      @@AmiyaPrasadMD Dr 2 weeks ago I did a my elevate procedure (lipo+laser tightened+ sutures from one ear to another ear) swelling is pretty much gone but it hurts when I yawn I can feel the sutures also under chin I have two bumps. Seems like that will remain as I see no changes in that bump. After a week of this procedure my doctor told me that I don’t need to wear faja anymore. Please advise

I had a neck lipo today while my doctor was doing my neck lipo she mentioned that I have lot of scars tissue, I had Jplasma 2 years ago under my neck and chin & that must have caused me scaring, now am so worried that my neck lipo will not work or give me bumps due to scars after Jplasma.       Amiya Prasad, M.D. i need your opinion i m 50 years old 5’5 tall weighs 130 pounds. Most people think i am only 40.
      I met several plastic surgeons regarding my jowls & double chin. 2 surgeons said that i need lipo+ platysmaplasty but 2 other surgeons said I don’t have platysma muscles so there is nothing to tight there that I only need lipo & that can give me definite jowl & chin
      Please advise dr.

      I can't say which is my recommendation because unlike the other doctors, I haven't seen you, or examined your neck area, jawline, and chin.     I've had 2 lots of Kybella. It hurt so much and I looked like a bullfrog for months. It didn't work so now I am going to have liposuction. I would have saved so much money if I just got lipo first up. (2 lots of cool sculpting as well)

      Sorry to hear that. Kybella can take up to 5 treatment sessions, with swelling and bruising that can last because of up to 50 injection pokes needed. Liposculpture can be done in a single session lasting about an hour, or even less. I made a newer video explaining that minor surgery done with local anesthesia can be more convenient than repeated non-surgical injections - czcams.com/video/y4p7nUeILu0/video.html

      I don't normally advise facial exercises for lumps after submental lipo because all my patients usually need is some compression after the procedure to prevent lumps. I hope this helps you.

    I like your honesty, Doctor! Thank-you, for this informative video. My daughter wants to be rid of her double chin but the cosmetic surgeon said that "he would not give her a neck lift because she is only 38yrs old......He recommended "cool sculpting". What is your opinion in this?

      COOLSCULPTING DOES NOT WORK!!! I know a doctor personally and he said in very clear terms that it does not work. I have also had many clients that have used it with ZERO results. Well it gave a much lower bank balance. The way they get away with the scam is by saying that everyone reacts differently. So when it doesn't work, they are not liable. The ONLY way to remove fat from under the chin for a guaranteed result is lipo as it is actually removing material.

      I read the previous comment incorrectly. Coollipo is what I perform in my practice, which is facial liposculpture/liposuction. CoolSculpting is a separate procedure which I don't perform. I agree that fat volume needs to be removed for the procedure to be effective in treating double chins.

      While I do use radiofrequency devices like Pelleve for collagen stimulation, I don't use RF devices like Trusculpt for fat reduction. I personally find Coollipo to be a more predictable treatment. hope this clarifies things for you.

    I might have missed something here but I didn't see a lot of elaboration on the cons of lipo. From my research, there are 'nerve' risks associated with chin lipo, can you please advise?
    Also recovery time and mandatory banding for six weeks for healing, is this the process for after the procedure?

      The risk of nerve injury is very low when liposculpture is performed properly. Wearing a band in the chin is normal for recovery, but I have my patients use it for no longer than 2 weeks, so 6 weeks is quite long. I hope this clarifies things for you.

      Thanks so much for your clarification. I live in Australia so our terminology is a bit different to the US. I keep trying to find places that do liposculpture however there aren't very many. Can you advise on the main difference between general submental lipo and VASER lipo? (I see only vaser lipo being offered in Australia).

      VASER has been around for awhile, but it's the doctor's skill and judgment in using these tools that are important, not the tool itself. I have found that many of my middle aged patients have less submental fat than they first thought, so I can use a laser to melt fat and tighten the skin at the same time, without an invasive procedure. Again, this is about the doctor's skill and judgment in choosing the right device and procedure for the individual patient's needs. I hope this clarifies things for you.

      Hi Nats Eksig,
      Sculpsure is a 1064 YAG laser, so while I don't have this particular brand, I do have the laser. What I like about the one I use is that the wand it has allows me to be more precise. This laser is often used in the submental area for middle aged patients, where they have less fat than they think they have, where liposculpture to remove the fat may leave their skin sagging and/or wrinkled. The advantage of using this laser is that it can be used for moderate fat reduction, while tightening the skin at the same time. I hope this clarifies things for you.

      Amiya Prasad, M.D. Thank you for your reply. I’m considering the 25 minute Sculpsure treatments for my submental area vs the more aggressive fat removal mode of liposuction. I understand that fat cell death is about 24% with Sculpsure. I was also looking at Coolsculpting but have decided against it considering the possibility of indefinite nerve numbness due to freezing the area. I’m so grateful to have found your channel.

    Thank you doctor, I had liposuction for my double chin/neck 4 weeks ago. After the first week it appeard a swelling like a bump line under my chin and it is still there. I still feel the whole area numb and like it is streching. Is it normal? Will the swelling/bump dissappear?

    Thanks for the video... So double chin could be caused either by fat cells or by skin laxity. Cause may not be definitely known until Doctor actually opens the skin. If it is primarily due to fat, the Doctor will typically lipo the fat cells. If it is primarily due to laxity, he could just lift the excess chin and likely forgo the liposuction (since there is minimal fat). Is that the idea?

      An examination is needed to determine what the best treatment for your situation is, but I normally perform liposculpture for patients who are within 10lbs of their ideal body weight, and have previously lost weight through diet and exercise. I hope this helps you.

    Dr. Prasad - I’m 53 and thin, have platysmal bands and minimal jowls. I recently had a consultation with a surgeon who recommended Lipo for this. You mentioned neckline lipo is generally recommended for younger patients and that this could further affect the appearance of my platysmal bands. ? He said I’m not ready for a facelift. What would you recommend?

      I don't perform fat transfers, which are commonly called stem cell treatments, for skin rejuvenation, and would use the more simplified and predictable platelet-rich plasma (PRP) possibly combined with a laser treatment or hyaluronic acid for skin rejuvenation.     Hello Dr. I did my double chin lipo 5 mths ago and never saw results why is that I notice that some patients they did 3 Incisiones on my procedure they just did one below my chin..and planning to do my 2nd round with the same doctor do you think it's safe to do a 2nd or 3rd until I see results?..I did use my faja for 3 weeks and them for 2 more weeks at nights only

      I can't recommend a third treatment since liposculpture for a treating a double chin normally takes a single treatment, and perhaps a second if there was residual fat or complication from the first treatment. Fat cells are permanently removed with liposculpture. If your doctor is not an expert with liposculpture, I suggest you get a second opinion from another doctor which is not just asking them online, but actually seeing them for a physical examination. I would need to perform a physical examination before I can make specific recommendations for you. I hope this helps you.

    Do you recommend anyone of your experience in London please . It is hard to chose a surgeon without knowing their true experience . Many might show you fake photos claiming they preformed the work and notice some photoshopping.

      At least topical anesthesia is needed for Kybella injections because it does require many needle entries for treatment. Facial liposculpture is a surgical procedure, but not a long one as it can be done under an hour, so it will need local anesthesia. A patient feeling pain and discomfort without anesthesia will be at least fidgety during the procedure, which will affect results, and may cause injury. I hope this clarifies things for you.

    Hi Dr. Thank you so much for this informative video. I just had a BBL + chin lipo a week and sadly I see no results in my chin at all. I know it is still very early and swollen but I am depressed about it. I paid $1500. Is this normal for patients not to see any results the first week or 2? I didn't have any pain but had A LOT of bruisings in my neck area. I just don't see any results.

      Sorry to hear about your problem. Temporary swelling in the chin area may be obscuring your results. I suggest you wait a little longer for swelling to resolve, and then assess results. If you have any concerns, you should follow up with your operating doctor who knows your anatomy and details of your surgery. With the added heat, skin contraction is a factor that needs to be considered, and the heat does make it easier for the performing doctor to glide through the fat, but it does take skill on the doctor's part to avoid any potential issues with adding heat to liposculpture.       Hi Joe service,
      There usually isn't much residual fat after a liposculpture procedure under the chin, and fat cell removal is permanent. Fat cells don't regenerate, but remaining fat cells can gain volume. Procedures after submental liposculpture are usually for the skin not retracting as expected, so a skin tightening, or skin lifting procedure may be needed. I hope this clarifies things for you.

      You normally don't want to remove all the fat as it can lead to a hollow or unnatural appearance. There is healing time needed between liposculpture procedures if a repeat procedure is needed as it is still surgery, so swelling and other post-surgical healing issues need to resolve before an accurate assessment of results.     I have double chin plus fat in my neck around my thyroid , making my neck look fat and making my thyroid look enlarged. Do you remove fat from around the thyroid without disturbing my thyroid? How long does it take to be back to normal ? Post operative ? Once the fat is surgically removed , will it come back in that area when I gain weight ?

      It's important that you get within 10 pounds of your ideal body weight before considering cosmetic procedures, otherwise the procedure won't be effective, and results won't last as long.       I don't usually perform or advise buccal fat removal as buccal fat is eventually lost with aging. I only perform buccal fat removal when it has descended and is affecting jawline definition and contour. Buccal fat removal is a separate procedure from liposculpture of submental or chin fat.
