In this video, I'll walk you through step-by-step how to use the Nursing Diagnosis Handbook by Ackley and Ladwig (Elsevier) to create A+ nursing school care plans.
This is my favorite resource for writing care plans in nursing school, because it has everything laid out for you: the NANDA® nursing diagnosis, the signs and symptoms, the patient goals, and the nursing interventions (including rationales!)

do you know a pocket size book that i could use?
Abigale Alexander, great question! We aren't aware of any pocket size ones, but you could make a little reference of the most commonly used ones that you can access quickly and bring with you easier. That might be the best option!
