Great video with lots of effort put in. It was good to see you on the beach and your son showing interacting. This could be one of the best videos you've done! As for another lip surgery, you'd be a good candidate for the Plasma Pen Lip Flip (despite you not being a fan of this device). As it's comparable to the CO2 laser, subtle looking like there's 0.5ml of filler, it needs to be done about 3 times with 6 - 8 weeks apart, comparable to the CO2.

Yes Ellacor will help both smooth out lip lines and shroten the philtrum as well. I do not believe the Nira would be strong enuf to make those changes but I do not have data to back that up.
What do you think about the lip lift procedure where the skin is removed from the outline of the lip versus under the nose? Do you think scarring from that particular procedure might actually be flattering, where scar tissue might offer a bit of a volumizing effect almost like lip liner?

Thank you!!! It depends on the texture and quality of the skin. Fraxel is my favorite but if there is a little more severity to the texture or loss of elasticity a CO2 is a little stronger and may be better. Hope this helps!!

Hi, thank you so much for all this information! As you mentioned Thermage, it made me think of another topic I've been looking into. Would Thermage be a good option for treating malar edema? If not, what might other options be? Not sure if it merits a whole video but as it's super off topic for this video, maybe something to consider for another time if the answer is not simple?
Thermage can help reduce the appearance of malar edema and I've seen this in many of my patients. Although it does not address the cause of malar edema directly, by tightening the skin on top, everything just looks better. If that makes sense?
@@DrStefaniKappel Actually here's one more for ya, and this one is on topic (mostly)! Regarding treatment for gummy smile - as I understood it, Ellacor/Co2 are good alternatives to lip flips, so they would help create more lip show when smiling and reduce the amount by which the lip curls under when you smile. What would be a good non-surgical alternative to botox treatment for gummy smile (if any)? I ask because I tried it once and I felt like it lengthened the distance between the nose and upper lip at rest, and I didn't care for it.

As I always say, it is important to get clearence from your primary doctor who manages your condition. In this case with Graves disease, it would be your endocrinologist. HiFU should not be contraindicates from a dermatology standpoint but it would have to be approved by your specialist because any medication that could possibly interfere and intricacies of the endocrinopathy that are beyond the scope for dermatology should not be overlooked. Also, I am unable to give medical advice over social media but wanted to answer your question as best as I could given these limitations. I love Paris!! One of my favorite cities :)❤❤❤

Hi! I got a lip lift and rhino then went back for corner lip lift. Well the sides still go downward. I have a strong middle area is that the cupids bow? Anyways im going for filller for the first time in the lip. 1ml and i have had 1 ml in the cheeks. 0.5 in each cheek. Anyways ive come so far in the mouth area had jaw surgery. Do you think late 20’s between early 30’s is too early for lip lift? I’m not sure if should wait till my 40’s. I have a long philtrum but also have a small forehead and the philtrum prob balances my face out from being too short and wide. I’m sure with time it will be far too long.
Doctor, what is your opinion on Glutathione injection or IV for skin lightening? I know many practices do it, but interested in your take on it...
I think as an antioxidant it is a great active for the skin. I think it’s important to not overdue IV however because over time it can cause vein scarring over time so when these comes a time when you really do need an IV it’s fed to access the veins.

I would ask your surgeon. Sometimes the surgeons who are best at lip lifts are not great at rhinoplasty and vice versa. Get a few opinions.

👋 I’ve been looking into microdosing accutane, because I think it could really help with a reoccurring problem I’m having. Of course I’ll also consult with my prescribing physician, but I’m wondering what that might look like. (Also, my physician isn’t a board certified dermatologist.) I think you said you do 20 mg a month. Does that look like 20mg once a month or 10 every other week? Do you still have to have your liver checked when it’s that low if you don’t have any applicable risk factors? And can you stay on that low of a dose if you’re pregnant or doing laser or electrolysis procedures? Actually a microdosing accutane video might be nice. :)

I'm currently not on it but over the years I have microdoses accutane and LOVE the antiaging effects it has on my skin. Typically its 20mg a week or every other week or once a month as many of my derm colleagues do. I do get my labs checked but it is not necessary if it is a low dose and you are otherwise healthy. I'd follow up with your prescribing physician however.

I looked into this after seeing my plastic surgeon collegues post impressive before and afters. It does require a cannula (like liposuction) and heavier anesthesia which is usualluy who its performed in the OR along with surgical procedures done by plastics. I think its a great treatment however.

Dr. Kappel, I m in my early 40 and started my up the hill battle with nasolabial folds and pre jowls. When I look at your video from 5 years ago I see myself now and your most recent videos of you are showing an amazing, plumped and snatched skin around that area. You mentioned doing Thermage and Fraxel but you have been doing that since your 20 and 30 and I don’t understand how you are seen with folds and slight prejowls at your 40 and then at your 45 they are gone and you only have been doing Thermage, Fraxel and Botox in that area only according to the videos I have watched? I’d recommend Thermage or threads. Ellacor can’t be performed on the forehead currently because of the depth.
Hi Stefani 💕Are all sun visor face shields all the same for uv protection? I see ones priced at like $18 and $70… so confused on this.
She recommends Mineral Sunblock, doesn't matter spf. She says to do 8% or more for the active ingredient in the mineral sunblock (ie: zinc oxide)...
Yes I believe sum shades are really useful but they shouldn’t replace sunscreen. Remember UV light can reflect off the ground too so even with a sunshade, back up with topical sunscreen is important :). And Heliocare if you want extra protection (ie if you have melasma, etc) Hope this info helps! You can check for the sunshades approved by the Skin Cancer Foundation but you shouldn’t have to spend a whole lot of $$$

OP, please don't claim to represent 90% of people. There are plenty of people, that love this non sponsored content and the way she shares her personal knowledge and experience on a large breadth of topics related to her specialty in a personable way. There are also many people that can afford a procedure like this that costs less than a few iPhones. The Dr does have many videos on what you are asking for.
I apologize this is off-topic but I live in Dallas TX and we have many of the newest machines that come out. The Neogen PSR seems very promising. It is a nonablative laser with less downtime than the Co2 and better results than the Fraxel. Could you do a review on this one? Another one is the fotona 4d laser. This laser improves the nasilable folds by using the device in your mouth. This one seems to good to be true. Like all the energy devices people have reviewed fat loss. This may be very dependent on the technician/dermatologist's experience, not the machine.
Both are very operator dependent but you have some great dermatologists in Dallas so you should be in excellent hands. I am not too impressed about the Neogen. Its not talked about much at out laser academic meetings ( when something is giving impressive results you''l hear about it at the cosmetic meetings among the experts) Don't fall for marketing overstating claims or else you will be disappointed. I think Fotona can be a good option as its an Erbium laser with impressive results but go to an expert to assure you get the best results with minimal complications. Good luck and I hope this helps!!

Is it possible to remove small red spots on the lip and vermilion boarder?
Yes you can use Vbeam laser (or any other laser where hemoglobin is the chromophore like Exel V, KTP, etc) but the trick is that you can only remove the red spots (which are most likely broken capillaries) on the cutaneous (skin colored) part of the lip. You cannot use these lasers on the red part of the lip because it may permanently discolor the red part of the lip. Does that make sense? So you have to be ver careful with Vbeam around the mouth for that reason. But yes it works great!

I would be interested in seeing the process of ellacore and a lip lift. I was born with a long philtrum and remember kids teasing me (cause it kinda resembled a chimp's mouth). Now that I'm 59, it has gotten longer. I've made peace with it but am still curious on the before and after.

Was your lip lift only skin or muscle lip lift? I had a bad rhinoplasty and I'm saving to get a revision, my philtrum does look longer and I will probably get a lip lift in the future.
It was skin only which is possibly why the results didn’t last as long?
@@DrStefaniKappel I never had a lip lift it's something I'm considering getting done because after my rhinoplasty, my philtrum looks longer. I had a rhinoplasty many years ago with Dr Michael Elam from Newport Beach and I'm not satisfied, my nose doesn't look natural 😢. Anyhow, I read that there are several types of lip lifts like skin only, muscle, lip corner lift, and so on. I was just wondering if the muscle lip lift was safe and if it lasted longer than just skin lip lift. Is there a good surgeon that you recommend here in California?

Lip lifts are one of the most anti-aging cosmetic procedures out there, if not the most out of them all. So for old age individuals it makes wonders most of the time. In young people, it’s a double edged sword though. Not all young people will get the best results: some might end up looking like some kind of cartoon character and will regret it forever while others will absolutely love it. Be careful, and consult with a plastic surgeon who will do the right calculations in order to achieve harmonious facial proportions, and be honest to tell you “NO” when you don’t need it. I have seen very awful cases in which the subjects got their face ruined for nothing, as their surgeon was not honest enough to tell them that their nose mouth gap had already the right length.
This is unrelated to this topic but, does Heliocare block red light therapy treatments?
That is a good question and there is no way to really know if it does. I do not believe that the polypodium leukotomos in helio care will protect it from any insult from red light but it may repair some of the changes that are induced by it. There are no studies or data to show this but from a physiologic standpoint, that would be my guess.

Not related to lips at all, but I have really deep 11 lines that botox will not help. What's the best thing to do in a case like mine to diminish those? I'm really tired of looking tired or angry when I'm not. 🙂

Yes I only had the skin removed, which is why I don't think my results lasted as long. If I have a second one, I may go to a surgeon who takes muscle and skin. I'll vlog it for you guys.

Only one! We recommend three but the two before and afters I posted only had one Ellacor treatment. And yes for the younger patient I also did Ellacor under her chin.

I asked my plastic srgn he said he didn't want to do lift on me because of scarring could be bad. I do have brown skin but after deep plane face lift i have no problems with scarring. Maybe i should get second opinion
He may not be comfortable doing it which is why he talked you out of it. Plastic surgeons do that a lot unfortunately.

Based on me reading inbetween the lines regarding that other service that I shared with you....
It appears to me that it's simply greedy capitalism. Where that in particular Clinic just rename Thermage to present it as something new and more than likely charge a ridiculous price point for the service.
Is it safe to assume that is your viewpoint as well based on my question being deleted?!?.....

@DrStefaniKappel, After further research and watching comparison videos this evening in my spare time - it appears that Sofwave is moreso comparable to Ultherapy (not Thermage).
My apologies for my lack of knowledge. I am just now looking into different laser-type options.
In closing, since I value your honesty Dr. Stefani, my questions now would be - is Sofwave truly safe for skin of color?!?....
And/or, would you still recommend Thermage over Sofwave (especially for those with skin of color)?!?....

Yes, yes! I won't say who but I seen a recent pic of this girl I grew up with she's been beautiful her whole life but her philtrum is super long and it aged her face so bad. I use an app called B 13 to fix my philtrum it's not bad but once I do my jawline I'll get a lip lift.
It’s so interesting hue it changes the whole facial aesthetic
@@DrStefaniKappel yes it drags your face down because it becomes a focal point to the eye but I'm 41 I'm planning to do it next year. My jawline is needing more symmetry and strength.

Do all surgical lift lips expose more of the gum line when a person smiles? Having a more gummy smile is not desirable.
I believe it can but most people with a gummy smile do not typically have an elongated philtrum.

I recall seeing a YT channel where the woman had lip lift surgery and had an EXTREME allergic reaction. to the anasthetic. Her lip swelling was insane. The poor woman.
Thar can happen with any procedure. Even just going to the dentist. A lidocaine allergy is actually very rare. It’s usually more a hypersensitivity to the epinephrine that is more common

Kim natural face was fine , when she started messing with her nose and trying to make it a different shape his when she needed a liplift. But her nose and the ratio to her upper lip was just fine before all this nonsense.
Unfortunately us girlies with upturned noses make the philtrum look more visibly long :(
