@@ytsux9259 You'll need a prescription. Talk to a dermatologist and tell him you want to use dutasteride because finasteride isn't working well enough for you.
If you're having troubles then talk to a hair transplant surgeon as they're generally more liberal about prescribing dutasteride.

From what I remember, Merck tested a selective AR-1 inhibitor called MK-386 on stump-tailed macaques and found that it was ineffective against hair loss. So, yeah, it's mostly dutasteride's superior inhibition of AR-2 that gives it additional strength over finasteride.

Hi, what is your dosage? I just did my first dosage today 0.5mg dutasteride, doc also prescribed me 0.5mg daily. Never been on fin , 1,5-2 Norwood and some hairline thinning. But head full of big curly hair. Can't say I'm agressivly balding.
.. age 42.... I'm wondering if 3x weekly or even 2x is good enough, or just follows Doc's dosage

Glad to have you here choom! Nah, you don't have to be that strict. Finasteride has a short half life, only 5-8 hours so just take it daily at roughly the same time give or take a few hours.

time of day doesn't really matter because finasteride has a short half life although it is probably best to take it at least a roughly similar time give or take a few hours.
Both finasteride and dutasteride can be taken with or without food.
@@haircafekevin I think evening is best to optimise. Since the main testosterone boost is in the morning you'll decrease its conversion to dht.
Practically speaking though it doesn't matter.

Imagine you got a hungry cat at home who can't satisfies its belly even fed all day for a week ...you got a feeder machine capacity of 2 gallons ,which draws 200ml milk for your cat every time it asks for ,,, consider around 5pm it has malfunction so it draws 600ml milk at exact time on daily basis ,,by the end of the day hungry cat won't even leave a drop of milk and it would drunk 2 gallons of milk alone
Same way the frequency is ED or EOD so it doesn't matter when you take the pill, DHT will be fucked all the time whenever it kicks in .. don't think im crazy

Thank you, yes I have used it and I got comparable results to finasteride. Only difference is dutasteride gave me an increased libido which was annoying. I dropped it years ago.
Hey Kevin, med student here, I just finished my clinical rotation at the department of dermatology. A very knowledgable dermatologist in training told me that there is a shift in the consensus on what causes hairloss. Of course, androgens play a huge roll but he said that the thining of the skin of the scalp area, especially in the frontal and vertex area are what causes dht to miniaturize the hair follicle due to a yet to be elucidated mechanism. He said that this is further backed by research on hair transplantation data which shows that despite transplanting ''dht insensitive'' hairs to the frontal and vertex area, people still experience hairloss of these so called dht immuun hairs. Moreover, the fact that older people have less hair doenst make sense if DHT is the sole reason for hairloss since older people have lower test and therefor lower DHT levels than say a 25 year old male. Therefor, he advices to start with minoxidil in the case of young man and in conjuction adding finasteride. This also explains why some people lose hair despite being on fin for more than a year. What are your thoughts on this? I know you are always mocking the bloodflow theory, however this mechanism is different from the bloodflow theory as it pertains to scalp skin density and traction. Love your content by the way and I am seriously considering Fin due to your eloquent video's and your use of scientific research instead of anecdotal evidence like other YT'ers!
This scalp tension theory is another variation on the blood flow theory, it just doesn't involve blood flow anymore because that aspect has been debunked. I have read the arguments that there are some cases of failure of the "donor dominance" rule during hair transplant, but I think these occurrences are more likely due to problems with transplant technique, because in the vast majority of cases donor dominance is the rule, and hair follicles maintain their DHT sensitivity regardless of where they are transplanted to. As far as older people having more hair loss, androgenic alopecia is a chronic condition, and the more hair cycles that we go through the worse it gets. Testosterone levels don't fall that much in men with aging, and even women with much lower testosterone levels than men can get androgenic alopecia. The problem with androgenic alopecia is not too much testosterone, it is overactive 5AR enzymes and androgen receptor sensitivity in the affected hair follicles. There are a lot of things going on with aging, and almost every medical condition gets worse as we age, and hair loss is no exception. So I still don't buy that there is some "Factor X" related to scalp tension that is the cause of the balding pattern seen in androgenic alopecia, and I think even the dermatologist you quoted would admit that there is a large dose of speculation in these scalp tension theories.
That being said, glad you like my videos, and I'm happy someone studying medicine still finds them of interest.

At the end of his Finasteride Vs Fluridil video Kevin mentioned that lesser known treatments aren't taken as seriously and therefore don't leave a strong impression even if they give results. Conversely, finasteride has a strong placebo effect because it's so well known and researched and even minor results on fin will be taken as resounding victories

Have a question. If scalp dht is inversely correlated with hair counts, not to say it’s not relevant, why is there less of an increase at 24 weeks vs 12 weeks for 2.5 compared to 0.5. Is it possible that scalp dht reducing to just 50% can have almost the same effect as a 79% reduction in scalp dht over a few years since the dht in the hair follicles would be close to 0% for both ? Trying to understand as I posted a theory on your most recent video

But now i'm seeing online that we shouldn't split film coated tablets as that may be dangerous and/or reduce the efficacy of it. Do you have information on the subject ? Thanks man(n)
Nah, that is bogus. The film is to protect women from skin contact since the drug absorbs through the skin. It has nothing to do with efficacy.

If it’s not caused through increasing dht, then taking dut or fin wouldn’t cancel out any Kratom related hair loss I would assume. Are the people claiming Kratom causes hair loss just experiencing regular age related androgenic alopecia and just blaming the Kratom? You bet! The Kratom subject is extremely mixed in terms of research. I'll probably get to it eventually but right now its on the back burner. Definitely an interesting subject though.
I've heard anecdotal stories about kratom causing hair loss. I haven't seen much in the way of actual medical literature, but I've read enough anecdotal reports to think there might be something to it. It likely wouldn't be due to DHT, as it appears to occur in random patches, as opposed to the typical course of MPB.
On a side note: please don't take kratom recreationally. I can't tell you how many people I've talked to who thought it was a safe, non-addictive herb, & ended up having a h3ll of a time trying to come off of it, replete with severe physical & mental withdrawals. It's very pernicious in that it could be a while before you realize anything is wrong, but it does eventually sneak up on a person if it's used daily over the long-term. People will often dogpile you if you speak negatively about it in any way, but the reality is that kratom is essentially an opioid in how it affects our brains, & thus, is habit-forming in much the same way that traditional opioids/opiates are.
I'm just telling my regime , I use topical fin with minox 5% once/day and use dut once every one or two weeks , Sundays basically

1. There is this Doctor named Amiya Prassad I think, and he promotes this treatment called PRP + ACell treatment or AKA TrichoStem®, and it sounds cool to me judging from the results I keep seeing. I was wondering if you could look into this and see if it is legit or if its just a cash grab. Some people will like something like this more than Minoxidil due to just getting a shot regularly rather than doing Minoxidil daily plus it seems to have the effect of rejuvenating miniaturized hair.
2. Fortunately I live very close to a dermatologist office, and I am considering going there to get either Finasteride or Dustasteride prescribed to me to preemptively prevent loss early. What should I expect? How should I go about this? I am rather young, never got a prescription ever in my entire life or have done anything medical on my own.

The best french youtuber has like 17k subscribers and he can get away with a video where he tells people that minoxidil has an "indirect DHT blocking effect". However, channels bragging about hair transplants are blowing surprisingly. J'ai commencé le minoxidil à 20 ans (ça m'a aidé mais n'a pas arrêté la chute des cheveux) le minoxidil a seulement ralenti mon alopécie. Il y a 3 ans j'ai fait une greffe de cheveux (1200 unités folliculaire) qui fut un succès. Je suis passé de Norwood 3 à norwood 2 grâce à la greffe il y a 2 ans.
Et depuis 1 an j'ai ajouté finasteride à mon régime (j'ai jamais arrêté le minoxidil bien évidemment car cela fonctionnne) . Et aujourd'hui à 30 ans j'ai presque les mêmes cheveux que j'avais quand j'avais 18 ans !!! (J'ai juste mes golfs légèrement creusé).

Yo bet! Since you are kind of getting into the game late I would say it is fine to start dutasteride right now, 0.5 mg daily. If after 6 months you are still losing ground then I'd consider 2.5 mg of dut.

Hello Kevin, I’ve been taking finasteride for 2 years 3 times a week and switched over to Dutasteride 3 times a week for almost a year now after getting a second hair transplant. I still have diffuse thinning around my crown area. My hair transplant doctor told me before to take Dutasteride daily for a while first then do 3 times a week later on. I did not follow his advice. Would taking it daily now make a difference or would I have reached the same level of efficacy now that its built up in my body for over almost a year of taking it 3 times a week?

kevin i got a question for . do you think dut could be helpfull for acne and even skin aging ? since 5 ar1 is predominant in skin tissue if i'm not mistaken maybe it has an effet on acne and especially in skin aging ( i read a paper on dht causing elastin loss hence giving wrinkles and some product that were trialed to topically reduce dht on the skin were supposed to be good for skin aging). what do you think about that theory? thanks
It's a good question. You would think that dutasteride would be effective for acne, but a study published by Leyden in 2004 using a different specific type I 5AR blocker found no benefit from it in acne. There are some case reports of dutasteride helping acne, but no good clinical study showing it works. It may be that DHT isn't as important in acne as was originally thought, and maybe testosterone and the testosterone precursor, DHEA are more important in causing acne. Regarding 5AR blockers and skin aging, the data is really preliminary and not good enough to know if they really do anything for skin aging. It would be nice if they worked for this, but we'll have to wait and see what the studies show.
Is there a way to prevent any effective hairloss drug (fin/dut) from going systemic? I would love to use a topical formulation but I want to prevent any of it from leaking into my blood stream as I think my body is sensitive to fluctuations in DHT? Any methods/techniques/dosages I can try?

Finasteride can take up to two years before you see its full benefits.
Broccoli would be way too nuclear at this point. I'd try minoxidil first or if you want to just stick with 5AR inhibitors you can switch to dutasteride. If you don't notice any benefits beyond finasteride you can just switch back and not lose any ground.
Hi Kevin, I’m currently taking finasteride and I was thinking to adding dutasteride just to give an extra boost to my hairs before having a hair transplant. Does it make sense for you to take both of them or just switching to dutasteride 0.5 mg would be enough?
Either will do. Adding dutasteride once or twice per week with finasteride on the other days is a good way to get a bit of extra dht suppression.
Hey Kev, i think I might be experiencing a bit of a shed on my hairline after starting dut a month ago. Is this to be expected? especially, considering I didn’t shed at all on finastride(it worked well, but lost efficacy over time)

But curious if you could point in the direction to find out info on this topic, so I was previously taking minoxidil for over 2 years (both 5% and 15%) and never experienced any side effects. Now I'm taking topical finasteride (0.1%) combined with minoxidil (5%) and have noticed weird heart palpitations/constricting feeling. I'm likely going to see a doctor to rule out any underlying issues but wondering if you had any thoughts on why this could be happening?

Hi Kevin. I'm glad that you have made a video about dutasteride. It's such a shame thought that only one country approved this drug to be officialy used to treat AGA. By the way, you said that dutasteride dosn't have generics, but in the country where I live there are generic versions of Avodart and they cost the same as generic finasteride. Oh, and I have some questions:
1. Do you think that dutasteride will be FDA approved for treating AGA in the near future?
2. Why do you think there are many doctors/dermatologists that fearmonger about finasteride/dutasteride? In the country where I live (especially in the eastern part of it, but this occurs very frequently in other parts too) many (if not even most) dermatologist talk how bad finasteride, dutasteride or even minoxidil are or do not even know that they are used for treating AGA. Additionally, they think that some vitamins, shampoos and herbal-based topicals are effective ,,treatment" for hairloss. I mean, shouldn't they be taught about how AGA works and what is effective in treating it and that fin, dut, min and such have a high safety profile? Because now it looks like they receive their ,,knowledge" about AGA from - I dunno - some witch doctors/shamans, sites like propecia help or just some gossips.
3. Can minoxidil be used less frequently after it succesfully regrow hair (and after about - lets's say - one year) just to maintain the results? For example once or twice in a week?
I agree it's a shame it has not been officially approved for AGA, but it doesn't matter too much, since it is available and can be used off label. I don't think anyone will bother to apply for FDA approval now that it is off patent. Regarding fear-mongering, I haven't seen that much among doctors here in the US, but it must vary in different countries and I don't really know why. Maybe the medical educational system is not as good in some countries? I don't know. There are a lot of "alternative medicine" providers here, but they are usually not medical doctors. They are chiropractors or naturopaths, though admittedly there are some high profile real doctors who are quacks like Dr. Oz that make money selling supplements.
Regarding minoxidil, most people seem to lose ground if they stop it, so I think you can possibly taper it a little, but once or twice a week may not be enough to maintain its efficacy.
Dutasteride is OTC at any pharmacy where I live. Approved specifically for hairloss or not, doesn't matter much. Maybe it's a good thing, as demand would rise with awareness, increasing prices.

0:51 A minor correction here: Hypertrophy is not the same as hyperplasia. In hyperplasia, there is greater cell proliferation than normal. In hypertrophy, individual cells grow in size (this is what happens when the liver "regenerates"). In BPH we have hyperplasia, not hypertrophy.

Yo! No, hats cannot cause hair loss. That is unless you wear them really tightly in which case you may give yourself traction alopecia but that should reverse on its own. I may cover alfatradiol again although there isn't a huge amount of data on it. It is an adjunct treatment with mild effectiveness but certainly not a replacement for finasteride.
Hey Kevin im on Finasteride 1mg EOD for arnd 6-7 months now and i think it has accelerated my receding hairline. No shedding just increased miniaturisation... Any ideas why? I know its working because my hair fall has reduced. But still my hair is minaturizing at an increasing rate. I know its the finasteride because before this my hairloss wasnt so aggressive before. Great video. I’ve just added another day of dut to my stack. Fin 1mg on Mon Tue Thurs & Fri. With Dut 1mg on sat, 0.5mg on sun & Wed. Carry that on for another month then add dut another day. Would a month be ok?

Kevin I'm a bit confused. In the previous video, the optimal dosing of finasteride, the study claimed that scalp dht dropped by 64 percent using 1mg and in this video it says that 5mg only dropped scalp dht by less than 41 percent? Please clarify this if you can.
Different studies with different methodologies often give different results. Scalp biopsies might give different results depending on the location and size of the biopsies in the different studies. Also in the study with the 64% drop the biopsies were taken 42 days into the study, while the other study did biopsies after 6 months, so it's possible that there is a greater initial suppression of scalp DHT with finasteride that tapers off over time due to up regulation of the 5AR enzymes. So I don't think these studies are in any way contradictory.

What about combining Finasteride and Dutasteride? I know several doctors who recommend something like 2-3 days a week of Dutasteride 0.5mg and the other days Finsteride 1mg.

@@arigold418 I am not a doctor, but if you want quick results I would go with a daily dose for a few months at least. Then maybe combine it with Finasteride it just take it fewer days per week. My guess is that 3 takes of dutasteride per week are slightly better than Finasteride every day.
Hey Kevin. Got a question. Why did I respond to dutasteride and oral minoxidil on the first 8 months but after that I'm slowly losing ground again? If testosteron is not the culprit, I don't know what could it be. I've checked my vitamins and thyroid and all seems to be okey. I also shampo with ketoconazole 2 times per week to reduce inflamation and take MSM supplements every day.
Hair growth is cyclical because it is impossible to permanently keep hairs in the anagen growth phase. Even people without androgenic alopecia go through seasonal shedding phases. Self assessments aren't really a good metric to assess progress anyways. I'd go to a university dermatologist and get a phototeichogram and then do a followup 3 months later. I seriously doubt you are losing hair on dutasteride, at least not in the long term.
On the off chance you really are, dut can be safely used up to 2.5 mg daily.

No, it is about genetic sensitivity to dht and that varies from individual to individual hence why people who have low androgens can still have hair loss.
There is no blood marker that can predict how well you are going to respond to finasteride. You've got to just use it and see what happens.

is better to take 0.5 fin everyday then 1 mg every other day. that way is more effective and ur safe on the side effect profile.
Hi Kevin, I see multiple sources saying that it is ok to apply minoxidil only once a day, but they never seem to mention how much you should apply. Since the recommended dose is half a capful twice a day, should I then be using half a capful once a day or a full capful once a day?
Once a day is fine because the topical has a 23 hour half life. As for how much you use, that depends on how much coverage you need. If you are only applying to the problem areas then a capful or half a cupful should be fine.

Sadly, minoxidil is the best growth stimulant so there are no replacements that are as effective but if you want a growth stimulant that is different and still helps a little bit then I'd try stemoxydine.
Hello, I've previously commented on your channel and stated that I have severe hair loss for a 16 year old (diffuse thinning NW6 pattern, hairline NW2). No, I went over this in the video. Testosterone doesn't cause hair loss at a normal physiological level. The studies showed greater hair count growths with dutasteride even though the testosterone was higher.

You may be aware of this. The history of Finasteride/AGA. Merck developed the drug because of the discovery that people with 5AR2D were never affected by BPH -/AGA.
They have the same T level production and, (don't "miss") with the 5AR2D (T to DHT conversion deficiency) - significantly higher "scalp T", but again, they never bald.

No, you are confusing serum DHT with scalp DHT. 0.5 mg of dutasteride suppresses about 50 percent scalp dht. You can try using it once weekly with finasteride on the other days to get an extra bit of dht suppression.
I've been on dutasteride for 2 years, it filled in my crown really well in the first 18 months. The past 6 months the hair on my frontal scalp has really started to lose density. I've Increased the dose to 1MG daily up from 0.5mg

In the dutasteride study it shown daily 0.5mg to decrease scalp DHT by around 50 percent, & daily 2.5mg to decrease scalp dht by around 80 percent. The results also shown the 2.5 mg to give moderately better results over the 0.5. But of course this then increased side effects, I'm not going to go crazy and take 2.5 daily but I'll stick to 1MG & hopefully gain stability, if not I'm throwing the towel in
The studies you're citing are on finasteride, not dutasteride. Dutasteride at 0.5mg reduces dht by 95%

I don't know about other people taking duta, but I've found that taking it every third day doesn't seem to work for some reason.
EOD on the other hand seems to work perfectly, it's the dose I got most of my regrowth at. I switched to every third day twice and both times I had shedding that stopped within a month and a half of returning to EOD.

One of my concerns is the by the time you know finasteride isn't as effective for x individual, wouldn't changing to dutasteride be a little too late in saving the follicles you lost?
Also looking at the side effect profile and % chances, I can't see why someone wouldn't start dutasteride after a month or so of being on finasteride and them having no adverse side effects. Other than dutasteride not being FDA approved ofc. Have you done videos over Korean/Japan literacy/studies of their androgenic alopecia regimen?

Would be great if you could do a video on topical dutasteride. I was still seeing hairloss on finasteride so I hopped on topical dut from strut. So far I've noticed I have a stronger sex drive and less sides then fin (never had bad sides on fin, just slightly lower libido and maybe higher body fat from increased estrogen). I've only been applying twice a week, haven't noticed any difference in my hair yet but I've only been on it for a week.

Im on dut for about 5 months , prior to dut ,i was on fin 1.25 mg/daay for about 8-10 months , but i ran out of because of prescriprion , and i said 'lets hop on dut ' i havent had any sides with neither of them besides my beard .I ve noticed while on dut ,my beard got thinner and it takes much longer to get somehow thicker ,as far as improvement of my hair , i cant say dut was more efective than fin,and im thinking to go back of fin mainly because of my beard aslong as i didnt see much improvements while i was on dud.What do you think?
Yeah if you haven't had any benefit from dutasteride over finasteride and you are having problems with beard growth, it sounds reasonably to go back on finasteride.
