  • In which John makes an unscripted video from an unusual room about how diseases need "a constituency," and how activists working together across the globe really can dramatically lower the burden of an illness and increase the availability of treatment. This week, I've been reminded of an old adage: Never underestimate nerdfighteria

I came from East Africa and I was orphaned by TB before the age of seven. I don’t think people living in first world countries truly realize what a nightmarish of a disease that TB is. Mr. Green, I thank you from the bottom of my heart for your effort in bringing awareness to this problem that many third world countries are still struggling to deal with. I am happy to be in this journey with you and people of goodwill in this community. 😊

As a Peace Corps Volunteer in the early 1990's in central Africa the first person I saw die there, died from tuberculosis - although he had HIV/AIDS he died from TB and it was then I began to learn how prevalent TB still was worldwide, how stigmatized TB remained and how it was still looked upon as a disease of the impoverished and not to be taken seriously. I cannot tell you how pleased I am that you have taken this on as a cause to be taken seriously and how many have joined along with you. Thank you. Truly.

A few notes to my over-simplifying brain:
    1. Fighting TB includes not only making its historical impact better know, but making its impact today better known.
    2. A comprehensive response to the TB crisis includes (but is not limited to) early identifying and diagnosis of the disease, building networks for easier access to TB treatment as well as access to preventative medication for people living with someone going through the disease.
    3. TB is, for the most part, a consequence of impoverished conditions and signals our urgent need to build more equitable communities around the world.
Thanks John, all these points bear frequent reminding !

Equity sounds too close to communism for my taste.

I don't think we are allowed to do any of that unless it also makes the already wealthy slightly richer.

I went in to see my pulmonoligist today. While discussing the possibilities for my abnormal lung CT scan, tuberculosis came in to the discussion (No, I don't have TB). I mentioned that I had recently learned that TB was the number one infectious disease killer in the world. My doctor was delighted at my knowledge, and I of course gave credit where it was due. We fangirled over John Green and Nerdfighteria and the events of last week for 5 solid minutes. And we were so proud of you! You're making a huge difference, John. An absolutely amazing book on the development of HIV and the early research is ‘And the Band Played On’ by Randy Shilts. It is a superb piece of writing which expresses in stark detail the dangers of doing nothing. But also, the change that can occur when people continue to fight for recognition and for governments and heath organisations to treat a disease with the seriousness it deserves.
The fury in this book is something that we should apply to Tuberculosis. A fury over inequality
and unfairness, even more so that TB is curable.

My first job out of college was working as a lab tech in my local hospital, and I remember being shocked that it had a dedicated TB lab, that regularly got samples to test. My home town, turns out, had a pretty high TB rate among the homeless population as well as testing all samples for our province (including many asylum seekers coming in). It was the first time I had any idea that TB was still an issue.

For 2 months I had to take 11 tablets a day (as well as some drinks to fatten me back up after weight loss) which went down to around 6 tablets a day for the next 4 months. Missing too many days would make you resistent to the medicine. I can only image how difficult it would be to keep up a routine like this in a 3rd world country

Thank you John for your dedication informing us about TB. I'm from Sweden and TB is rare here now but I remember my grandmother telling us about friends and relatives who died of "the white death" when she was young, and how dreadful it was. Yes, the whole world must be engaged in the fight against TB - and poverty.

"Crowded living conditions" got me. We had an outbreak of TB on the Chinese university campus I teach at this past semester. The students live in dorms of 6-8 people (3-4 bunk beds) each and they were locked into their dorms during the end of 2022 because we were having the worst time from CoVid-19 (worst for the whole time we were dealing with the pandemic). The students have no choice. We were all sent for chest x-rays only but obviously that is not enough. One of my students was hospitalized for their TB and never returned during the semester. I have no idea how they are doing. That is terrifying for all of us. Also, the part about people in rich countries needing to stand up about this is so true. Before this semester, I had no idea that TB was still a problem anywhere. I appreciate these videos teaching us more and giving us resources to become an agent of change. Thank you.

My grandfather was orphaned by TB at the age of 7. His mother couldn’t get access to medicine before it killed her, his dad was a drunk and didn’t care. This was in haverill MA. She was an indentured Irish servant. This was in the 1940s. It’s still a disease servants, healthcare workers, and more are reckoning with. Thank you for giving voices to the lost ones and helping prevent more deaths.

I’m very close with a man who works as a project manager at a pharmaceutical company (he’s like my second father). His team is currently working on a vaccine for malaria that they hope to put into action in Africa within the next couple years, and I believe he said another team is working on another treatment (or vaccine maybe?) for tuberculosis. Thankfully, there’s research currently being done, and it’s not being totally ignored. Obviously it’s still a big problem, but there are people who both know of the issue and are capable of finding and developing a solution, which are the two most powerful things to have in human innovation.

My brain is not doing so good today, but even if I was at my Full Wording Strength, I think I’d still be in this feeling I’m in now: speechless, with a heart full of awe and joy and shock and hope. Collective action really can do something when we all make the commitment to be careful with our attention and lead with compassion, rather than react with fear and dismay. With any other community, I might be worried that we’d all move on swiftly and forget about this issue - but having seen how much we’ve committed to the PIH collab in Sierra Leone, for example? I know for certain that this is just the beginning of what Nerdfighteria are going to do.
God, I love this community so, so much. Thank you to John and Hank both for bringing us all together and leading by example… but thank you, too, to everyone here. You keep hope alive. I really do feel, with you all, that the power of imagining others complexly has never been stronger. ❤❤❤

I know I'm preaching to the choir when I talk to Nerdfighteria, I do want to bring up that TB is still around in "first world" countries with money, it's just around in populations that are considered...less than desirable by society and are kind of naturally disjointed so it's hard for them to be the squeaky wheel, so to speak. People suffering from drug addiction, migrants, etc. It's obviously much easier to access treatment in a rich country than an impoverished one, but it's still hard. If you're in the states, don't forget to fight for medical justice in the US with your votes and your buying dollar whenever possible!

    1. Make sure they're not in the bottom third of the screen so they are not covered up by CC captions.

    3. Larger font size, please. Bold helps, too.

So I knew it was floating around here, and presumed it was affecting vulnerable countries/populations substantially.

His disease wasn't recognized by the corpsman aboard his ship, so he went undiagnosed for a month or more before an x-ray was taken.
He was immediately taken to Balboa Hospital in San Diego where they made the formal diagnosis and put him in isolation as they began treatment.
He was on "several kinds of pills and injections" before he started getting better after 3 months.
He was still in the hospital when they finally decided to discharge him.
His former shipmates were angry with him because he exposed all of them, but it wasn't his fault. He didn't know! How could he? So many people thought, as others do, that tuberculosis was a thing of the past.

John, consider yourself blessed enough to be able to think (at one time) that Tuberculosis was a thing of the past. I live in Asuncion and I am acutely aware it is not. We have a medical center dedicated to the care of those with TB. It is lacking in almost everything, except people with TB. A friend of mine got sick with it a while back. She has thankfully healed and is now married and has beautiful kids. But there was time we thought she wouldn't make it. So, yes, I am very aware that bacteria is alive and well around the world. Wharton funded several convalescent homes for tuberculosis sufferers, and was particularly interested in what she called American-style clinics, which encouraged patients to take plenty of fresh air and practice good hygiene. Long after the war ended, she remained moved by the plight of tuberculosis victims; in 1930 she contributed to a new hospital to treat the disease."
