      @@AmiyaPrasadMD I don't know if uou can help understand something..I noticed that after getting a chin surgery one of my friends saw the shape of her face become more round while before it was long oval with a V shape chin.. What kind of operation did lead to this result (at least get rid of the long v shape) and is it possible to get it with injections only rather than surgery ? Thanks in advance 🙏

      It is possible that your friend had chin implant, as well as cheek implant surgery to make the entire face more round. And yes, it is possible to achieve the same in the cheeks and chin using injectable fillers only. However, when it comes to the cheeks, filler placement technique is very important to prevent a soft, pillowy look, and to stop the filler from migrating.     hello, loved your video! i am thinking of getting my chin filled to balance out my face. however, i am afraid that it’s going to migrate and would make my chin/jawline “bulkier” as the filler spreads and migrates as time goes by. any thoughts on this? would i need to get it removed any time i want another mL of fresh filler? thank you!

      Hi Mary Margaret Parco,
      I place filler at the bone level, where it's held in place by the muscle layer to prevent migration. I only recommend hyaluronic acid fillers, which can be reversed by dissolving them with the injectable enzyme hyaluronidase at any time. However, hyaluronic acid fillers, depending on their thickness, usually metabolize in 1-2 years. I hope this clarifies things for you.

    Hi! Great video! By the way I also feel insecure with my asymmetrical face, weak chin/jawline and relatively chubby cheeks and double chin despite that Im skinny (18% body fat) Im going to get as lean as possible so i can get rid of the face fat but I will still have a weak chin/ jawline.
    I'm thinking of getting fillers, are they safe? Long term wise though.
    Lets say I get fillers for the rest of my life
    Are there any side effects that would affect my face?
    Also the fillers can last from 6 months to a year, does that mean that they dissolve slowly by little or when Ive reached a year since I got the fillers, they dissolve all at once?

      Hi abgg 2000,
      Hyaluronic acid fillers like those from the Juvederm family are very safe as hyaluronic acid does occur naturally in the body. Hyaluronic acid fillers are metabolized safely by the body, but this is a gradual, slow process. The thicker ones used for the chin and cheeks like Juvederm Ultra Plus and Juvederm Voluma actually last 1-2 years. There is little chance of filler material having any side effects on your face, but things like encapsulation around the filler does happen in rare cases, but the filler can be dissolved if this happens.     Hi Doctor, great video. Is there a maximum amount of filler you can put in the chin area? I have seen examples between 1-4 ml. If someone has a very recessed chin can you put more than 4ml to build the chin out? Thanks!

      There is a limited amount of filler that an individual's anatomy will allow, but this limit depends on the person. There is no limit that applies to everyone. A recessed chin can be improved by filler, but a physical exam with a discussion about your goals is needed to determine what is best for you. I hope this helps you.

    I had a chin implant about 40 years ago in the states. The first time it was done, it was off centre (implanted through the chin). I had another doctor correct it. He opted to go in through the mouth. He did a good job. Now after 40 years of course my face has changed, and the implant looks off centre again. I have had some filler to help balance it....but now, I am thinking of having the implant removed - and just continuing with fillers. My biggest concern is a sagging and wrinkling in the chin area (once it is removed), which has already started a few years back. Does it make sense to have the implant removed now, and just continue on with fillers. I am 67, and fortunate enough to be near the La Prairie Aesthetic centre in Montreux, so I am confident of finding a good surgeon. I just don't know whether to keep the implant and fill in around it or go back to the original facial structure and enhance from there....Thoughts? and thanks!!

      I would have to see how the implant still fits your face to say for sure, but generally, facial dimensions change as we age due mostly to bone volume loss. Even when I do chin implant surgery, I often still apply filler in area for more precise volume enhancement. The advantage of doing the entire augmentation with filler alone is there is no surgery needed after removal, but it does need to be maintained periodically as the filler is metabolized. A physical evaluation and discussing your options would be the best way to arrive at a specific recommendation for you. I hope this helps.

    This is very interesting. My Dr recently suggested a chin implant to supplement an upcoming rhinoplasty and the mixed reviews on implants have me nervous to go that route. Approximately how much filler would be needed to replicate the implant results and is every 2 years the longest duration available currently?

      The amount of filler needed is highly dependent on individual bone structure, and the amount of projection needed, so I can't give an approximation without a physical exam. About 2 years using a hyaluronic acid filler is the safest treatment. There are semi-permanent and permanent fillers available, but I don't advise using them as they can be highly problematic and prone to complications as they stay in the body too long without being metabolized, and require extensive surgical removal.       My cosmetologist recommended me Versa that last about 8 months ,I asked for a filler that last longer and she told me versa is good for the chin area.
      I wasn’t happy with her recommendation but I did it anyway because I was already there ,but since a syringe was not enough may I get a different filler that last longer in that area?
      I got this a week ago.

    Are fillers effective with cleft chins? I would much prefer to go the filler route rather than an implant!

      It is very difficult to have the depth of a cleft chin reduced as there is a strong connection to the bone, and very few doctors are experienced in treating this issue. You would need to find a doctor with experience in it, and have a physical exam to determine what can be done. It's something that I don't have much practice in. I hope this helps you.

      I mainly administer hyaluronic acid fillers in my practice because of safety, predictability, and reversibility. The results of these fillers are shown in the video. Hyaluronic acid is naturally present in the body, so these fillers are easily tolerated by the body. They can also be dissolved with the injectable enzyme hyaluronidase if the patient wishes to reverse their results. These fillers are also safely metabolized by the body in about 6 months to 2 years, depending on the thickness and viscosity of the filler used. I hope this clarifies things for you.

      Reduced chin projection can make the nose appear prominent, especially from the side profile. Adding volume to the chin with filler can make the nose appear less prominent as it does reduce its proportion to the rest of the face. I hope this clarifies things for you.

    I’m 20 years and I really wanna go with chin fillers and jaw Botox to give my face more structure and make it sharper. From the front my face looks a little round, mostly the lower half of my face because of my jaw and chin. I really want to achieve a nice and very slim defined v-shape profile without undergoing surgical procedures. So can chin fillers and jaw Botox do that? Also the left of my face is slightly fatter than my right side, like my jaw is little more fuller and wonky on that side so can these procedures make my face more symmetrical? Last concern is blindness by fillers. I done research and discovered that blindness can be a serious complication with fillers but it’s very rare. Has chin fillers ever caused blindness? How much of a risk is there for blindness by chin fillers?

      It is possible to enhance chin projection with fillers, but I would need to examine your face before doing it for someone as young as yourself. Botox in the chin is usually only for chin dimpling like a golf ball, where Botox can reduce the activity of the mentalis muscle to improve this look. The risk of blindness from injectable filler is when under eye filler is injected directly into an artery or vessel below the eye, which can be avoided by an experienced doctor in the eyelid area, but it isn't a risk for filler in the chin area.     Hi Dr. Prasad, can you talk a little bit about why you don't use Sculptra for chin and/or jawline enhancement? Doesn't it last longer? I'm fine with my chin projection in the front, but have some concerns with the width of my jaw on the sides and how it sort of runs together with my neck too much.

      While I have administered Sculptra in the past, the results, especially for the chin and jawline, and are not as predictable or customizable as a thicker hyaluronic acid filler like Juvederm Voluma. Sculptra uses a biological reaction to create volume, so there is no control over what results will look like. With filler, I can mold the material to the precise look we decide on with the patient. Also, the results of Sculptra are semi-permanent, while hyaluronic acid fillers are gradually metabolized, and reversible if it's not the right look as the material can be dissolved with the injectable enzyme hyaluronidase.       You can refer to 5:20 in the video explaining that hyaluronic acid fillers are safely metabolized by the body, and can also be dissolved with injectable enzyme hyaluronidase if you want to have it removed. I don't recommend or perform semi-permanent or permanent fillers because of the high risk of complications associated with their longevity. I hope this clarifies things for you.

    Hello Dr., I recently got a chin implant along with rhinoplasty (almost 4 months now) and I am still experiencing partial numbness on the bottom right side of my lip. I was wondering if this would go away or if it's possible that I might need it fixed? I know it's still early in the recovery process but I feel very anxious about it. My bottom lip is also smaller than before and I was wondering if that was common with this surgery? I kind of wish I had done more research about filler before because I am not sure how I feel about this implant.

      Numbness, tingling, or swelling can happen after a chin implant because of the mental nerve being affected, but this can resolve after surgical year after about a year. Keep in communication with your surgeon if you have concerns. As for the bottom lip, it may appear smaller due chin projection changing, but it is unrelated to your procedure. You may want to compare older pictures to newer ones if this was an issue with normal volume loss due to aging prior to your procedure. I hope this helps you.

    Dr what about the dangers of filler, especially on the chin and cheek? If you go to a certified medical professional who is trained in facial aesthetics, do the cheek and chin fillers pose a danger of blindness like when injecting into the glabella or nasolabial folds?

      There is a risk in injecting filler into a blood vessel called vascular occlusion, which I detail in the videos below.       I prefer Juvederm Ultra Plus and Voluma as Radiesse is not reversible in case there are any issues. Also, Radiesse doesn't last as long as although it is supposed to be long lasting filler with harder calcium hydroxyapatite, much of the projection it provides is through the gel suspension which does get metabolized faster.       Filler application when done properly with blunt cannulas leave minimal trauma to the skin, even right after the procedure, and even in very fine, sensitive skin like the lips. While there are fillers that last more than two years, they are not as safe and predictable as hyaluronic acid fillers. The reason hyaluronic acid fillers last about two years is that they are safely metabolized by the body, as hyaluronic acid does occur naturally in the body. Fillers that last longer are resistant to metabolic processes, which can cause complications as the body can recognize them as a foreign body. While hyaluronic acid fillers can be dissolved if there is an issue, semi-permanent and permanent fillers need surgical removal, and surgery may not remove all of the material, especially if the filler migrates or dissipates. And unlike permanent fillers, silicone implants can be completely removed; while permanent fillers may not be totally removed because they break apart, and can migrate, and worsens the foreign body reaction.     I had a chin implant procedure three years ago that had to be removed because it moved and it was uncomfortable. I still cannot feel my lower lip and chin because of nerve damage. Am I a good candidate for fillers??

      One of the main reasons why I prefer filler for the chin is that my patients are often uncomfortable with the feel of an implant. You are likely a candidate for chin filler, but your issues with losing feeling in your lower lip and chin need to be properly diagnosed before I can recommend treatment.     Again, Dr. Prasad the major problem with fillers is their lack of longevity. With people like myself on a fixed income, this is not very cost effective. Though I have experienced one of the problems mentioned in your acessment of chin implants, (mine shifted and the ends started poking through my skin). Rather than replace it, the surgeon elected to cut the ends off. The whole point of choosing that particular implant was to give my face a "square" shaped chin rather than a pointy one. Needless to say, after he cut the ends off the pointy chin is what I ended up with which I hate. On the other hand, that was eight years ago and your right...the face...the bones, the muscles underneath change with the aging process. Now I look as though I am constantly "petulant" even when I'm not. Saving my "pennies" so when I do come to see you I'll be prepared. Love your aesthetic eye!

      They are not the same, as a chin implant is the placement of silicone implant on the chin, while a sliding genioplasty is reconstructive surgery that involves sawing the chin bone from the jaw, and sliding it forward. I don't perform sliding genioplasties, which are most often performed by oral surgeons, and maxillofacial surgeons. I hope this clarifies things for you.

      Amiya Prasad, M.D. thank you for your amswer. I am however really torn now since my doctor had specifically recomended augmentation with fat transfer instead of an implant. He pointed out that he would be able to shape the chin better with fat and that implants hurt the bonestructure in the long run, damaging and making it weaker/regress. What would your opinion be on this? Again thanks for replying. I am trying to gather as many perspectives as possible to take a decision

    Hi One of my main insecurities of my body would be my what I have found is called a weak chin. I have already inherited a pretty large nose and this makes it look a lot worse.       Generally, you can still have a chin implant or filler without taking away your cleft chin. A chin implant may make it appear less deep due to the added volume raising the space, but it is actually quite difficult to to completely get rid of a cleft chin with chin augmentation.     I had a chin implant in 1985 and haven't stopped crying since. I dont recommend it to any one. The Dr. Stated I needed one...never saw what he saw..i look back at old high school pics and I didn't need one...
    Now I feel that is one of the reason why I have major creases down each side of my mouth. Not that it the only cause but major resason. I have been told if I had it removed I would have the old witches chin...so here I am 2019 and continue to cry.

      Sorry to hear about your problem. It is important to not have any cosmetic procedures that you are not comfortable with, and a good doctor would not force it on you, but it's too late for that now. Chin implants are not permanent as the dimensions of our faces change with age as we lose volume from bone, muscle, fat, and soft tissue, but particularly from bone loss. You may want consider removing the implant, but you should get a second opinion from another doctor since your original doctor didn't leave a great impression with you. I hope this helps you.

    there are videos and studies saying the fillers don't really disappear, with MRI scans showing fillers still being there after 10 years. It is a scary concept, what is it your opinion on it?

When I perform this surgery with local anesthesia, patients can go home the same day as surgery, but a required overnight stay also depends on the doctor performing it.       When I place filler at the jawline, I place it at the bone level, and it's held in place by the masseter muscle, so it doesn't shift or migrate. With more conventional filler placement that may place it close to the surface of the skin, it is possible for it to dislodge or migrate as it is placed above the masseter muscle.     I am having pain in my chin after almost 2 years of getting a chin implant. It’s like I get flare ups. The pain comes & goes

      Sorry to hear about your problem. Pain and discomfort from a chin implant are not uncommon, as the implant can impinge the mental nerve, or slight movement can affect other nerves in the area. This is why many people opt for a filler instead, as it can be easily dissolved if there are regrets for chin augmentation.       Hi Bluepark Bluepark,
      We lose facial volume as we age, notably bone loss, so our faces get smaller. Eventually, a chin implant won't fit your smaller face, so it will need to removed and changed, so they are not permanent. I hope this clarifies things for you.

      Silicone implants are proven as safe as silicone is the most common implant material. Complications happen when liquid silicone leaks and spreads into the body, but chin implants are solid, not liquid silicone. If you want something more convenient and safer to a certain degree, you can consider hyaluronic acid fillers as featured in the video. Hyaluronic acid naturally occurs in the body, and hyaluronic acid filler is safely metabolized by the body over time. I hope this clarifies things for you.

    I wish I could afford a chin implant but sadly no😢, I've been bullied because of my backward chin 💔💔 I just want to sleep forever 💔

      Sorry to hear about your problem. If you watch the video, you can see how chin filler can be an option. While it still entails a cost, it does not require surgery, so costs are lower, but periodic maintenance is needed after the hyaluronic acid filler safely metabolizes, but it can last up to 2 years.
