Is Diarrhea...Good? - Gastroenteritis Explained (Causes and Treatment)

  • Diarrhea and loose bowel motions that develop within 24 hours after eating something dodgy are most commonly caused by an infection (also called acute gastroenteritis). Diarrhea is both good and bad: loose bowel motions help excrete the infection along with any potential toxins. However there is a downside - your body loses a lot of water and electrolytes in the process. Dehydration is a common complication of infectious gastroenteritis.
    The most important treatment for gastroenteritis is rehydration: replacement of both water and electrolytes. For mild cases of diarrhea, fruit juices and sports drinks can be sufficient. You can even make electrolytes replacement drinks at home by mixing 500ml of water, half a tablespoon of salt, 2 to 4 tablespoons of sugar or hone and 250ml of orange juice. But for more serious diarrhea, you may need to buy oral rehydration solutions.
    A slow and steady approach to re-introducing foods is the best approach. Initially, the best foods to re-introduce are those that are easy to digest. These include starchy foods like noodles, rice or pototatoes, soups, fruits and vegetables. It's a good idea to avoid high fat foods because these can be difficult to digest. Also, it is common to get a brief period of lactose intolerance after gastroenteritis, so it's a good idea to avoid to temporarily avoid dairy.
    Anti diarrhea medications (eg. Loperamide) are effective for reducing the frequency of motions, but they need to be used cautiously. If used continuously, these medications may prevent excretion of the infection and lead to fluid build up in the intestines.
    In research studies, {robiotics were associated with a modest reduction in the duration of diarrhea, with an average reduction of 25 hours. Probiotics are worth considering if you would like to reduce the duration of gastro as much as possible.
    In healthy adults and children, gastroenteritis resolves by itself in 4 to 7 days once the immune system has cleared the infection. So antibiotics are not used routinely. However if you have severe gastroenteritis or see blood mixed in with diarrhea, then antibiotics can be helpful.

    Once I have bloody diarrhea, my medical prescription also has Zinc besides what you've mentioned. So what can Zinc help in this situation? Should I take zinc every time I have diarrhea?

    • @DocUnlock
      @DocUnlock  Před 5 lety +51

      Great question Duy! There is definitely a role for Zinc in the treatment of Gastroenteritis, but only in specific situations.
      For children who live in areas where the risk of malnutrition and zinc deficiency is high (eg low to middle income countries), Zinc has been shown to decrease the duration of diarrhea by about a day. We don't know exactly how Zinc helps in gastroenteritis, but it is thought that Zinc may help with restoring the lining of the intestine and may help with immune function.
      However, there are some caveats:
      1. Even though Zinc reduces the duration of diarrhea, it increases the risk of vomiting
      2. Zinc has only been tested in children, so we don't know whether it is also helpful in adults
      3. In high-income countries where zinc deficiency is rare, zinc is not recommended because no benefit is expected
      It sounds like you had quite a severe case of bloody diarrhea, so your doctor probably prescribed Zinc to help you recover as quickly as possible. Assuming that you are an adult and are well nourished (ie you get enough Zinc in your diet), you probably don't need Zinc every time you have diarrhea. However it is always best to check with your doctor to get medical advice specific to your situation.
      If you would like to read further about the role of Zinc, here is a great summary of the research: www.cochrane.org/CD005436/INFECTN_oral-zinc-supplementation-treating-diarrhoea-children
      Thanks again for a great question! :D

      Doc has prescribed zinc, pain meds(though he dont have pain, only pains just before passing stool) and ofm ds, probiotic sachet . when will it get better? When will this blood go? When i updated his doctor about again blood in diarrhea he asked to add zantac syrup.

      I came down with hives - I called dermatologist & she said just stop eating for a couple days before my appt with her. 2 days later, all hives were gone. I stopped eating FRESH ORGANIC veggies; hives disappeared - kept eating egg yokes & grass fed local beef for protein. Fine for a couple weeks, then diarrhea started. I called docs office for an order for parasite test but no response. The lab test reported I'd had them (possibly from anywhere - maybe trip to Mexico for son's wedding a couple YEARS ago.) A doc friend suggested that while a follow up test read negative after a round of antibiotics, they are prone to return because the drug didn't kill every speck and that bacteria/worm wants to live. So.. I'm going to eat bananas because here in Hawaii, we have really good apple bananas. Thanks for this revealing & helping discourse. It's been a week; now wondering if this is an infection that has to run its course before returning to normal & then I will have to determine foods to eat.

Cows milk has ONE PURPOSE... to feed & nourish baby cows/calves until they are old enough to eat & drink on their own. Humans are not cows. Humans are only designed to drink their own species milk (their mothers) ..but ONLY while they are newborns & up till the first year. Then our system changes.. & our bodies are from then on, NOT equiped for digesting milk...COWS or HUMANS... or eating ANY dairy products. I was prescribed charcoal tablets once. How does this help?

    • @DocUnlock
      @DocUnlock  Před 5 lety +8

      Activated charcoal is normally used in situations of drug overdose and accidental poisoning because of its ability to bind to chemicals and prevent them from being absorbed into the body.
      Some people have tried using charcoal in gastroenteritis with the theory that this may be able to absorb the excess fluid in the gut (therefore reducing the frequency of bowel motions) and may even be able to bind to bacteria/toxins. There isn't a lot of research backing this up so charcoal isn't used routinely in the treatment of gastro.
      What was your experience with activated charcoal? Do you remember if it helped?

    • @janelee4484
      @janelee4484 Před 5 lety +3

      @@DocUnlock Yes it did. But then again it could've been because the condition was already improving. I can't really tell.

    • @DocUnlock
      @DocUnlock  Před 5 lety +9

      I totally agree - in many illnesses where the immune system would clear the infection anyway, it is really tricky to know whether a particular intervention is making any difference!
      That's why we need randomised controlled trials to compare between a group of people who are given a particular intervention (eg charcoal) and a control group that doesn't receive the same intervention.
      In that sense, probiotics have pretty good evidence behind them - there have been 63 trials of probiotics VS no probiotics (or placebo). So apart from rehydration, probiotics are a good option to reduce the duration of diarrhea.
      Thanks for your question and for sharing your experience :)
