VITAMIN B12 dosing strategies - 3 examples (and what I recommend)

  • Why is there such a range of doses in B12 supplements? Why do they often provide tens of thousands of percent of the RDA? Is this safe? Why is it so hard to find more conservatively dosed supplements? How much should you take?
    And do you really need to take thousands of micrograms of the vitamin to get enough?
    It is important to remember that the pros of ensuring a good B12 status far outweigh the potential negatives associated with how we must dose it to achieve this status. That being said, there may now be value in using a little more finesse in our dosing strategy and determining a dose that will provide enough B12 without ingesting thousands of micrograms unnecessarily.
    Today we'll be exploring the relevant aspects of B12 absorption and how you can use them as a starting point to work out a conservative dosing strategy that still has a high probability of achieving adequacy, including three examples of regimens to make this topic a little easier to understand.
    Please note that the strategies discussed in this video apply to healthy people with no known absorption issues. High-dose B12 supplements exist for a reason and are very effective for acquiring and maintaining a good status in some people with B12 problems, which are very common. In this case, the benefits of high-dose B12 supplementation far outweigh any potential drawbacks and you should persist with the regimen prescribed by your healthcare provider.

      Nooch is a very handy source, I use at least a small amount with most things I cook, I love it. :) That seems like a sensible B12 regimen, do you get your levels tested to make sure you're getting enough? Make sure you take the supplement 4-6 hours apart from any B12 fortified foods!

    Thank you for sharing this information but i have one suggestion, when we say daily requirement for b12 is 2.4 mcg that means it is not actually 2.4 it is 1.2 actually and we assume 50% efficiency thats why daily requirement is 2.4 but when we calculate total amount absorbed that means we need not to divide it by 2 hence actually we need 1.2 mcg, if this is the absorbed amount.

    Does taking B12 empty stomach/after food effect the rate of absorption through passive & specific diffusion in any manner?
    And what should be ideal Weekly/Daily dose for maintenance of other Vitamins B like Folic Acid B9, Niacin B3, as they are generally given and packed along in the B12 supplements, which are also required daily as are not expected to be stored in liver?

    • @DrDesHarrington
      @DrDesHarrington  Před 2 lety +3

      Hello! B12 absorption isn't as dependent on prandial state as other nutrients - whenever you will remember to take it is the best time! Unfortunately I haven't looked much into these issues for other B vitamins since they're not nutrients of concern on a WFPB diet. My preference would be to find a supplement that is just B12 and without these other B vitamins unless you have a specific reason for taking them, too. I am a supplement minimalist - I prefer the targeted approach rather than just taking multivitamin supplements, which have conflicting benefit at best as per the science!

    Thank you for this video. Firstly, I have the brand Nature Provides which was recommended to me via a GP because my MCV levels were out of whack. Cyno B12 wasn't working it would seem on it's own. Cynoc at 50 mcg a day which seems very low. So I started taking Nature Provides daily at 3000mcg for 6 weeks and my B12 levels shot back up into the middle of the reference range.
    But my question is this. I have been taking the sublingual B12 which is methyl and adeno by Nature Povides" is 3000 mcg. So is this too much a day? I did make the dropper reduced to 1000 mcg (just by judging it by eye I might add) and taking that every x days but is that too much too. My teenage kids taking 1000 mcg roughly x2 a week but that's with me judging the Nature Provides drops by eye.
    We also take fortified foods in milks and nutritional yeast. But that's it for fortified.
    Can you help?

      @DrDesHarrington  Před 3 měsíci

      The specific B12 absorption mechanisms are vulnerable to failure for many reasons, so in your case you might need the megadose to take advantage of passive absorption. If you need that to maintain an adequate functional level then just do that - the health complications of B12 deficiency far outweigh the increased cancer risk associated with megadosing.

    If you dose several times per day, would these ingredients not have any effect on your health? (this is from the b12 supp of 100 mcg you've shown) - microcrystalline cellulose, dicalcium phosphate, vegetable cellulose, vegetable stearic acid, silica, vegetable magnesium stearate.

    • @DrDesHarrington
      @DrDesHarrington  Před 5 lety +7

      I certainly wouldn't take that particular supplement multiple times a day, once is definitely enough. There are no B12 supplements intended for multiple daily dosage that I know of. It's okay to take less frequently than the intended use, but I'd always be cautious about taking anything more frequently.
      The only realistic way of doing the multiple daily dosing approach at the moment is with fortified foods! Despite the inconvenience, I'm actually more in favour of this approach now than before, hopefully as more people go vegan more and more foods will be fortified making this strategy easier. :)

  • @PetrilloCinema

    What study refers to increased cancer risk due to excessive B12 intake? I saw one on WebMD that really isn't convincing. It found that people over-supplementing with folate and B12 got lung cancer, but all those patients had smoked in their lives. So I gather nothing from that.
    I'm really wondering if my body is more efficient on the day to day if I ingest B12 daily as opposed to once a week. I would think that if I take it once a week, the B12 I ingest on day 1 runs out by the end of day 2 (cause it's water soluble) and then I have to draw on my liver stores. Then it's a question of whether having to draw on the liver stores will eventually deplete the liver stores, or if there's enough restocking happening because of the copious amounts of B12 I'm ingesting on day 1 (assuming 2000 mcg). Please help

It looks like the one you have a photo of might be Solgar brand by the packaging, but you don't show the label so I can't tell for sure. When I do a search for 100mcg cyanocobalamin, I get just one hit on Amazon from a brand called "Major" which I've never heard of before and has zero ratings. As we learn more about the potential negatives of high-dose B12 and they are confirmed a market force for lower doses will emerge, but we're stuck in this situation for now. Even better would be if you could get a test that reflects B12 _functional status_ and not just serum level - holoTC or MMA - but unfortunately these tests are not widely available! My leukopenia on the other hand is probably responsible for not clearing my HPV infection despite having it for over 2 years at this point.

      @DrDesHarrington  Před 2 lety +2

      Hello Nikhil, you are looking at repletion here rather than maintenance, so the concerns outlined in this video fade into insignificance until you can get your level back into a healthy range. Assuming you do not have any issues with absorption that have lead to this low level, you should take a daily oral dose of around 1 mg for a few months and check your level again. Once you're back in the healthy range you can go with one of the maintenance strategies outlined in this video.

    What I do is blend a 100 mcg pill in a smoothie and take a few sips every 6 hours. Do you think this a reliable method? How long will the B12 last if the pill is mixed in food?
    You could theoretically use only one 100 mcg pill per week with this if you blend it with a food that you can separate in 21 portions (3 x 7 days) getting 4.76 mcg each time.

    • @DrDesHarrington
      @DrDesHarrington  Před 5 lety +5

      If it is cyanocobalamin is would probably last a long time since this in one of the forms that is used to fortify food, it would vary depending on the excipients though and the only way you would know for sure for a particular product is to contact the manufacturer.
      Yes, that could certainly be a viable strategy for getting enough provided your specific absorption is working normally. I would definitely combine it with the occasional blood test though to make sure you're getting enough!

      It would only need to be a tiny dose, in the order of 1 mcg. As you say though, B12-only supplements with this dose don't exist! B12 is sometimes dosed in this range in multivitamin supplements, but they're not usually taken multiple times per day, either. Actually, the study I referenced linking high-dose B12 with cancer did not find that effect with B12 from multivitamin supplements.
      The only real way of doing multiple daily doses of B12 is with fortified foods - they usually contain around 1 mcg of B12 per serving. This is why I'm actually leaning more towards this strategy now, despite the inconvenience. Hopefully as more people go vegan, more (and healthier) foods will be fortified with B12 and all of this won't be as much of an issue! :)

    Just had to have B12 shots.
    Not everyone absorbs at the same level.
    In addition to the shots I'm temporarily taking 1000micrograms per day for 12 weeks until my next blood test. If all is nearing normal I will continue with at least 300per day to see how that maintains.
    It was quite a shock to see how low my level was.

      Hello Robert, yes megadosing B12 like this has become standard practice since it was thought there was no negative to doing so (and a lot of positives in the case of quite common absorption issues), but as I describe in these B12 videos this thinking might be about to change. Either way, I think it is sensible, not too inconvenient and very low-risk to try and devise a supplementation strategy that fits your lifestyle that also exposes you to the least amount of B12 as possible.
      Most B12 supplements would be fine for dividing into smaller doses, yes - the main reasons why you should not do this are if the preparation is extended-release, must bypass the stomach or requires more intensive preservation than usual, none of which is required by B12.

      @@RobertSmith-rs1cz I don't think supplementation should be handled with broad brush strokes like this. A WFPB diet can be a great source of all B vitamins except B12, and you should always aim to get your nutrition through whole plant foods. If you have plugged your regular diet into Cronometer, found a nutrient you consistently don't get enough of, and cannot make the appropriate dietary changes to support enough intake, then you can consider supplementation. NOT taking high doses isn't particularly safe either, if you're deficient. I had all kinds of horrible problems until I started taking the high doses. My vision was all messed up and I was heavily depressed among other things. Gotta watch out for the additives and fillers in those supplements too, the companies producing them are not on our side either, as far as I can tell. My solgar supplement contains Mannitol, which seems to be make me feel rather dizzy. Why the hell is it in there? Other supplements don't have it. Trying to find the right one is a bit of a minefield. Also tried the Best naturals one, but appears to be less effective. Gonna try Jarrow for a bit and see how it goes.

    • @DrDesHarrington

      Absolutely, once the goal is repletion rather than maintenance the considerations are completely different. The consequences of under-treating deficiency are far greater than the comparatively small incremental risk in going from a low dose to a high dose. I strongly recommend you search 'Sally Pachalok B12', as she knows the medical details of it more deeply than I. I've seen multiple doctors about my B12 issue an none of them had a damned clue and chose to patronize and gaslight me instead of help, so now I handle it myself. given a choice between potential brain and nerve damage (significant evidence of this in other members of my own family), or taking matters into my own hands, is there even really a choice? Again, there are no recorded damaging side effects in medical literature releated with b12 overdose - it's a cheap, readily available and non-toxic vitamin, much in the same way Vitamin C is. I've so far experienced only positive benefits from the B12.

I also get confused and clumsy and start dropping things, having little accidents, bumping into things etc. Thankfully, I've learned to recognize these signs and supplement immediately when I see them.
      Regards my protocol, I'm currently taking Solgar Methylcobalamin 1000mg sublingual, B6 100mg, Folate 1000mcg and Igennus Super B-Complex to make sure I'm getting all of the co-factors. I'm also self-injecting weekly Hydroxycobalamin which I've been lucky enough to source from a supplier here in the UK (they clearly get theirs from Spain as it's a Spanish bottle). I tried injectable Cyanocobalamin from a pharmacy in Germany for a while, injecting into belly fat though never managed to achieve the same effect I got from the Hydroxycobalamin I received as part of the NHS protocol 3 years ago. Apparently the Cyano form is not as good as the body doesn't get access to it right away, and also we pee out a lot of it and it doesn't get used.
      Obviously self-injection is something you don't want to half-ass but again I haven't really been given much of a choice. I am super careful and have read multiple medical documents explaining how to do it. I'm also fortunate in that I'm fluent in Spanish and can watch numerous videos in that language on CZcams of Doctors explaining how to do it safely. In combining the weekly hydroxy injections with the Solgar supplements, I'm starting to see my vision clearing up again. This seems to be corroborated by Sally (Pachalok's) B12's experience as she injects and takes the sublingual in between.

Unfortunately the clinics in UK are all trying to take advantage of the lack of availability and think its ok to charge sky high prices. Regards dosage, aside from the B12 I’m aiming for what’s called a clinically effective dose, in the best, cleanest, most easily absorbable form. I selected the supplements above because they use the most easily absorbable forms of each vitamin for the body, and they aren’t full of extra unnecessary filler ingredients and allergens (yikes!) and the dose is big enough to make a difference (most multivitamins are a waste of money as they don’t cover all three points).
      Some people can treat the deficiency sublingually; it depends on why they’re deficient to begin with. It could be genetic (MTHFR), malabsorption due to a digestive issue (gluten intolerence, coeliac disease etc) or something else. I’ve found it takes some experimentation to find what works best. In my case, I’m assuming malabsorption and possibly MTHFR but it’s impossible to be 100% sure and my docs won’t even carry out the correct tests, even though I offered to pay and found a clinic to process it. So I go with this: if it works, it works. The end result is the most important thing.
      I wouldn’t worry about side effects. There will be some, because your body has to repair the damage done due to the deficiency. I get tingly nerves, tingly brain, sometimes I feel tired and occasionally confused, so be prepared to rest after a dose if needed. Look up ‘cerebral atrophy b12’ and ‘demyelination b12’ on google/google images and you’ll start to understand that the side effects following b12 therapy are a sign things are getting better. You know deficiency also causes some kind of optic nerve atrophy preventing the eyes from communicating effectively with the brain. I’ve experienced sore eyes this past week as a result of taking a higher dose but my vision has improved every day, so I know it’s working.

      I have 500mcg tablets, your math tells me it is one every 2 days
      Actually according to my math, it's every 2.7 days, which goes to 2 days without risk of low intake

      There is no justification for eating meat to get B12 when there are perfectly viable supplemental options, and B12 deficiency is absolutely epidemic in lifelong meat-eaters.

    I don't get what's wrong with supplementing 2000mcg a week, if only 20mcg is absorbed (meaning the rest passes out of your gut as poop) and most of the 20mcg is used up over the week 🤔

      I suppose all the excess could still have effects before we eliminate it, even if not absorbed into the blood! Intestinal microbiome springs to mind!
