    I really appreciate the before and after photos being taken from the same angle with the same lighting and no makeup. I see too many photos with the patient lit with harsh light from above, no makeup and looking down before, and softly lit from the front with their head up and full makeup after. It makes me wonder what the surgeon's hiding.

    Dr. Joe,
    One of the things I like best about your videos is hearing the obvious pleasure you get from your work. It's fun to hear you tell about especially good outcomes and some of the nice patients you have encountered.
    I can thoroughly relate to both feelings. Although I recently retired, I was fortunate to love my work in a challenging field, and enjoyed the pleasure of helping clients with tricky problems. Aren't we lucky?

    I would like to ask do you do face lifts neck lifts on patients who have APS
    ( anti phosphoid lipid ) I take warfarin but very rarely bleed . If anything I run on an INR of 2.5 to 3
    Previous PE 2016.

      I use 2-0 Vicryl on the deep plane SMAS and on the skin, I use 4, 5, and 6-0 gut. Thanks for watching

From what I've seen, you use your comprehensive facelift to achieve the most dramatic results possible, so I was surprised to see the patient at 13:36.. from the front she still has some pretty significant marionette lines/nasolabial folds postoperatively. Was it a difficult case, or were there other factors contributing to her outcome? Thank you!

      I try to add all types of cases, not just great ones. Not sure which patient, but facelifts don't necessarily do much for the central face.

      That is where we do brow and forehead lift, upper and or lower blepharoplasty, cheek implants, chin implants. Any of these address different areas of the central face.@@Corrans

    Dr, I have a question for you that seems important to me. Do you check for what reasons we get more wrinkles than others; like blood test to see where our estrogen levels and collagen levels? Do you think these levels make a difference?

      We do not test as aging is extremely complex and involves inherent causes (heredity), extrinsic factors (smoking, drinking, sun etc.) disease states etc. We treat the aging regardless of the causative factors. Thanks for watching.

    I´m facial plastic surgery from Brazil and apreciate a lot the deep plane technique. But in the major part of my cases i realease the ligaments under the skin and above the SMAS to ensure more security and perform a short flap with smas and platisma. The results is so good like these with more security in my hands. But congratulations for your job.

    Hi Dr. Niamtu, in this vid you mention that a facelift doesn’t really address marionette lines and the droop in that area of the face. What surgical procedure does address that (if any)?

      It does little for the central face, so nothing for brow, eyes, nose and around the mouth. Lasers, fillers, etc for nasolabial folds adn marionette lines.

    Do you do rhinoplasty? I feel like my nose has gotten bigger especially since crossing the big 50! Another issue I have is one eye appearing smaller than the other in photos and I seem to have a larger bag under only one eye! My symmetry does not translate on photos at all!

    Everything I've read suggests deep plane f/lift lasts longer than other procedures. This would be a reason I would go down this route as the investment is significant for me. Is this correct?

      There is no science that they last longer. I do deep plane, but any well done facelift with SMAS management should last.

    of hyaluronidase under my eyes. It dissolved even my own skin's hyluronic acid. My undereyes look extremely hollow now.
    Will I get my own hyluronic acid back? In how much time??

      The S lift procedures are usually for minilift situations. If you have a significant amount of neck skin you may want or need a more comprehensive lift. Kenny Rodgers looked strange due to his brow lift, not his facelift. I wish I had his hair :)

      It does help the smile lines and marrionnette lines, but only moderately. If you pull this area too much, patients look distorted, like the Joker.

      @@jniamtuiii Hello, sorry it was my usual gripe against the use of young perfect women to promote diets and face creams and surgery ! I worked in fashion all my life so perfect beauty was my constant ! I went through some terrible years then stopped looking or seeing myself so it was a shock to find out that I was no longer the young for my age, great body person I knew. Knowing I couldn't do anything about it, it got worse of course. Then I dieted, lost weight and moved to the south west of France and am decided to get back to a slightly better looking me, not a young version as that would be silly but something that matches my brain ! God bless (if we are still allowed to say that! ) x

    Why do you not go behind the tragus for incision as I see in nearly all of the CZcams videos I’ve watched? And your scars look beautiful basically undetectable

      That is because they don't show you the bad ones when the tragal cartilage collapses. Go ahead and Google "blunted tragus after facelift" and look at images. It looks horrible, you can see right in someones ear and there is no real fix for it. That is impossible with my type of incision. I did my first 800 facelifts with post tragal incisions and all I can tell you is that I get my most aesthetic results with my current incision. Thanks for watching. Dr. Joe :)

      I have noticed in so many after pics that the tragus is hidden behind the hair so that you are unable to see the outcome, whether good or bad. I have also noticed a deformed looking tragus on after pics. I think this plus scar visibility are things that would make me anxious.

    26:11 Why would it be “inappropriate” to make a woman look as young as plausible? I love to see when women look like their “moms” in the before picture & their “daughter” in the after. I never met a woman that stated that they love looking old-even in their 7th decade. I don’t mean to sound rude, I’m genuinely curious.

      You are correct, it is awesome to have a case where someone looks decades younger, but it really depends on the person. Your face has to match your body. Some patients would look very unnatural with tight skin, big cheeks and lips at an older age. We do our best to listen to the patient and match all reasonable rejuvenation.       I think that it looks weird to have an obvious facelift if you look old everywhere else. The other day I saw an old man with jet black hair. It looked weird, maybe his hair is youthful, but it was not matching his overall aura. I think that maybe he did not mean inappropriate, he meant I think make it look believable and natural.
      I noticed that too. When he said that, I thought , Why? but I think he is thinking big picture.

    Hi dr Joe. I have had a face lift. Its more than two montjs now. But there is alot of infection at different places on my face. On both sides of face on jawlines and on cheeks too. My surgen is a certified surgen and according to him this is the reaction of sutches under the skin.       I don't think there are any true studies to prove that. I think that most well done facelifts, regardless of the type have similar longevity.

    Deep plane is currently being marketed. It is true. Let's suppose that for whatever reason, a patient desires deep plane. And let's assume the surgeon is very experienced with smas, good reputation and getting very good results with smas. The surgeon has no experience with deep plane, but agrees to do the procedure because the patient requests it and the surgeon thinks he will get a good result. Is that acceptable? Or is it better to seek a surgeon with specific deep plane experience. What is your opinion on this?
    I had asked this on another surgeon's channel and he indicates that any surgeon who does smas well is also qualified to do deep plane. 🤔

    Q: Not clear on this. Aren't there many surgeons that have performed many smas facelifts but with little to no experience with deep plane?...       Lots of ways to answer this. A doc with significant SMAS experience should not have a problem with deep plane technique. But it varies from doc to doc. Deep plane is a good technique and I usually do it, but experienced docs with great SMAS control, get great results also.

    When I look in the mirror I no longer see anyone who feels like me so I want to see 'me' again, just for me, for no-one else. Next year I am going to do the full works, this year it's my teeth that need sorting then next year my face, the year after my tummy.

      @@jniamtuiii Thats great! You should really promote it. As I was looking through your before and afters I noticed the long upper lip often diminishes the effect of a facelift, as it's an underestimated but strong indicator of older age.

    Thank God someone tells the truth about deep plane facelifts. This is a procedure not widely accepted in plastic surgery circles for the last 20 to 30 years. For some reason, certain less than truthful surgeons are now promoting it as the best technique ever. Problems with the deep plane technique include:
    1. It has a fair amount of subcutaneous undermining so the claim that it is tension free on the skin is not true.
    2. It doesn’t address retinacular ligament extension in the subcutaneous plane except for the amount of subcutaneous undermining, which is limited.
    3. By itself, it doesn’t do anything to rejuvenate skin quality. The last two are clear failures to obtain optimal and durable results missing 50% of the potential result.

      Although I do a deep plane technique when indicated, I agree that it is not the holy grail. Show me a successful facelift surgeon that has done over 1,000 lifts with patient safety and great outcomes and I think they have it right, regardless what their technique. Deep plane is great in trained hands, but other SMAS procedures are also equally successful. Look for the experience and the result, not just the technique.

      You can get a cardiac arrest before a facelift too. Can a facelift cause a cardiac arrest is probably what you meant to say, and the answer is highly unlikely. I've never heard of anyone having a cardiac arrest because they did a facelift.

      Not all plastic surgeons perform cosmetic surgery. Cosmetic surgeons only perform cosmetic surgery and nothing else. There are excellent surgeons in both groups. I have a youtube video on the subject

    First, must define the word better. However, it is a fact, the most fundamental reason for sagging in the face is.....A weak structure, bone structure...Two, sagging muscles, ligaments tendons. If you do not cut out the excess tissue beneath the skin, the so-called face lift will last as long as the skin does not stretch. Could be one or two years cause the underlying tissue will pull the overlying skin downward, creating a droopy looking face again. Of course a more extensive deep tissue face lift is superior to a skin lift.

    Deep plane is more invasive and dangerous. Surgeon could hit a nerve and damage your face for good. I say a good facelift depends on your surgeon. SMAS, much safer with excellent results.

      In experienced hands, all facelift approaches are safe. You are correct that SMASectomy can provide excellent results. And yes, regardless of technique, the experience and competence of the surgeon is what matters most.

    Why are 95% of after photos of deep plane face lift procedures (regardless of the surgeon) shows MINIMAL results and most patients only look 5 yrs younger? Whereas, other forms of face lifts (using reputable surgeons) the results tend to remove 10 to 15 years younger appearance? Many deep plane face lift patient often look 80% the same as they did before. Out of all the after photos of deep plane face lifts are not that impressive vs other types of face lifts IN MY OPINION. There is an elephant in the room and no one is saying anything about it ! 🐘🐘🐘🐘

      It totally depends on the surgeon. A poorly done deep plane face lift is not better than a well done smasectomy or smas plicaion. You are correct that much of this is propagated by social media and many surgeons do very fine facelifts that are not deep plane however there are advantages to deep plane face lifts. it is more about experience safety and patient outcomes and each surgeon should do what produces the best outcomes and the safest manner
