    Dear DR Prashad, many thanks, is Silhouette Instalift or Sculptor similar to structural volumising? And are there clinics in Europe , especially UK that perform structural volumusing?

      Hi Cathrine Katsigianni,
      Silhouette Instalift is a suture suspension procedure, which is a type of minimally invasive surgery, so it's not the same. I couldn't find any details about the Sculptor procedure, so I can't comment about it. Structural Volumizing is also known as the Y Lift, and there is a growing network of Y Lift doctors that you can research, but I'm not sure if it has reached Europe yet.       Projection of filler depends on the type of filler and where you place it. Orbital rim implants are not permanent as the volume and dimensions of your face change with age. I would need to perform a physical exam to determine the best treatment for you. I hope this helps you.

    I’ve been told that getting filler every two years makes your face saggy because it’s like a balloon that’s constantly blown and deflated. But as you mentioned, what worries me about face implants is how they will look with age when one loses more bone structure. A doctor said the implants become part of the bone structure, but I don’t know!

      Fillers do not stretch your face, and I did make a couple of videos about why they don't. Facial implants do not become part of your bone structure as that it implies the bone grows around them, but bone mass actually diminishes around the implants with age.     I'm going to get submalar Voluma placed and I was wondering from Your experience is there such a thing as too much cc's placed during the first session? Not just in terms of esthetics (thicker hyaluronic filler draws more water I assume) but in terms of safety as well?

      Hi GlacialImpala,
      It's not about the quantity of filler, but the technique of placement. For example, if a moderate amount of Voluma is placed in the dermal or soft tissue level, it may look like too much because it can't be supported by soft tissue. However, when we place Voluma at the stronger bone level and in between the muscle, we can add more filler than what was used in the soft tissue level, but it won't appear excessive, rather more defined and structured.     O.k. I watched. So..if u have implants in the cheeks along with your lower face and neck lift u don't go thru the mouth to place them ? Would it cost less doing 2 in one than 2 separate operations?

      Cheek implants can be placed through the mouth. It would cost less to the facelift and neck lift with the cheek implants in one surgery as you are paying for anesthesia, support staff, and operating room time only once instead of twice. I hope this clarifies things for you.

    Doctor first of all i just want to thank you for spreading all this valuable information. I wanted to ask you about bellafill. I got bellafill on my cheekbones and I love the results as I now have high cheekbones. Will Bellafill sag with the skin over time or will it stay in place since it is injected at the bone level?       Filler placed at the bone level, and held in place by the muscle layer should not descend or sag if placed in an optimal way. As I'm not certain about how your doctor placed the filler, you can consult with your performing doctor about it. I hope this helps you.

      Injecting filler at the bone level is actually not commonly performed, as it does require knowledge of facial surgery, even if it's a non-surgical procedure. Most filler treatments are placed at a more superficial level near the skin, such as in the dermal level, or fat and soft tissue level, which are not as strong, so there is a possibility of filler migrating, or looking more "doughy" in appearance.     What do you think about Bellafill? I'd rather pay more up-front , but concerned you can't get the structural filler look like with Voluma.

      I don't administer Bellafil because I generally don't advise permanent or semi-permanent fillers, as they are prone to complications because the stay in the body for so long, and are costly and difficult to remove. Hyaluronic acid fillers are much safer to use as the body metabolizes them safely, hyaluronic acid is naturally found in the body, and they can be dissolved if needed.       Cheek implants aren't going to descend as they are anchored to the bone. However, as explained in the video, cheek implants eventually won't fit as facial dimensions change due to age-related volume loss. I hope this clarifies things for you.

Also, what do you think of fillers and their very rare risk of blindness? I freaked out when I recently learned this. Thank you

      I perform check implant surgery with local anesthesia with LITE IV sedation, and I don't use general anesthesia. The risk of blindness from fillers is minimized and avoided by the skill of the doctor, so if you have a skilled doctor perform the treatment, the risks are low.       Platelet-rich fibrin matrix, which is what you are referring to, can help with skin quality, but it won't add significant volume, nor tighten the skin. Filler can help with volume, and perhaps a laser treatment may be suitable for some tightening. A physical exam will determine what approach is best for you. I hope this clarifies things for you.

    I’m thinking of getting jaw implants, however I’ve heard that over time it will degrade the jaw bone. I’ve also seen that the degradation it causes will make little change over a period of years. Is this true ?

      There is no evidence that jaw implants cause jawbone loss, as the jawbone already loses volume over time in people who never had a jaw implant. I hope this clarifies things for you.

      Your results are based on the doctor's expertise, aesthetic style, as well as their technique. For lower bleph, I recommend doctors who know the transconjunctival method to avoid common complications from the more common transcutaneous approach with an external incision on the eyelid skin, with some surgeons performing unnecessary skin removal to treat wrinkles.       It depends on the person, and how the procedure was done, but cheek implants can feel like a foreign material at first, but you can get used to it with time. Cheek implants can become more uncomfortable as the face loses volume due to aging, so they no longer fit. I hope this helps you.

    Thanks for this video! Your advice makes sense to me. But what do you think about the long term safety of facial fillers? Lately, we are hearing that they hang around for longer than it was thought and can cause scarring.

    Dr i have few questions to ask.. is hyaluronic tablet is good and safe to have ? If yes which one is best that you can recommend? Any side effects of the tablets? Dose tablets treat collagen and elastin ?

      Hyaluronic acid should be applied directly to the areas being treated by a professional. If you consume hyaluronic acid in a pill, they will be digested, and may not be usable hyaluronic acid after they are digested. However, the body does synthesize hyaluronic acid from all the food we eat. I hope this clarifies things for you.

    Hi Dr Prasad, i notice that everybody is talking about cheek fillers and the filling is always done in the area under the tear trough known as the upper cheeks. I understand there is the lower cheeks as well. How about the area which is more at the side of the face behind the buccal region? I have no idea what that area is called. Can filler be injected in there for patients who have unevenness for both left and right side? I tried looking it up and it's the region at the back of the buccal fat, somewhere above the platysma mandibula area. I do hope my explanation is clear. Must be the most difficult comment you have ever received? :P

      Hi,
      That is the buccal area. Hyaluronic acid filler in this area is more predictable, as results can be seen immediately. Sculptra is less predictable as it takes time for volume to be created by the body, so results are not as precise, and may take several treatments for results to show significantly. Good to know that fillers can be used there as well..So far I have not heard of any injector or doctor talk about that area for filling. In fact more to remove the buccal fats to make a smaller lower face. I presume it is not possible to do filler at bone level there since there is not bone there unlike the cheeks? :)

      I don't recommend any medical cosmetic treatment at your age because your face is still growing and maturing, so you still have many facial changes ahead. I suggest you be patient as these issues can resolve on their own as your face matures. If you still have these issues when facial maturity is more stable in your early 20s, then treatment can be considered. I hope this helps you.

      Cheek implants are long-term, but they are not permanent as the bone volume where the implants are placed diminishes with age, so they will need to be removed and/or replaced when you get older. Using smaller implants and filler for maintenance will not prevent bone from diminishing where the implant is secured. I hope this clarifies things for you

      Yes, there is an art to surgery or filler placement, so it best done by a doctor you trust, and whose aesthetic style matches your own. As someone who can place filler at the bone level instead of the soft tissue level, I can get more cheek projection without the results looking soft or doughy, so I advise you look for a doctor who can also place filler at the bone level.     Hi doctor. I'm a 28 years old male with significant loss of volume in the cheeks and midface. I had 1.5ml of volume to each cheek and it looks the same. I did 3 fat injections that didn't last. Honestly, I'm depressed as I'm a young male with loss of volume and flat cheeks makes me look old. Do you think that in certain cases where the volume loss is very significant that cheek implants is reasonable?Thank you.

      Hi Omega Destroyer,
      Bellafil is supposedly permanent, and uses polymethyl methacrylate (PMMA) microspheres, which remain in the body. I don't use Bellafil in my practice because it does not meet my standards of safety and reversibility like hyaluronic acid fillers so. I hope this clarifies things for you.

      Although I do know colleagues around Chicago, we all have different aesthetic styles and practice focuses. When it comes to cosmetic fillers and facial implants, experience and style are important things to consider. Filler placement at the bone level is something most doctors don't do, so you can look for doctors who perform the Y Lift. However, even doctors trained in this technique do have different styles which show in their results, so it's important to look at before and after pictures to find a doctor's style that resonates with you.       Sorry to hear that. Cheekbone reconstruction would be needed before having a cosmetic procedure like an implant or filler, so you do need to see facial reconstructive surgeon first. While I have done cheekbone reconstructive surgery in the past, it has been awhile since I've performed it as my practice is focused on cosmetic surgery, and cosmetic procedures.       When it comes to cosmetic fillers, the material used matters a lot. Hyaluronic acid naturally occurs in the body, so hyaluronic acid fillers are safe to use. Older filler materials like bovine collagen can cause allergic reactions, and permanent and semi-permanent fillers like polyacrylamide can cause serious complications because it can be recognized by the body as a foreign material, resulting in a foreign body reaction.       @@christopheclugston Sure, deep plane facelift is amazing and anyone over 40 can benefit from it, but it's meant to redistribute the tissues, not make up new volume altogether. So it wouldn't necessarily create enough cheek volume.
