Pulsatile Tinnitus: 7 Different Anatomic Causes of Hearing Pulsations in the Ear

  • Pulsatile Tinnitus is the sensation of hearing a rhythmic noise, such as a heartbeat, swooshing or whooshing from an internal source. It may be heard in one or both ears on an intermittent or continuous basis. For most, it is a mild irritation, but when it becomes continuous and loud, it may become debilitating.

    Pulsatile tinnitus can have many different origins, some fairly benign, but others can be potentially life-threatening. Medical conditions that may cause pulsatile tinnitus include any condition that causes the heart to pump harder. Such medical conditions include high blood pressure, hyperthyroidism, and anemia. Ear conditions that may contribute to pulsatile tinnitus include any type of ear blockage whether earwax, fluid, or even a tumor. Such ear conditions block external sounds from entering the ear thereby amplifying internal body sounds like the beat of blood flow.
    Beyond these medical issues, there are 7 main anatomic causes of pulsatile tinnitus which include dural arteriovenous fistula, carotid stenosis, intracranial hypertension, brain aneurysm, venous sinus stenosis, sigmoid diverticulum, and a high-riding jugular vein. Unfortunately, in about 30% of patients, no medical or anatomic cause can be identified.

      Hi doc.please help me. 3 weeks ago i was having a bad virosis( flu cold) and i was taking ibuprofen and paracetamol. In the last days i was bite by a dog and make a big infection ,taking amoxicilin and ibuprofen. ( it was a bad infection) when all come to normal i drink 2 bears and put Minoxidil in my head for hair loss ang go to sleep. I woke up with pulsatile tinnitus in my left ear and stay for 1 week ..... it can be related to my story or is a venous problem? I am scared of all this venous arterial problems. Here we dont have doctors that known about this. Thanks

      Mine isn't like any of those . Mine is high pitched and loud. It's like regular tinnitus but goes in time with my pulse.

    Everyone here should get their B12, vitamin D, magnesium, folate, calcium and iron checked. Also get TSH, T4 AND T3 checked and lastly get checked for gluten intolerance/coeliac.

    I wish I had seen this years ago. Went through so many treatments and have seen so many doctors but no one knew what’s wrong. I felt so alone. Fortunately I have found a doctor who can help me now. It‘s not gone yet, but it will and I am still full of hope.

      First I would go to neurosurgeon or vascular surgeon. They were the only doctors who knew about the issue. I'll be getting a catheter angiography soon. They will check if there are fistulas. Can you influence the noise? I can suppress it by pinching my carotid artery. This is an indication that it is not necessarily triggered by the ear.

    I have been suffering from this disease for 15 years. In 2020, I had a brain angiography due to the complaint of ear ringing. But it didn't go away. The humming sound decreases in winter and gets louder in summer.

    After 17 months dealing with double vision, squiggly veins covering the whites of my eyes and a pulsing in my head keeping time with my heart beat 24/7 I finally got a diagnosis and surgery. I had 3 CT scans, 2 MRIs, hearing tests, 3 ENT doctors, I was referred to a neuro ophthalmologist who said the only procedure left was an angiogram. I had it done and they found a cranial cavernous fistula. I had one failed surgery and was sent to San Francisco to a neurologist who does this kind of surgery and has the tools and experience to get through a narrowing in the vein that the radiologist couldn't get through in the first surgery. So if you have pulsatile tinnitus, I would find a good radiologist and have the angiogram done. They don't always find the issue, but pulsatile tinnitus is the one tinnitus that can have an issue you find.

      @anthonyduggan2095 Not like they said I would. They told me when I left the hospital that headaches could be part of recovery, but I never had headaches then or from the beginning of the issue. Every once in a while, I would get a sharp pin in my eye but it only lasted a minute. But I take a lot of supplements and I was loading up on eye supplements. The fistula raised my eye pressure. I was close to losing my sight because of the pressure and interruption of blood flow.

      @@user-ni1he8qv6u I'm a 73 year old woman and I'm doing fine now 8 1/2 months after the surgery. My only issue now is a big floater in my right eye that drives me crazy but I'll take it over the double vision. If you live in SF Bay Area, Dr English in San Francisco Sutter did the surgery and he's excellent. If your tinnitus is the pulsing, it's most likely found with an angiogram. Don't suffer for months like did with ENT doctors who are a waste of time.

      I started sleeping with 1 ear pod in. I listen to rain, airplane white noise or Spanish words....all 5+ hour long You Tube videos. Hope that helps.

      i had mri from my head, ultrasound of the carotid artery, the doctors checked my ears inside. But in non of the checks they found something

    I have the swooshing and pulsating sounds in my ear (and i can feel it). I also get pain when there is any kind of pressure in my head e.g. when you laugh sometimes there is pressure in your head. this has been happening for months now and i’m not someone who typically gets headaches..

    i used to have that as a kid and it drove me crazy, especially when i was in super quiet areas. sometimes, i wonder what caused it and suspect asthma meds i took (albuterol, theophylline, terbutaline) may have been culprits.

    What I hear is like a heart beat but it skips it’s kinda like: bump bump bump…………………..bump bump and then it just stops but it’s not loud at all it just feels kinda ticklish when it happens so it doesn’t really bother me

    I’m getting a bit worried as I get irregular whooshing sounds in my head that is coupled with a pulsing sensation with each whoosh if that makes sense?? There isn’t any rhythmic flow to it as such like say following a heart beat pattern. A slight dizziness comes with every whoosh & pulse but only lasts as long as the 2 … It is a very weird feeling sensation … I think I feel it in the top & front halves of my head … It comes and goes and isn’t always there. Woken up this morning and it’s pulsing away like mad right now, not painful but it’s and extremely weird sensation. Could it be this??

    I am suffering from migraine since 2019. Recently I heard the heartbeat like sound on right ear and the bad thing is this sound trigger migraine and vice versa. Now GP told me I have hypertension also. Recently I hv done Lipid Profile test, though im in 20s my cholesterol is a lil bit elevated.

    I don’t know if this is considered pulsatile tinnitus but I only hear the heartbeat when I hiccup and yawn but then it just stops until I yawn or hiccup again but the hiccup thing is gone so it’s just the yawn

    This is dangerous.i was forced to have these symptoms from physical abuse from the same people that used dark witchcraft on me along with the use of a digital type gun that shoots electric threw wood floor and sheet rock.this happened to me in Central islip.becareful and watch who lives over your head in apartment buildings.

    When I googled pulsatile tinnitus it said it could also be caused by muscular and skeletal problems like TMJ and uneven bite. I just started Invisalign trays and my bite is uneven. I wonder if it could have caused it cause the timeframe syncs up. Going on 6 months now.

      It is stressed induced. Your blood vessels weren’t bad enough a habit wouldn’t fix it. The more you stress the more blood flows through your brain.

    So I started feeling pressure that runs down my left side of my neck and under my ear and in my ear I hear and feel pulsing on my left sometimes mt right but not enough to concern me but my left is the most sensitive and it causes headaches at times and my face aches sometimes and I press on pressure points in my face to relive some discomfort on my face I noticed I didn’t have it for almost a whole day and later that evening I cried and I felt full pressure again and pulsing in my left ear is this related? I have POTS too and I’ve had pots since I was 13 and I never had this problem I’m also on beta blockers for my pots. Im also on fludocortisione for years now I’m only 24 years old too

      Front of the neck? Or where? Do you have trouble popping your ears? Eye pressure?

      @@Hondo34sorry I barley saw this comment yes the pounding and pressure was in my ears and ran down my neck beside my ears and I would get headaches like behind my eye and sometimes pressure points help it but It has improved throughout time also stopping a medication they did a cat scan on me to see for pulsatile tinnitus and everything came out fine I think so they didn’t go forward with my symptom I also bought this cooling cap for the head from Amazon and it helped during those times and just relaxing you should check your blood pressure when you get that and see if it’s elevated because mine was from a medication I had to stop it

    I had the flu a couple of weeks ago and at one point my head felt stuffed up, then i had a ringing in my ear and now i have this whooshing sound in one ear...can a flu cause this? I tested neg for covid.

      Does the whooshing match ur pulse? Just curious, but yes tinnitus can absolutely be caused by a cold or flu along with allergies, plane rides, exposure to loud noises ect. Eustachian tube dysfunction. Medications, nasal sprays, nasal irrigation, balloon dilation, ear tubes may cure it. If it gets disruptive to ur life, see an ENT Dr

    Hello, i am from Turkey. This began to happen to me (only my left ear) after i had angiography and one stent insert into one of my 60% percent narrow artery and doctor started a lot of medical pills to me. Those medicines are beloc, ator, ecoprin pro (ASA), norvasc, atakand 32/12,5 mg, plavix and stomach guard pills. Could one or more of these medicines cause these annoying sound on my ear? It makes me feel that i can die in anytime especially at nights when silence around.

      @@etdmike Thank you for your assistance, however, i am 49 years old and i havent experienced this and any other kind of tinnitus symptoms in whole my life until these medication and angiography/stent operation. I am suspecting from the pills i started, before these treathment i've been using snri antidepresant and desyrel and i started these medications additionally. Could two or more medicines combined together cause this annoying symptoms, all i wanna know is this?

      @@etdmike Thank you sir, but i ensure you i have never experienced any tinnitus symptoms in my ear before the treatment that i mentioned before. This is totally new.

      @@uzakgolge4255 I am referring to "other" symptoms "aside" from Tinnitus that occurred "before" the treatment that you tried.       I had Bells Palsy when I was 20 yrs old, and it last for 3 mos. I saw a doctor for it, and they put me on medication. I couldn't close my right eye, and had to wear a patch at night when I slept in order to keep my eye closed. To this day, many years later, that side of my face is the weaker side of my face.

      Music and other white noises could help. But as you saw in this video, the sound's origin might be potentially dangerous and yo u should defenetly visit a team of doctors. The point about pulsatile tinnitus is that the cause detection could take some time. At first, you should take a blood test to make sure you dont have anemia. Then, you should visit an ear, throat nose doctor to make sure the problem's not from your ear anatomy. Then if not cured, you should visit a cardiologist to make sure you're veins and arteries are doing fine (to make sure there is no narrowness of the veins or fistulas or any cardiology causes which might end up to pulsatile tinnitus). The docor will prescribe mra/mri to check the cranial veins and might also prescribe propranolol (beta blocker) to reduce the heart beat and blood pressure and at last, the sound. Until here 63% of the patiants' cause, gets diagnosed and cured. If not, you should visit a neurologist to make sure there is no hypertension or tumors. And at last if no doctor could diagnose the problem, it is the proof that your pulsatile tinnitus is not a big deal and there's nothing to worry about. But also, there are some routines which could reduce the noise with reducing the heartbeat and blood pressure such as sport, drinking enough water, maintaining a nutritious low salt diet and stress reliever activities such as meditation.
