Dr. Mariela Glandt - 'Reawakening the pancreas in Type 2 Diabetes'

  • Dr. Mariela Glandt was born in Argentina, received her BA from the University of Pennsylvania and attended the University of Texas-Houston Medical School. She continued on to a residency in internal medicine at Harvard’s Beth Israel Deaconess Hospital in Boston. During residency Dr. Glandt did research on beta cell failure at the Joslin Diabetes Center. She then held an endocrinology fellowship at Columbia Presbyterian Hospital in New York, where her research focused on the reversal of Type I diabetes.
    Dr. Glandt spent the following years in Israel, as director of the Diabetes Clinical Trials Center at Hadassah Hospital in Jerusalem with Prof. Itamar Raz. Dr. Glandt and published articles in international journals and was involved in clinical trials.
    Dr. Glandt now works as an attending endocrinologist, as well as the associate program director of the internal medicine program at Bronx-Lebanon Hospital. Her current focus is on improving patient and community health education, particularly in the areas of obesity and diabetes.

    I started Keto 4 months ago. Took me less than a week to get off glimiperide, metformin was reduced from 2,000 a day to 500-1,000 a day for a month then stopped. After just over a month I was pulled off of my heart and high blood pressure meds. My diabetes isn't totaly gone but my average glucose is 50-60 points lower off meds than before with meds. I've had glucose readings in the 80's and 90's since starting keto with no meds. I've had readings of 300 before with meds.

    If u have been diabetic for 30 years and u got him off insulin is amazing. So he’s still producing insulin after all this time. Amazing !!

    This is a terrific explanation and wonderful to see her enthusiasm when she talks about various patients recoveries, including her father. She follows up on what Dr. Hallberg said in a recent presentation about how having diabetes for 3 years and for 8 years is like a different disease, and how 3 patients, after being compliant with the low carb program for 3 years, finally, had their diabetes go into remission and she was able to take them off insulin. The three had been diabetic 15-20 years!
    I bump into people who have been diabetic for 10 years and so I can tell them to stick with the low carb and just stick with it as there is a good chance the beta cells will recharge and get going, eventually.
    Dr. Berry has been talking about the fatty pancreas for awhile in his youtube videos so these functional medicine doctors are way ahead of the curve.

    Dr. Mariela you are the best. Such a comprehensive and clear presentation. Thank you. I remember you from your days at the Joslin with Gordon and Tony Monaco. Your enthusiasm has only gotten greater. Speak often, you will give primary care folks the confidence to cross the bridge to rational diabetes care.

    Believe I got Type 2 from my Covid spell (bad) a year ago. Was not obese (6'1" 190#) and not inactive nor have a family history. I lost upwards of 20# in 8.5 weeks as a result of cutting carbs way down.

    Thank you Dr. Glandt for this presentation. Hopefully, lectures like this one circulate widely enough to where patients implore their doctors to abandon conventional approaches to the treatment of diabetes. It is my hope that, as there is more and more of a grassroots movement to use low carb as the first line of defense and treatment for type 2 diabetes, that eventually it will become codified into our standard of care. While we are far off from this happening, it would be nice to see a time where physicians who do not dispense low carb advice for the treatment of diabetes could be considered as being guilty of malpractice.

    Wow!!! That’s powerful! Fasting, diet and exercise is a must for all diabetic patients. No other options. Meds are administered based on the sugar/glucose levels especially when the patient is slim and there no room for weight loss. Such patients carry A1c >10!!!

Nonetheless, just read about theodicy from a philosophical point of view - there are many academic papers that deal with the topic.

    If you eat a ketogenic diet then your body naturally normalizes hormones, blood sugar, blood pressure and internal inflammation. Bariatric surgery reduces the absorption of nutrients. It may seem like a good idea, but if you just started a Ketogenic diet and intermittent fasting then you could bypass the bariatric surgery to begin with.

      Ketogenic diet would promote lipotoxicity which would de-differentiate the beta cells making diabetes worse in the long term. Keto may be only good for the short term but certainly not long term so be careful.

Was fasting a lot for about a year and only having one meal a day whilst doing weight training and martial arts, and then boom got diagnosed with hyperglycemia and ketonuria (high keytones in urine). My pancreas stopped producing enough insulin as well.

      @@purepositivity7404 Ketogenic diets have been proven to heal the pancrease and reduce related cancers. lipotoxicity is caused by carbs, not fat. I'm guessing your a vegan.

    I would say not just PCP’s, but Ped’s need to know about this. Our CHILDREN are grossly malnourished but severely over-carbed.
    I recently put my whole family on a ketogenic diet, and I’m pregnant with my fourth. This has been my healthiest and fittest pregnancy so far, and I’m 37. My new baby will eventually be weaned to LCHF. #isupportnoakes

    I don’t quite get her time duration of diabetes argument. She brings up her case study who had an A1C of 10.8, you don’t suddenly develop a 10.8 A1C, you go through 5,6,7,8,9, and 10 over multiple years to get to that point. So how long has her case study actually had it and how long did the people in the studies actually have it?

    Shower thought: if every cell in the body has insulin receptors then beta cells have them too. Since they are excreting insulin they are likely to be more stimulated by insulin than any other cell in the body. They then die from this, and not from being "worked to death". The alpha cells of Eskimos never get "worked to death" making glucagon for their zero carb diet. This "worked to death" explanation seems to only apply to beta cells and not to any other, including the alpha cells right next to them in the islets of langerhorn. Insulin kills and it kills the cells stimulated the most by it first.

      That is flase. Check out the case for low fat diets by Denise Minger on CZcams. Eating sugar and rice cured diabetes in 60% of the group. No fat, very low protein. But I am not recommending this. But it deserves we recognize the merits.

      Insulin DEFINITELY lowers blood sugar, but has many side effects, so not a good answer to diabetes. But as blood sugar level will be OK, mainstream doctors like it. Elevated blood sugar is not the illness, is a symptom only, and the goal is to cure the illness and not the symptom.

      well yes but actually no. insulin will cause the blood sugar to go down fast, sure, but the sugar is still in the body. This is normal as your body uses glucose as fuel. However, it's not good if you are diabetic as it means you have too much sugar in your body.

      You're thinking is very simple minded if you're implying that as long as you cut out all carbs and sugar, you can reverse type 2. Many type 2's are not only insulin resistant, but low producing in terms of insulin secretion. Cutting out sugar and carbs is not enough for those who are low producing in insulin.

    Could it be just as easy as parasites that love sugar and just do a detox and eat bitter & sour foods.

      Bingo!! But you will never see that here or anywhere...as it also cures all the other big money makers, such as C... depression...MS...you name it, 90 percent of it is parasites. Fasting works because it also kills parasites IF you do it long enough, since parasites not only require our glucose but also our water, thus dry fasting is most effective and by day 7 all worms will be dead at zero cost to the patient. And zero profit to dr, pharma, educational and research institions, talking heads, writers, etc etc.
      parasites stun the beta cells with their secretions, they WANT less insulin so that you blood glucose goes up and they get fed, it is that simple. Why is ivermectin practically outlawed and driving the pharmacreatures nuts...?? Guess what it kills.
      Personally I like to mix it with Praziquantel.

    I wonder if giving choline (and perhaps inositol too) would help. The choline will push fat out of the liver but then it may be necessary to add a baby aspirin to fight the effect of this fat being thrown into the bloodstream.

      NISAIDs will thin the intestinal mucosal layers and make you prone to bacterial infections. The lipid levels will stabilize as the cells use up the fats. SIBO is not fun

      hi, typically 72-96 hours to transition to fat burning. The glucose levels will drop quickly after that. The liver will make glucose from fat as needed (usually in the morning which wakes you up). Hope you consider it. Try Dr Eric Berg DC for diet guidance

      It took me a few months to recover from 350. But, I was mislead by my PCP to eat a low fat, high fiber diet out the gate. I'm now keto and hovering around 100 w/out insulin. I think it would have taken me only a month with the right advice. Granted, I took insulin the first two months as well.

    How does eating steak lowers ectopic fat in the liver and pancreas? Pardon me, I am still learning and I understand that there are 2 schools of thought here. One on low carb and another on plant based but good carb diet.

      It's not the steak. It's the lack of glucose and insulin that allows the ectopic fat to be reduced. The idea is to reduce carbs to less than 20g / day, for example. 120g of carbs is a really heavy load of glucose to stuff away every day.

    At first it seemed like she does not fully get it, but I guess she does. Type 2 diabetes is a disease of too much insulin not too little. The high glucose levels come from the insulin resistant liver making way too much glucose and the person piles on with bad high carb diet.
    So there are 2 components of glucose load. If you correct the diet, you just have the gluconeogenic load to deal with. This is my situation. I have a high glucose in the morning but great response to proper meals. My A1C runs 5.2 to 5.6. If I didnt the big spike, my A1C would be in the 4"s!

      I have type 2 diabetes and I can describe how I felt before changing my eating habits...................constantly tired,eyesight not so good, skin breaking out in sore pimples and hot........my arthritis in my feet and hands was cruel,,had to take strong pain killers which caused chronic constipation........burning feeling in my feet at night....bloated because of the medication........ I was taking 3different meds to try to control my blood sugar........my GP...kept suggesting that I use insulin but I somehow knew that would be worse................... I’ve been on a low carb keto diet for about 3 months now........... I still have to use metformin just now but not the other two........arthritis in my feet and hands is gone.......skin clearing up...starting to feel better.... I used to have very dry itchy skin....this has improved greatly.... I was so sick I thought that this was the end........my diabetes was brought on by taking prednisone for about 8 years.......I’ve done untold damage to my kidneys and other organs with this stuff...... I don’t think I will ever be terrific but life is so much better.....

      Actually, if one converts to fat burning by way of a near keto and keto high fat diet, episodic calorie restricted days aka fast days become very doable and a useful tool as recommended by Jason Fung among others.

    I think pretty much everything she's saying about type 2 diabetes has been proven false. People with T2D aren't creating too little insulin. They're creating so much of it their bodies have become resistant. I mean, I appreciate the Superman references because they're nerdy, but I don't think she's correct about her assertions.

      @@mirjammaver8670 The beta cells in the pancreas are reduced. And she is absolutely correct. I had twenty years of high insulin levels and low blood sugar episodes before my blood sugar remained too high all the time. A low carb diet then helped me control drop my HBA1C for many more years, but now that I am getting older, my beta cells are disappearing and I have to take metformin. I can't get my blood sugar totally normal even with metformin and low carb combined. It is much better but not like before. On diet alone my HBA1C used to be 5.3. Now on medication and diet the lowest I have gotten it to is 6.9. The problem is everyone tries to extrapolate their own body and own experiences, thinking this is what happens with me, so it must be true for everyone else. but they don't yet know the changes that come the older one gets.

      @@Jan96106 - I second what Defeqel said. Search youtube for videos from Dr. Jason Fung and Dr. Valter Longo. Fasting induces autophagy which is the process by which your body breaks down old unhealthy cells and builds new ones. This is shown in both Type 2 and Type 1 Diabetes.

      For some that is true that early on, the pancreas over produces insulin to counteract the high glucose. But the pancreas has a shelf life and will eventually burn out and underproduce insulin when you need it most. Also, when you graduate medical school and become an expert in your field, people might listen to you more.

    She lost me at 23:28 ~ Most (if not all) studies show that high-fat (low-carb) diets, especially saturated-fatty-acids cause insulin-resistance and beta-cell destruction, whereas low-fat diets based on whole-grains and beans are protective.

      Library Files if you’re new to the trend. Join a bunch of intermittent/extended term fasting and ketogenic support groups on Facebook!!
      Insulin producing foods with low glycemic indexes is a good place to start “fat”. Paired with higher than average amounts of salt (low carb keeps your electrolyte stores constantly depleted if you’re fit at all or drink coffee [diuretics]) along with some sort of intermittent fasting to help with any hormone rebalance, as well as raising IGF1->HGH to aid in lean tissue production to boost resting metabolic rate.
      Most of all just stop eating sugar and processed foods. And find jesus. Because we could all use some 40 day fasting motivation 🔥🥩
