I had the pleasure of speaking with ‪@dr.sergiovanogalvan6394‬ earlier this year, and he gave us some top-notch information regarding the safest protocol for stopping hair loss. No, it's not 'bludflow' or any other nonsense. It's tried and tested dutasteride. In a study published in the Journal of the American Academy of Dermatology, they tested various dosages of dutasteride and finasteride. They found that two capsules of 0.5 mg of dutasteride were as effective as 1mg daily of finasteride.
Furthermore, Dr. Vańo states that in his practice, they observed that patients who suffered from side effects with 1mg daily of finasteride did not have any side effects on low-dose oral dutasteride. Low-dose oral dutasteride is safer and as effective as finasteride in their practice.

Fin 1mg 3 times a week, or Dut 0.5 mg to times a week. Both work as well as every day. Fin 1 mg has a half life of around 2 to 4 weeks, Dut half life of around 4 to 6 months. It’s in the scalp tissue that we measure, not in the blood.

It's interesting. 8 years of Finasteride zero side effects. One cap 0.5mg DUT/weekly added and a noticeable numbness-ache in a certain area for months. After 6 months I chose life balance and stopped this pharmaceutical. I was very surprised by this effect tbh. Everything is case dependant.

I was the complete opposite. Very marked side effects on 1mg/day finasteride, none whatsoever on 2x 0.5mg/week dutasteride. With greater doses of dutasteride I did suspect that I was beginning to feel some sort of side effects, although they were really very minimal. Really my experience on the two drugs was like night and day. I actually think you touch on a somewhat neglected aspect of Finasteride side effects. Side effects like "low libido" or even "erectile dysfunction" are obviously undesirable but ultimately sound quite benign. For me, the real issue was more a general numbness and physical discomfort, just as you describe. It really was not just same as usual but with reduced libido.
I also had blood tests to confirm that DHT reduction was at least as great as 1mg/day finasteride. Of course with intermittent dosage the level will not be constant, but on average I was getting slightly greater reduction than with 1mg/day finasteride. Incidentally, the concentration curve is not linear; the average concentration will be higher than a simple average of the endpoints. This average is based on a model of the rate of change of DHT concentration enabling more accurate prediction of the concentration curve. Using the same model I would say that for most people 0.5mg/week is roughly equivalent to finasteride, and 0.5mg twice a week is better.
One question is whether taking an average like this is valid, i.e. do the effects of DHT on hair follicles vary linearly with serum/scalp concentration? The results in Olsen et al 2006 and Roberts et al 1999 suggest that it might be, although even this evidence is not completely satisfactory. Perhaps somebody who knows more about the biochemistry of the hair follicle can provide a reason whether it is likely to be or not.
As for the surprising differential in the side effects: complete speculation, but I have considered the possibility that allowing DHT to raise during only part of the week could be enough to satisfy requirements in other parts of the body. I really do wish to stress that this is completely baseless conjecture on my part. Indeed, equally spaced doses of 0.5mg dutasteride 3x or even 2x a week result in reasonable suppression of DHT throughout the week, so if these are well tolerated other mechanisms may well be at play. Nevertheless, possibly worth throwing it out there.

Half life of dut is dose dependent. Each capsule has a half life of 3 days . If you take it > 2x a week it I’ll build up to steady state and reach full half life potential. If you take 1x or 2x a week spaced out this will not happen.

Dutasteride also shuts down DHT production in the brain though, since it blocks the Type 1 enzyme which is all over the brain tissues.

There is literature attesting to the efficacy of dutasteride dilution delivered as mesotherapy (injecting the solution directly into the scalp, as is done with PRP) once every 3-months. This was shown to be highly effective at preventing hair loss and to have little or no effect on serum DHT levels. In other words, the drug seems to stay localized to the scalp.
The only problem is finding a provider who will perform the service for you. Mesotherapy doesn't seem to be a very popular practice in the U.S.
