpigmentation when mn then waiting a little more than a week (about when the itching that occurs on 5th/6th day settles)to apply tret. But tret, on its own, and mn on its own has nevee caused me hp. I go the max I can on the derminator which is 2.5mm. I also use it on my face around 1mm and never got hyperpigmentation there (I’m also a darker skin tone) bc I made sure not to stack tret and mn. My mother who is nearing 60 has had great results from micro needling as well. She uses it on her face. Just sharing my two cents. Appreciate your video and your experiences, all info is valuable!!!❤

    100% agree. I had RF microneedling a few times and then the last time I had trauma and it triggered SHINGLES! My dermatologist said this can happen, but not common, especially because I was so young. I now have scars on my face because of it. It was one of the most traumatic experiences and I still have nerve damage in my face from it. Sooo not worth the risk!

      @leslie I actually didn’t know about the shingles risk prior to my appt but it was on all the pre op/ post op paperwork she gave me when I did a consult. She said they require that and also give a short dose of prednisone for the swelling. I have healed very well! I am sorry you experienced so much damage, esp nerve damage. That is scary!

    I’ve done several micro needling in the past and honestly have had several different reactions to the process.
    I have also seen medi spas take advantage of us consumers that go to them for a service and end up looking botched more than anything!
    We need more people like you in this industry!

    From someone with darker skin who is extremely subject to hyperpigmentation, I LOVE RF Micro-needling (Virtue RF). I combine mine with PRP and only need to go twice a year. It’s a procedure that I would recommend to any darker skin woman. My skin is tighter, my dark spots are gone. I love it.

    Dr Kappel, I take the time to look at your videos, not because I have the time, but because I trust your opinions. I have been using medical grade skincare consistently for many years and my 64 years old Afro Caribbean skin is impeccable...has absolutely nothing to do with 'black don't crack'. I have been having laser treatments from my dermatogist based on your recommendations, and I love the results. Recently, she suggested we try micro-needling, and I also love the results. My face is line-less, poreless. So I understand your caution based on your extensive professional experience, but perhaps the possible microneedling related complications could be as a result of the provider, and not necessaryily the efficacy of that mode of treatment.

    I totally agree with you. I have been micro needling with PRP from the time I was 45 and I am 53 now and it has literally tightened and plumbed my skin. I have never had Botox and it helped even get rid of forehead, wrinkles and I wrinkles and like you it is one treatment that I will do quarterly forever.

    I have an at home micro needling pen (Dr. Pen) and I find that it is the only affordable way to do fully body treatments. It would be incredibly expensive to get fraxel or another in-office device over your entire body. Do you think it is alright to use on your body? Are there any other effective full body treatments?

    Hi, I’m so glad you did this video, last year I did 3 sessions of Morpheus 8, and I regretted so much, I was 44 years old now I’m 45, my face it’s worst I did it because I started to see sagging skin in my lower face, and started to notice marionette lines.
    But your right about marketing it’s pretty extensive, even the kardasian’s talk about how wonderful and amazing Morpheus 8 was, so I did it and I regretted so much, my skin changes a lot but for worse, before I used to have good texture my only problem was skin tightening concerns, now I have sagging skin even worse, and texture problems, and also it was not cheap. That’s the reason a create a video in Spanish because it’s my first language and I want to share my honest experience, because nobody said the real true it’s just money for everybody.
    So congratulations for making this kind of videos, thank you for your honesty, I would love to have you in my city. ❤️

    Excellent video, as usual ❤. There is nothing like Dr. Stefani's scientific point of view. When I first heard about microneedling, it sounded good to me, logical. Something about the technique seems right to me. Then I started looking for information about it and even then they warned of infections and scars, so I never tried it. This video only reinforces my gut feeling to stay away from it

    I did microneedling with RF years ago and it actually gave my skin better texture so I was pleased with it. I did 3 treatments. I was in my late 30s. I’m in my 50s now and I do have good skin (some is genetics). I won’t do it again because for me Clear and Brilliant gives me the same results after one treatment. I did Fraxel (series) years ago with good results as well. I did microneedling at a cosmetic dermatologist office and the only thing I didn’t like was before I saw results I had to wait until I finished the series. I won’t do it again though. I really love the Clear and Brilliant treatments so I’ll stick to that. I’m wondering if the provider doing microneedling makes a difference? Anyway, that’s for your honesty! ❤

      Thank u soft sharing this! Clear + Brilliant and Fraxel laser are both amazing. It is a red flag that your provider wouldn’t let u see your before and after until you finished the series. That was insightful that u picked up on that. Also most people who respond to microneedling are younger so the fact that you were in your 30s when u had a good response makes sense.

    Hi, my doctor in Singapore has recommended me to go for sylfrim x + pico laser to treat my melsama. I tired 3 sessions, and seeing slight improvement. Shall I continue?

    I microneedle at home with hyaluronic acid and make sure to be really careful and sanitary. It helps my very oily forehead not be so oily and therefore I'm not producing sebaceous hyperplasia as fast. I find my skin looks awesome about 2 weeks after microneedling and results lasts for a month and half-ish. I appreciate your videos. Makes sense that it's an easy procedure so med spas are all about it. I will consider some alternatives like laser and better skincare options like vitamin c and peptides :)

    Hi Dr. Kappel, I've been following you for awhile now. I did the Cutera Secret RF Microneedling (1 treatment) at a "reputable" office. It left me with red pinpoint gridmarks on the sides of my face permanently, it's been over 3 years since I did this treatment. I've gone to a few different offices to guide me in the direction to remove them and everyone has recommended different devices and treatments. Would you have any recommendations?

      I'm pretty sure she was referring to MN in general but yes she also looped in RF and PRP at the end. I thought that MN with PRP was somewhat successful but I can see that some people may get a reaction to MN (hyperpigmentation) but I've done it a few times and not seen that and also have a Qure system at home but only used it a coupe of times. In general I dont see much results from MN but I can see how it could help based on so many people's results, even from the ppl in the comments area on this video.

      I'm curious, what kind of scarring did you get? I did an RF microneedling treatment once and it left red pinpoint grid marks on the sides of my face.

    Thanks for going deep on this tech and why to steer clear. I'm curious how microneedling is different than Ellacor? You touch on it a bit, but I'd love to hear more on how that tech doesn't cause the same issues as microneedling.

      Microneedling is different than microcoring aka Ellacor. I’ll be doing a video on Ellacor soon as we are having great results with this device in my office. Ellacor removes cores of skin vs microneedling doesn’t remove any skin at all. Ellacor is a type of skin excision to tighten skin. There is also no heat or RF involved.

      @@DrStefaniKappel It reminds me a lot of aerating a lawn. They take plugs of grass out. This is a treatment I am actually very interested in. If it isn't outrageously priced it might be my very first procedure ever. I need it all around my jawline/lower face :/

    Hi Stephanie in your experience have you seen Thermage tighten lower eyelids skin? I am debating about getting full face Thermafrax or a blepharoplasty. I would like to avoid a blepharoplasty if possible. I do not have significant eye bags.
    As I am not a patient, I know you cannot comment about my specific scenario, but can you answer about your experience with thermafrax tightening, lower eyelid skin?

      I think the best teatments that are non-surgical for lower eyelids include Fraxel, ThermaFrax, CO2, Thermage and then blepharoplasty. But nothing compares to the results of a bleph if you are the right candidate.

    Thank you for your info and time! What laser would you recommend for patient's with melasma? Unfortunately, there are no providers near me with a Clear and Brilliant Permea laser, which I know you have recommended in the past. Also we do not have any dermatologists that perform the laser procedure, they delegate them all to estheticians or nurses. I have had good luck with microneedling and Moxi was ok. Fraxel caused PIH and was awful.

    Good morning! I wish I would’ve seen this a few years ago before wasting money on micro needling. Not only did it not do anything, but it caused hyper pigmentation.
    My question to you is, what about micro channeling? Do you feel it is just micro needling witha new name?

    Hey Dr Kappel. Does pico help with wrinkle prevention and box scar revision? I'm Fitzpatrick V and live in Sweden where there are very few darker patients and I feel scared to do Co2 or fraxel

      It can yes, but fraxel is much better for acne scar revision. Pico is an option but pico is better at treating hyperpigmentation. Pre and post skin conditioning with fraxel would most likely be a better option to get the best result while minimizing the risk of pih. but you’d have to be very compliant with the aftercare

    Hi Dr. Kappel! Are you familiar with Thermitight? Before committing I wanted to get your thoughts on this procedure. I know 1 person who's had this done under the chin and neck area 8 years ago along with some lipo as well and the results still look amazing after all this time.

    Hi, i have a hypertropic scar from removal of a burn scar, its still super fibrous from the inside of the skin and quite visible, mostly pale but reflective. would you recommend scar revision?
    i've tried co2 and microneedling but it really seems like a moneypit, i think co2 and microneedling might be more effective after the scar revision

      It really depends on the scar. But both can increase drug delivery and be use with kenalog, 5FU and other immune modulators for best results.

    Thanks for this. Wondering if you've changed your position on red light therapy. BTW, I get microneedling regularly and have had no ill effects that I can see. It seems to help. But I do so much for my skin it's hard to tell what's working.

    Ok this makes sense, and it actually answers some questions for me! I had RF microneedling on my lower thighs right above me knees at my dermatologist office. I was led to believe it would help with a little bit of laxity. I lift weights 5 days a week, but it was bothering me. I wish I had never done it because now it’s really noticeable to me. Soon after it actually looked worse and I suspected it was the RF, but now I’m almost positive. It makes me upset that this is a treatment that can possibly do the opposite of what we all want:( is there anything I can do to reverse the damage?

    What do you think of Morpheus 8 to dissolve fat in the marionette area? The pico is phenomenal. More expensive but I’ve never had an ablative laser bc of sensitive skin. My skin looks amazing.

    I have genetic loose neck skin and now in my thirties, want to get rid of it. I did a halo but that didn’t help at all. Is surgery my only option if it is genetic? It has been since childhood so I dont think it is collagen loss although it has worsened over the past few years. Just dont wanna keep wasting money on non invasive treatments if surgery is actually needed. I trust your opinion more than anyone at the medi spa!

      The micro needling increases drug delivery of the minoxidil but as far as antiaging effects there is no sound purely scientific data (ie not sponsored by the companies who manufacture the device being investigated) that demonstrate that micro needling is effective for antiaging.

    Dear Dr. Stefani, this video is so so timely, last night (before this video was posted) I microneedled my face for the last time. Something inside me felt that if I continued I would do irreversible damage to my face. I have watched my skin slowly thin and the areas I was hoping to improve worsen. I am a chemistry major at the University of Washington (but mom age) and do not have a lot money, but decided to return three boxes of microneedling cartridge and save up to buy MDAiRE products as I can. I only hope I am able to correct the damage I caused. Also, this comment is not meant to shame anyone who gets good results from needling, for me personally, it is not helping, but harming.

      I’ve been microneedling since October 2023 and was following the advice given by Master Esthetician and CZcamsr PennSmith. Since October I have increased hyperpigmentation, pop marks, pore dilation, spider veins, skin laxity - all of it. In fact, I’m kind of scared I won’t be able to fix the damage I caused till I graduate after of my masters and can afford dermatological treatment from a qualified provider. I’m still an undergrad, so it’s gonna be a while before I have the $$$ to do that. I am really trying to not freak out about it, but it’s hard. I am going to look into possible treatment at my university’s medical school dermatology department, maybe I can at least get a consultation and some advice. Also, like I said in my original comment, if needling works for you that’s awesome - seriously. It really is the golden ticket if it works for you. I sincerely want all humans to feel beautiful and comfortable in their own skin, which is all I wanted for myself.

    Omy, I've watched so many of your videos in the last week, i was pretty much settled on that morpheus8 device, i thought i was understanding all the different devices, but now i feel lost. I watched other videos of people, but yours i can understand. There's just so many options to choose from. So the first thing i would like to have treated is my eye bags. I'm 59, i went to Dr. Black to get a consultation on upper and lower eyelid blepharoplasty. I thought insurance would cover the upper lids, but he said they weren't drooping enough. Then he tried to talk me into a forehead lift. I left there very disappointed, and then your videos started popping up in my youtube feed. I love that you are more about laser and not so much about injections. I don't want any injections. So, what can you lead me to to help my under eyelid bags? And possibly upper lids too. My next thing id like to work on is my jowls that came on out of nowhere.

      I feel that aesthetics has gotten so out of control with respect to people pushing treatments and marketing left and right that it is very confusing and overwhelming to the consumer. It’s more important now more than ever to do your research and shop around to find the right provider for you. As far as eye lid tightening, CO2, fraxel and thermage are my favorite. But you need to find a provider who is comfortable treating around the eye and who can put in an eyeshield as many providers are not.

      It's tough to find any providers like you in Michigan. U of M has a medical cosmetic department, but I'm not sure how educated they are in different laser devices. Also, Dr. Michael Gray in West Bloomfield Township seems to have a huge reputation, but I'll have to look at reviews, which have a 3.9 rating. Other than that, it seems like most dermatologists near me push more injectables, and some laser treatments. Thank you.

    Unrelated but is it normal that I healed from my 3rd erbium laser quicker than my 1st two sessions?

      Sometimes as your skin gets healthier with each laser treatment in a series you will have a faster recovery. Either that or the settings may have been a little weaker. Was it the same provider each time? Erbium is an incredible laser. Im sure your skin is looking amazing!

    I did shallow microneedle treatments (12 total) for over 2 yrs and didn’t see a difference on my skin (47 yrs old then). I did 3 different laser treatments (CO2, Picosure & VBeam) last year and my skin had an amazing response to those treatments. My skin looks smoother, tighter and radiant now. My pores are so tiny now, love lasers!

    I had RF microneedling done a few years ago and it was not only worthless in producing any results, but caused a major breakout and inflammation that took about a month to resolve. That was my last visit to a medi spa!!

    I agree 100% with this post. I had 6 sessions of microneedling in the best doctors office in my town. Performed by a doctor, despite this I got horrible hyperpigmentation and small scars on my cheeks. I have done several fraxel dual and picolaser treatments to reverse this. Now 3 years later it finally looks better. A very expensive lesson!

      It’s basically glycolic acid, TCA and sal acid. I think as a chemical peel it can be very effective but the after care in skin of color must be managed properly to reduce pih. but overall I think it’s a solid peel. I don’t have it on my office but we have other chemical peels that work really well too :)

      It will soften their appearance. it depends on how deep the neck lines are. It has topical argireline in it which is like botox on a bottle
