Vitamin B12 FORM - Cyanocobalamin safe? Methylcobalamin adequate? (What I recommend)

  • In addition to the huge variation in dose of B12 supplements, there are at least 4 different forms of the vitamin to choose from. The most common of these are cyanocobalamin and methylcobalamin.
    But which of these is better? And why do some people argue that you should never take cyanocobalamin and that it is tantamount to poison?
    Indeed, why is there poison - cyanide - in cyanocobalamin at all? Should we be concerned that this is an unnatural form of the vitamin?
    Today we'll be exploring the different forms of B12 with a special focus on cyano- and methylcobalamin given their ubiquity, including manufacture, safety and conversion issues - and how I arrived at my recommendation for the form you should take.
    Please note that the discussion in this video applies to healthy people with no known issues with B12 utilisation, and you should persist with the regimen prescribed by your health care provider.

      @@lorah3005 Glad you liked it! :D I've seen it, not sure I'd call it a scam now. I agree with most of what he says - we absolutely can (and should) get all our omega-3 LC PUFAs from ALA by getting all the necessary lifestyle factors in place. Unfortunately some (probably most) vegans are not anything like this health-conscious, so for that group pre-formed PUFAs are a good idea, and certainly the pros would outweigh the cons in this case I think.
      If it means anything, I'm going to keep taking golden algae EPA/DHA myself, despite having the necessary lifestyle factors for good conversion most of the time. I'll accept a (possible) increased cancer risk if it means I am (likely) helping to maximise brain protection. Supplements are by their nature unnatural, and there will always be trade-offs when taking them! I also recommend a conservative EPA/DHA dose, which should help, too.

Was she sick because of dosage , or because she took it after bong , or because she has no intrinsic factor. She’s taken Multi Vitamins before. She said it made her feel really sick. I take half in am and half at night. I bought all my friends this for Christmas because it’s the best quality and it was the most expensive. B12 . So now she is quitting smoking, cigarettes and terrified of B12 , so she won’t take it. I’m scared because it’s legal here ,only 26 and her lungs are COPD / Emphysema sounding. Why would she throw up so bad ! ?
      It says lab tested , it expires in 2022 , and GMO. Free, . Would a B12 shot make her feel sick as well. I bought this at National Nutrition and I feel better on it .

    Methylcobalamin is better because its an active form that is safer on the kidneys and your body doesn't have to work as hard on converting, btw this man says he suggests cyanocobalamin because it converts to both methyl and adenosylcobalamin but taking methylcobalamin can also convert to adenosylcobalamin and able to convert back to methylcobalamin , this dr said you need to take adenosylcobalamin with methyl but thats not needed as methyl alone converts to adenosyl...and can convert back to methyl

      Yes. There's something about this presentation that doesn't ring true. Now, I'll do some real research into the different forms and their bio availability. I've studied B12 for a few weeks now, and never heard anyone (including Mds) suggestion the cyano version.

    After buying a B12 oral spray i watched a video saying don't take Cyanocobalamin as it is toxic but rather take the Methylcobalamine 🙄 I'm glad i dug deeper and found this information instead of throwing it out, cheers

    I have found 5000 mcg sublingual and the doctor told me to take this amount every day..... is this safe? On the bottle, it says.. (As Cyanocobalamin) so I'm not sure about the safety.. Thanks.

      If your doctor is involved I am assuming you are trying to correct deficiency, so yes in this case the benefit of a megadose like that greatly outweighs any risk added.

    I have chosen for a dose of 1000mcg cyanocobalamin 2 times a week as my regimen for B12. :)

Yes that regimen could work, just make sure you make the twice a week a habit and maybe get your level checked in around a year or so to make sure this is enough for your unique physiology, and you should be all set!

    Methyl, adenosyl, and hydroxo I take. As someone with the MTHFR polymorphism "remethylation disorder" (and 40% of the population has this) I wish you had addressed this more in relation to the active vs cyano forms. I understood that cyano can increase the blood test level results for B12 but is relatively metabolically inactive for the MTHFR polymorphic. Hence for people with the MTHFR 1 SNiP (polymorphism) there is a 40% methylation efficiency reduction and 2 SNiP variants are a 70% methylation efficiency reduction. So based on my previous research and as I understand it if one was taking cyano and your blood levels were 1000 then it would mean that your effective levels are 40-70% less, i.e. 300-600. If I am wrong then please correct my understanding. So while the use of cyano is due to stability, the deployment over an entire population is problematic for 40% of the population. It is my understanding that these decisions to include cyano as mass fortification were made before it was understood what the nature and extent of the MTHFR polymorphism issues were. Just like synthetic folic acid being not found in nature and possibly contributing to cancer this issue should be reexamined from a public health standpoint but it is not. While one can eat plant sources of folate this is not possible for B12 in the modern world. Cyano may be the best option for the 60% but what about the 40% minority?       Yes, I came across this article and others like it when researching this video, and I was originally planning to address MTHFR and B12, but the topic is just too broad and really does require it's own video - that's why I mentioned at the end that anyone with remethylation disorders should be under the care of a doctor for this, including their B12 regimen. You can't make a blanket B12 recommendation for MTHFR polymorphisms, because there are different kinds.
      As well as that, that article provides recommendations for remethylation disorders proper, including actual MTHFR deficiency, not just MTHFR polymorphisms. Recommendations for specific B12 forms of course are going to be a lot stronger for actual MTHFR deficiency - in this case, they are recommending parenteral hydroxocobalamin. I'm not convinced at present that those with polymorphisms need to give any special consideration to B12, but as I say, I haven't anything like fully looked into it yet!

However, in the 21st Century the real jobs of doctors should be to help bring these advances to patients as soon as actionable and not to just apply routine standards of care, which more and more will be taken over by ML/AI and lower level nursing. With your curiosity you represent the next generation of medicine delivering real value added from research.

    I am trying to locate a source of injectable methylcobalamin and all I can find is cyancobalomin. Why? When 1000 mcg of cyanocobalamin is converted to methylcobalamin at only 1.8 percent? WTF?

      Usually the injectable forms are hydroxocobalamin or cyanocobalamin. Just curious, why are you looking for a B12 injection yourself? Usually this would be prescribed and sourced by your healthcare provider to treat severe B12 deficiency or in the case of malabsorption.

    I'm very confused after watching multiple videos, some say cyanocobalamin is good and all the bad stuff i hear about it is not that bad and it's most stable, then some say it's the worst and i should stay away from it. Then some say the methyl one is much better and safer and others say it's unstable and i should take the cheaper cyanocobalamin

      From my reading I haven't seen anything to suggest that we should stay away from cyanocobalamin, but plenty to suggest that methylcobalamin on its own is inadequate. But I agree, nutrition can be very confusing!

    I have the MTHFR mutation C677T (Homozygous) and can only methylate 10-30% of the Folate and B12 that I consume, unless they are in their premethylated forms (L-Methylfolate and Methylcobalamin).

      This is a myth perpetuated by those wishing to sell supplements. The MTHFR enzyme is not involved in the conversion of cyanocobalamin to the active forms and, as stated in this video, methyl and adenosylcobalamin supplements aren’t ‘active’ anyway - they need to be reconverted by the body.

      Hello! This isn't especially important for B12, though I'd probably favour with food just because intuitively it seems a bit more physiological. :) The best time to take it is when you'll remember, especially if taking it infrequently like every week or so!

      There is no synthetic one, cyanocobalamin is produced by bacteria as well so it is an natural process and not synthetic.

    I am vegetarian for over 20 years, went and had a b12 shot 5yrs ago and almost died!
    I now suffer from pernicious anemia, Hashimoto's thyroiditis, and peripheral neuropathy. I believe I could not absorb the shot and my thyroid shut down and my body almost shut down completely.

      Thanks, man! :D I have a few doses left of a 1000 μg cyanocobalamin supplement, so I'm just going to take a quarter of this every two days until I can get something with a lower dose. If you really want some methylcobalamin in your life, alternating daily would be a good strategy!

    You mentioned in the video that other forms have their place such as for pernicius anemia.... I have pernicious anemia and have just purchased cyanocobalamin. My understanding is that since it is sublingual it bypasses the digestive system which renders the gastrtitis and lack of intrinsic factor irrelevant. Am I correct? I tried methlycobalmin for about 2 months and it helped in the beginning, but i noticed some symptoms returning and switched to cyan upon learning that cyan converts to all the needed forms. Again, am i correct about the pernicious anemia and should i continue to take my sublingual cyanocobalamin?

    Hello, I have just stumbled upon this video, and what a great channel. I love the strict evidence based approach without any additional assumptions or extrapolations.
    I'm on a WFPB diet and have been taking this sublingual B12 supplement composed of 1000mcg (900 cyano + 100 methyl) twice a week. I guess there shouldn't be a problem with it being a mix, have you found anything about it during your research?
    And about the dosage, after seeing your video about the risks of megadoses, am I correct in assuming that whether I take the two 1000mcg pills in one day or one pill every 3/4 days has no effect on the risks, but only on the actual amount absorbed?
    Would one of the two methods be preferrable over the other?

      Hi Max, thanks very much, yes I try to keep opinion out of it! No, there will be no problem whatsoever with that mix of B12 forms. My preference is to make your doses as frequent as possible so you can take less and expose yourself to the least amount of B12 overall whilst still ensuring adequacy as outlined in this video:

      Yes, the total amount ingested is what has been linked with the negative health effects, not the amount absorbed, so the two examples you give would be equivalent in this regard. Both should also work to maintain a healthy level in someone with no absorption issues.
      I will be returning to B12 at some stage with an update on the latest research linking high-dose B12 with negative health effects, but on my cursory reading so far they have not been definitively proven nor disproven yet unfortunately!

      @@DrDesHarrington Alright thank you for the quick answer. I will keep my eyes open for lower dose supplements for when I finish my stock, but they are quite hard to find, and often inexplicably more expensive per tablet compared to the standard 1000mcg ones.
      In the meantime I have watched your series on omegas, very interesting!
      Do you mind a question about those?
      I see many omega supplements use some form of vitamin E as a preservative (in the ingredients list either as mixed tocopherols, or straight up d-alpha-tocopheryl acetate), sometimes including it in the nutritional information table and sometimes not.
      Knowing about alleged correlations between Vitamin E supplementation and all-cause mortality, would such low doses used as preservatives be of any concern?
      I had my eyes on a vegan omega3 supplement by AAVALABS, which contains 150mg+75mg DHA+EPA, but also has 5mg of vitamin E per capsule.

      ​@@maxolina Sorry, missed this until now somehow! Yes, the megadosed supplements have become very widespread and dominant; remember the original thinking behind these very high doses was that they would ensure adequacy for almost everyone, even those with absorption issues, most notably with intrinsic factor-mediated absorption (pernicious anaemia et al.) This was fine when it was thought that there was no down side to taking these high doses - but remember the evidence emerging now to the contrary is still in a very early phase and certainly the magnitude of potential effects are not enough to warrant a massive overhaul of B12 supplement manufacturing practices at present. I wonder if lower-dosed supplements became the norm, would we do more damage by not providing those with absorption issues enough, than the damage being done at present by those taking unnecessarily high doses? Only time will tell! And furthermore, your question is not about public health but your own situation, and I applaud you for taking such interest in maintaining your own health, it is such a rare thing to see in my experience! The best I can tell you is to just keep a look out for a lower-dosed supplement, or just get the lowest-dose one you can find and divide it up into smaller doses - this is fine to do with B12 supplements.
      On to your question about omega-3 supplements, ah yes you are aware of the phenomenon of the negative health consequences of taking supplemental antioxidants including vitamin E! You are quite right to point this out, and when it comes to supplementing pre-formed long-chain polyunsaturated omega-3 fatty acids, including antioxidants in the supplement is simply going to be a part of the package deal - the molecules are simply too delicate to formulate into a supplement without this protection. This comes back to the ostensible axiom that getting our nutrients from whole plant foods is best - indeed I advise getting enough ALA and having a healthy enough diet to ensure adequate conversion to the LC-PUFAs as the absolute optimal way to get them, and you would get to skip including the extra vitamin E in that case. Having said that, if I was going the pre-formed EPA/DHA supplement route, I would much rather take in that little bit of vitamin E to ensure my omega-3s were well-preserved that skimp on the preservatives and take in rancid fatty acids. Also note that you're right in saying that 5 mg is probably more than is necessary for just preservation and I have seen supplements with much less than that so they probably are including it as an actual supplemental nutrient here as well. If you're really worried about it, you could certainly find supplements with less vitamin E or other antioxidants than this, but if you can't and you're going this route for your EPA and DHA, taking any supplement will be better than taking none, however much vitamin E it has. Sorry for the rambling answer, hope this helps!

      My question about vitamin E in omegas was mostly out of pure curiosity. Having never supplemented omega3 before, I'm using a 250+125mg DHA+EPA supplement from myvegan right now, whose dosage is perfect for my current needs, but as I get used to eating more flax/walnuts and especially making a habit of it so I don't just forget about it, I was looking for slightly lower dosage vegan DHA+EPA capsules. That's when I found the ones with added Vitamin E which sparked my question. I have found others since, which fortunately come with significantly less vit.E. Still I thank you for your thorough answer!
      One more thing if I may ask:
      Do you know anything about which DHA to EPA ratio is preferable? All the vegan omega3 including both are either 2:1 or 3:1, and digging deeper it appears that it's dependant on the actual source of the algae oil (Life's Omega appears to be 2:1 DHA/EPA, while another brand is 3:1, Lus Ingredients).
      Does it even matter at all, considering our bodies can supposedly convert either to the other?

      @@maxolina Yes, it seems going to the source of these molecules in the marine ecosystem the algae we farm produce 2 or 3 times more DHA than EPA. Once your supplement does contain both molecules and the ratio is no higher than this I am perfectly happy with it!

      I have meant to look into this in detail but I haven't seen anything convincing to suggest that cyanocobalamin will be inadequate for those with an MTHFR mutation. You could always look at trying to source a combined methyl-/adenosyl- supplement if you're worried!

      I work in genetics and people with the methylene tetrahydrofolate reductase genetic mutation absolutely cannot absorb cyanocobalamin.
      They cannot process any B vitamins that are not methylated. This includes folate. A person that has the MTHFR either hetero or homozygous will not be able to process folic acid and therefore will need to take l-methylfolate instead otherwise it can lead to a toxic buildup in the body.

    Can I ask something please? When I take a 1000mcg Cyano B12 sublingual tab twice a week I feel really sick in my stomach and it gives me 'bad movements'. I have yet to experiment with lower, more frequent doses, like you recommend. I was wondering why I would have such a problem with this supplement when other seem not to. What could the problem be?

      Hi Nick, it might be something as simple as a reaction to one of the excipients in the particular brand you are taking. As well as trying out a more frequent, lower dose regimen, you could also experiment with a different product - it might be as simple a fix as that!

      Cobalamin has multiple forms, one of which is cyanocobalamin.

      This is correct, the molecule is unstable without a functional group in that position and the cyano- functional group results in the most stable form of them all which lends itself to making good supplements.

Kudos on your diet, always delighted to hear from vegans - my goal is to keep us all as healthy as possible! :D If you have no problem with B12 (absorption, deficiency, etc.), that high of a dose might be overkill given the recently-characterised link with cancer as I outline in this video:

      I think it makes sense to expose ourselves to the least amount of B12 possible whilst ensuring adequacy if you are already replete and just looking to maintain, which I explain how to do in this video:

Until then, only on my cursory reading on the topics you ask about, time-released and sub-lingual B12 preparations are innovations pertinent to B12 malabsorption and the efforts to circumvent it somewhat, so might not be as relevant to the situation you are lucky enough to find yourself in. But don't take this as gospel yet, I will be fully researching these very questions you ask when I finish my current vitamin D series and revisit B12!

    I was diagnosed with a severe B12 deficiency of 87 about 6 weeks ago. My neurologist then referred me to my GP for b12 shots. I have ulcerative coilitis. She is prescribing monthly shots of 1,000 mg of cyanocobalimin but my balance and trouble walking is getting worse. I have read other standards of treatment in the uk are more aggressive and stress hydroxocobalamin. I am located in the us. Do u know of any publications that can address treatment for patients including dosages that I can show her? My GP. Said she never heard of hydrocobalAmin.

      Just eat plenty liver ,or get the nuritional yeast from Sari foods on Amazon .i have nerves isssues from my family as an african we have severe b vitamins deficiency speciLy at old age most of my family, have bad leg or back issue but again bome nerves etc.. one of my Aunt has paykinson im in NY .I USE BENFOTAMINE, NUTRITIONAL YEAST ,LIVER,ALPHA LIPOIC ACID AS WELL,MAGNESIUM ,ZINC POTASSIUM ,VIT D3/K2.I DO LOW CARB DIET AND INTERMITTED FASTING. 2YRS NOW ,more improvement i just went natural eating as much .avoiding gluten sources at best.

      T. Lukács It’s resveratrol. It is found in high concentrations in Japanese Knotweed, Cacao, Pinot Noir Grapes, and surprisingly in Camu Camu berry😁. It is not an essential nutrient although it does confer many benefits to the body such as the activation of the Surtuin 1 genes which are closely linked to life span.

      yes as an African/ American this B12 deficiency is often in our lifetime. Probably because of the people stolen from Africa into slavery. Also the slaves hardships in America. So both continents suffer stress. Important fact why our families suffer. However I believe those other cultures that were not a participant of slave ownership but that knew of the slave trade the traumatic experiences also suffered because they were forced into fear silence and not to speak for African/Americans. No one talks about the innocent ones even in the mist of your own family some still could voice their discernment . That was a double shotgun at them. I love all people . Hopefully we can forgive the unforgivable. We have to find that common ground to build trust and that could still take decades. We have to start somewhere somehow. We are all unique beautiful people.

      Thank you for this information. My lab results and my low energy indicated that I needed B12. I wasn't told which form or formula. I took B12 (methylcobalamin), drops, 5,000 mcg. per dropper full, and there was no improvement. 3 days ago, I began taking 500 mcg of B12 (cyanocobalamin)capsule per day with food and experienced more energy within the first 24 hrs. of the first day of taking it. I hope this helps someone.

    Don't bother cyanocobalamin vs methyl b there is no difference. Not enough evidence to jump to conclusion. // edit also wanted to mention cyanocobalamin where I live costs a lot for just 10mcg or 20mcg (120 tab)taken double tablets. What I got is methyl b12 800 tablets 100 mcg for almost 10x cheaper in same time 10x

      They are excellent for correcting severe deficiency and for certain malabsorption syndromes, as directed by your doctor. Not needed for maintenance of already healthy levels with normal absorption!

    Some people harvest the 'Spirulina' algae at home and sell the tablets commercially. My question - Is such consumption of tablets made from 'Spirulina' algae is safe ? Does it contain consumable vitamin B12 ?

      I have not researched spirulina in much detail. The only time I have come across it was during my research on LCPUFAs, for which it compared unfavourably as a source against golden algae. Spirulina is getting super-popular as a supplement and in smoothies and protein powders and stuff though, so I will definitely have to look into it in detail at some stage!

      How do you know it works great? How long have you been taking it? I have been using a cheap store brand multivitamin for 4 - 5 years, and in the past 2years seen a slow decline in my health, now culminating in peripheral neuropathy. I cannot say for sure if the supplement is the cause, but all things being the same in my habits, that is the one most suspicious factor. That and possibly the alcohol based mouthwash I have been using for 5 years. The food and drug industries are more than willing to kill us in order to take our money.

      This is because it's not usually used in oral supplements due to its inferior stability in these preparations - its main use cases are as injectable B12 and as a treatment for cyanide poisoning. Stick with cyano-!

    Very good, thanks. Unlike in the past, now I cannot find any supplement of cyanocobalamin unless it is together with iron and other micronutrients or as B complex, not alone so I have to take methylcobalamin. In recent videos Dr. Michael Greger and dietician Brenda Davis recommend 1000 mcg *per day* for those above 65 years. If you have views on this I would be grateful if you could share. Thanks.

      Yes, the specific B12 absorption mechanism is very complex and as I mention in this series is vulnerable to failure as we age. This is where megadosing comes in - of course the small increased cancer risk from high doses is dwarfed by the risk of problems if we don't get enough B12!

      No. Meat has many vitamins like A. Fat is important for liver functions etc. Try keto diet, that makes more sense.

    For me, cyanocobalamin works best, I can't sleep with other b12, I don't know why.

      Yes it's probably safe, but it's extremely excessive for most purposes! Unless you're treating deficiency with specific absorption issues, there is no need for a daily dose so high, and indeed given the recently-characterised association between cancer and high doses of B12 you might want to minimise the amount of B12 you take in whilst still ensuring adequacy, as outlined in this video:

      Hello, Rahul! Without knowing anything about your background the first thing I would be asking is do you have a malabsorption syndrome? How long have you been taking that dose of B12? Is this a daily dose? There are a number of things that could be causing this discordance between ingested dose and achieved level, but if you have been on this dose for a while and not achieved a healthy level you have to start thinking about problems with absorption. Whatever is going on, this level is too low and should be followed by your own medical team to try and figure out why it's happening, and also to ensure you have a strategy in place to get this level up to a healthier range (possibly by injection depending on the cause).

      @@rahulmojumder6936 Ah yes, I was thinking that would be an unusual preparation for over-the-counter. If you're only starting then just give it time, hopefully that high dose will have you back in the healthy range very soon!

      @@DrDesHarrington thanks . I feel very lethargic and tired. Actually I am medical student , so i have done many tests like lft, CBC, amylas e, lipase, usg . All reports are ok then I went to a doc he gave me b12 and d3 tests, my b12 is 96 and d3 is 11 .I am about to start my medication .

      @@rahulmojumder6936 Ah, good luck with your studies! :D Yes, you could certainly feel lethargic with that level of B12. Had it affected your CBC? I don't suppose you have access to the holoTC or MMA tests for functional B12 deficiency? That level of vitamin D is definitely too low, as well - check out my current series on vitamin D!
