what is the difference between the care planning process and nursing process?

The Nursing Process is the whole process. Care planning is part of that process.
Assess. Set Goals. Plan. Implement. Evaluate.
You would probably set the goals as part of the planning stage. Some use ADPIE, which adds "Diagnosis" after "Assess". Sometimes, it's not so simple to make a clear diagnosis though - Depends on the Patient/Service User.
