Premenstrual dysphoric disorder (PMDD) is a mood disorder characterized by emotional, cognitive, and physical symptoms that cause significant distress or impairment in menstruating women during the luteal phase of the menstrual cycle. The symptoms occur in the luteal phase, (between ovulation and menses), improve within a few days after the onset of menses, and are minimal or absent in the week after menses.[1] PMDD has a profound impact on a person's quality of life and dramatically increases the risk of suicidal ideation and even suicide attempts.[2] Many females of reproductive age experience discomfort or mild mood changes prior to menstruation. However, 5-8% experience severe premenstrual syndrome causing significant distress or functional impairment.[3] Within this population of reproductive age, some women will meet the criteria for PMDD. PMDD is most common from age 25-35, but can occur anytime during a woman's reproductive years.[4]
The exact cause of PMDD is currently unknown. However, because the symptoms are only present during ovulatory cycles and resolve after menses, it is believed to be caused by fluctuations in gonadal sex hormones or variations in sensitivity to sex hormones.[5]
In 2017, researchers at the National Institutes of Health discovered that women with PMDD have genetic changes that make their emotional regulatory pathways more sensitive to estrogen and progesterone, as well as their chemical derivatives. The researchers believe that this increased sensitivity may be responsible for PMDD symptoms.[6]
Some studies have suggested that those with PMDD are more at risk of developing postpartum depression after pregnancy, but other evidence has been found to suggest against that notion.[7] PMDD was added to the list of depressive disorders in the Diagnostic and Statistical Manual of Mental Disorders in 2013.[1] It has 11 main symptoms, and a woman has to exhibit at least five to be diagnosed with PMDD.[5] Roughly 20% of women have some symptoms of PMDD, but either have less than five or do not have functional impairment.[8]
First line treatment for PMDD is with selective serotonin reuptake inhibitors (SSRIs).[9] Hormonal therapy with oral contraceptives that contain drospirenone have demonstrated efficiency in reducing PMDD symptoms as well.[10] Cognitive behavioral therapy, whether in combination with SSRIs or alone, has shown to be effective in reducing impairment.[11] Dietary modifications and exercise may also be helpful, but studies investigating these treatments have not demonstrated efficacy in reducing PMDD symptoms.[9]

@@sophiaclark376 It’s a very sensitive line between anger and rage. I’ve had anger issues since I was little but the older I’ve gotten the more it becomes rage. I’m on a plethora of meds for keeping it in check but that week before my period it can be bad. It’s very hit or miss as it’s not ever month.

I don’t even feel angry or violent when i’m going through this. Just hopeless, on auto pilot, anxiety through the roof and sometimes suicidal thoughts too. I isolate myself and my work performance drops. It’s 10 days of hell and when I’m in the middle of it, I feel like I’ll never feel normal again

Yeah same 10/11 days of hell happens every symptoms of pmdd anger, irritation, cry, mental anguish sad depressed social insomnia, over sleep mental fog and can not function normally to normal task, muscles ache like hell and I'm actually in the middle of it feels like this will not end, you become so hopelessly helpless, tried anti depressant, hormone tablets now I'm thinking may be removing my ovaries and uterus, my heart goes out to you ladies who suffers pmdd please take care❤❤
I have had this for several years. Finally found treatment to work. I take Fluoxetine 2 weeks before my period and 2 days after My period. I am on Cymbalta for fibromyalgia and take flexiril as needed. I do exercise at least 3 to 4 days a week. Eat healthier, go to my chiropractor for back , neck adjustment, PT, and I get acupuncture 2 times a week. I also use, my at home infared sauna. Seems to alleviate my symptoms of PMDD. I also am a Christian, so I do prayer and go to church.

PMDD is the worst! Everything aggravates me, and I am hurting my love ones by my horrible behavior. I just want to cry, I get extremely sensitive. Need lots of prayers during this time because I can’t even recognize myself.

I recently tried the cyclease pms medicine and it has helped me a lot. Usually I’m sensitive and get really frustrated and the pms medicine makes me feel normal within 30 mins
So relatable. I’m the same. I’m always so teary, sad, angry and hardly understand my self. I almost feel possessed and completely different from my normal self.

You are definitely not alone, Im in pain 2 weeks of every month. I started doing yoga on youtube, it seems to help with lifting the mood. I do yoga with Adrien she is very good, 20 mins or so a day makes a world of difference. xxx

Me! I’ve always struggled with a mood disorder…. I have a long list of mental health issues but the last few years (like 1-2 )years I’ve gotten better … less crying and panic attacks, less paranoia attacks BUT … I always start to get bad again , anxiety picks up I start feeling suicidal and like I’m starting to go crazy again… I look at my birth control pack and ahhh yes , behind I’m on that last week of the packet right before the period….
How are we supposed to cope with these feelings no s when we KNOW they will come every month no matter what?
Not suicidal thoughts for me. I believe the soul lives on after death. I just want to stop existing, but since my soul is going to continue to exist anyway, then I don't have a choice. If I have to exist, I'd rather exist in Heaven than in Hell. So, no suicide for me. Just sometimes praying "Why do I have to be here? Can't you just let me die? Why haven't you come back yet Jesus and fixed this broken world?" I know God has a plan, but the anguish is still felt.

I think this is a mild description of PMDD. My experience during the luteal phase includes debilitating pain where I cannot function at all at times- even with proper nutrition & exercise. I would love to remove my ovaries but there are so many risk factors to that. Sending love to those affected by PMDD.
Try antihistamines, more specifically Claritin.
The only thing that’s actually helped me with PMDD is taking antihistamines 10 days before my period. I take one tiny generic Walmart Claritin in the morning and off I go about my day. I would say that my PMDD symptoms improved by 99%.
The other 1% is just me feeling slightly tired and very mildly off.
There is evidence that high histamines in the body is associated with PMDD.
Many women have seen great improvement with PMDD when they took Claritin.
I bought the generic version Claritin from my local Walmart, and it’s changed my life.
My extreme PMDD went down to very mild PMS. My biggest fear was the suicidal thoughts I was having.
My PMDD got worse after my 3rd child was born, and I remember thinking this is not me, I can’t recognize myself anymore. This person isn’t me. Something overtakes you during that darkness, it’s like you’re in a dark hole with no way out.
I knew that I was chemically off because I’m generally a very happy go luckily person until PMDD takes over.
So I did a lot of digging and came across a Reddit where women reported great results with taking antihistamines.
If I didn’t track my cycle, I wouldn’t have even know in was in the Luteal phase of my cycle.
If you take antihistamines and it helps you, please pass it along to other women. So many of us suffer is the most brutal ways and I want to help everyone.
Is it possible you could have endometriosis? Or both?
I got PMDD after getting a hormonal IUD. It was a nightmare. I felt possessed. Removed the IUD, PMDD went away. It was only for a few months; I can't imagine how hard it must be to have it for life.
I swore I did too my doctor told me it was from age I told her it was from that iud. But I still have pmdd my IUD was removed a year ago.
I got it after the microgynon pill.... Im not taking that stupid pill for months now and i still have it.

I got it shortly after getting my first period. I thought it was normal bc in America you are told you would be moody. I thought being suicidal once a month was what they meant. I went on a medication by my pcp and it was wrong it did nothing. I was told I was being dramatic. I went to an OBGYN and got the correct birth control. Then I needed anti depressants. After having COVID my anti depressants needed to be upped but now I’m decently steady.
I've had depression and anxiety since around the time puberty hit. I got so annoyed when I talked to mom about how depressed I was and she would say, "Are you about to have your period." We didn't realize it was PMDD until I was in my 20s, but it's been pretty consistent, except when I was running regularly and fairly athletic in Jr. High and high school, then my mental health was much better. I think then I just had normal pms symptoms instead of the pmdd. When I got the Mirena IUD, after a few months I began bleeding every two weeks. I was constantly insanely on edge, even angry, and I ran every day just to try to survive. Often struggled sleeping at night because of the intense feeling in my body, wanted to slam my head into a wall. Raspberry leaf tea helped, but man I am never doing that kind of birth control again.
All I wanna do is cry and sleep and get my memory back as I’m soooo forgetful during that it’s embarrassing

@@MedicalCentric Thank you. I have an appointment w/ the psych next week that I’m super excited about. Hopefully they can give me an antidepressant for just the 2 weeks prior as I’m perfectly fine the rest of the month.

While I do understand, i don't necessarily personally support "treatments" that includes suppressing or removing our biology. Something about it feels so very, violent.
The only thing that has helped my own situation and experience in getting to know my own pmdd, is listening and honoring my body's voice.

I have PMDD and ADHD as well! My ADHD meds help a lot, but we ended up upping my dose as needed the two weeks before my period. better now, but im 6 days till blood and I can't wait, I look forward to it every month bc I feel so sane and level minded during my period.

I have this and mild pcos so yay 😀. Either way I also tend to crave sweets prior to my period and all the emotional symptoms that was described I have them. Every month I’ve thought about leaving this planet, come to find out it was hormones.

I started having PMDD after the birth of my kids. I'm on antidepressants which don't help at all. I feel like I'm being possessed. I look forward to menopause sometimes.

i use to be just a little irritable while menstruating before i started taking antidepressants now it's like every time before i have my period i get bad thoughts of doing harm to myself i restrain but it sucks when those things pop out of now where and so does crying uncontrollably

Anyone dealing with this I would like to suggest cyclease pms natural tablets. I’ve been using them as a natural way to ease my pmdd and it helps a lot. I get to frustrated and upset when I’m pms. Those are my go to and they have be back to feeling normal within 30 minutes .

So my GP got me on Seratraline, but hesitant to take it, i was also critically low on vitamin D so got super high dosage and hoping it will be not as bad. 10 days before my period start I am having the peak, literally mental breakdown. I used to have suicidal thoughts but CBT helped with that, I usually check my calendar and 100% it is around those 7-10 days when I feel the lowest. I am often thinking how many women committed suicide not knowing that it’s not them, it could have been me many years ago. That experience set me on a journey of addressing lifestyle choices and that’s where I noticed this pattern. It’s a real emotional pain, usually settles in 2-3 days but completely leave when my period starts. As much as I love being woman and having gift of period, but this is no fun🥴

I got it after having exploratory laparoscopic surgery for Endo. The good news is that Endo wasn't found, but it changed my periods and mental health. I'm not on birth control cause it makes me sick, but I do use BIHT progesterone cream, Remeron and Lorazepam. Sometimes they help other times, they don't. Most of my PMDD symptoms involve sore breasts during the luteal phase, as well as thoughts of bad past things that have happened in my life along with paranoia and intrusive thoughts. After the bleeding starts, all of that crap goes away, and then I feel normal for 2 weeks. The PMDD attacks aren't always bad, and sometimes, they don't bother me at all. All I can say is that I can't wait to reach menopause because then the madness will finally end.
I also may be having a hysterectomy too at some point because after I had covid late last year, I went to see my PCP and I told him to look at everything, so he did a physical and found that my cervix is at high risk for cancer right now.
It is very difficult to cope with 😭😭

I'm wondering if this is what I have rather than just pms. All my symptoms are there about a week prior to my period. I literally inhale food and then 2 days before I just ruined my diet and walk around like a zombie exhausted from doing absolutely nothing. I'm actually relieved once I get my period bevause that all goes away.

Having PMDD is not easy for me because if left untreated, it'd cost me my relationships with my family and friends even my coworkers. As I get older, my symptoms worsen. I don't like losing control of my anger because its going to cause interpersonal conflicts. I'm trying to keep the peace, not start a fight nor an argument. Why is it so difficult to keep my temper and hormones in check? Oh I know why, it's because PMDD meltdowns are not predictable. I do get mad over the smallest things.

Friends who are struggling with this, something that’s helped me is going on a keto diet. I try to keep my blood sugar as stable as possible since a low blood sugar usually increases cortisol. During the menstrual phase I’ll introduce some carbs, but I’m overall avoiding them. I’d say the most carbs I eat in a day- even good days- is no more than 100g (equivalent of 4 oreos a day). Another important part of keeping your blood sugar levels normal is eating three big meals a day instead of snacking. I only suffer from insomnia and depression, though. I feel for people struggling with the pain of PNDD.

In my personal experience, I didn’t develop PMDD until after I had my child. I was diagnosed with PPD, wouldn’t be too surprised if they were linked. You got it tho, find what works for you, push through and never give up! ❤

Honest advice for women with PMDD. Stop having sex! Don't masturbate or have sex if you have PMDD. This helped me and I'm having much better periods, without medication. Serotonin even comes back, like your mood is way better than before. So because doctors have refused to remove my ovaries I found that complete sexual abstinence works well. Also seek Jesus Christ for help if you experience this. This sort come not but through prayer and fasting
