      @Luni ss. I didn't have radiation. Fortunately I was post menopausal and so they didn't feel I would benefit from radiation. I did wait 6 months after my last herceptin infusion. Herception really brought out all of the aches and pains. Felt and looked 20 years older than I actually was.
      We now have 10+ years of actual procedures now where I believe the doctors would have most of the kinks worked out. I understand that radiation is really hard on the skin. Maybe there is a lotion or ointment that you can use in preparation for the surgery.

    I had this done approximately 7 years ago. It was still somewhat controversial at that time. Very few surgeons did it. I had it done after cancer. I didn't want implants and I didn't want huge breasts. Usually you get a cup size bigger. I was a 34C to begin with. My breasts turned out beautifully and I wouldn't want to be any larger. They got bigger over time for some reason. A little over 34D. I disagree with the mammogram thing The one annoying thing is false positive cysts and fat necrosis that shows up as "suspicious" areas and three times I had to go through ultra sounds and biopsies for no reason. Costly. Still glad I did it my way. Funny how the two lights behind him look like breasts...just sayin.😂

      I’m having this done next year. 2 years ago I had a double mastectomy after cancer diagnosis. I’m not going bigger than I was before. I just don’t want to be concave anymore.

    I’ve had fat grafting to my breasts. 18 mos. later I’ve maintained 1/2 cup (or more) size increase. I’ve also lost two inches from waist/flanks from the lipo. Super happy with results! My issue now is I’ve always had sagging breasts, so my next surgery is a breast lift. ❤️

      Right, very honest. I always go with the MD that tells me " NO" first, versus the ones that see I'm paying cash and promise the moon and space ship. I was a - A , went in for consult. Wanted a D had the cash that I saved. Went to 3 Dr. Two told me yes, standing at 5 feet 128 lbs. One told me "no", and he was not willing to put his name on that "as my work is its own walking billboard." Take this (cocoa butter) and go home. Massage the boobs for two weeks, and when you come back, we will aim for a full C.
      I was surprised but felt reassured. Got them done and was happy as ever... The only downside is I'm approaching year 10 and scared that getting them redone won't turn out as perfect as these. So, lowering my expectations and a fat transfer might be the option.

    BY FAR THE BEST VIDEO I HAVE WATCHED BY A DOCTOR!!! I’ve watched hundreds!!! Amazing info. Thankful for this!!! Is there a test, Doctor for a silicone allergy, let’s call it, for example for a person who may have a silicone intolerance? Before receiving implants? So thankful for your time on this topic. You’re very honest. Thank you. Women (I think) chose to put themselves through these procedures for so many many reasons. This is a big deal. I gather you care and understand these are human bodies that have entire lives to live. I am going to get several procedures. I truly wish you were near Tampa. You have given me faith in medical knowledge. It’s real.

      Finding the right Dr. is extremely challenging especially with the number of online platforms and resources available. There a lot of great Dr's available. Definitely check to make sure your choice specializes in the procedure you want and spend the time to read through patient testimonials.
      You can always pick up the phone and call around also, find an office you feel comfortable with and set up a few consultations.

    I'm a TNBC survivor and my plastic surgeon suggested the fat transfer after my bilateral mastectomy because he doesn't do implants after Expander. He says the results will be similar to a expander and that i will need around 5 to 6 procedures every 3 months. I trust my plastic surgeon but after seeing your video got curious about your opinion. Love your explaining, its clear and easy to understand!

    Are there any studies that show whether there is an increased risk of breast cancer from transferring fat to the breasts? That’s honestly what I’m mostly scared of

      I agree with @ Am La. I'm not a doctor but the cancer wasn't in your fat. I would think it is unlikely for the cancer to return in your breast after this type of reconstruction.. The fat a skin are from your own body. As you know, once we have the big C, we are always susceptible to it returning. My understanding is that it returns in a different location most times. I don't know about you but I am hyper vigilant now in what I put in my body, the negative energies around me, and always keeping my appointments.
      As far as studies, you may want to ask the plastic surgeon. Usually the first consultation is free or very reasonable.

    I've been reading about how it's better to do fat transfer if you're skinny because the cells will be lean when transferred, instead of enlarged, and how some doctors actually ask patients to lose weight before the collection and what they do is collect a thin layer over a large area, giving preference to stable areas like love handles and thighs. I was wondering what your opinion is on that technique. And also if you think using extenders prior to the surgery help or not. I know brava is not a thing anymore, but just wondering

    I loved how much information was provided on this video. I had two kids back to back and have been breastfeeding for 2 yrs and 3 months nonstop and I am about to stop next month when my son turns one. My milk production has already slowed down. How long after I stop fully breastfeeding is the procedure recommended? And I am in Az so jow long should I be expected between follow up appointments?

      The ideal timing for breast augmentation when you are breastfeeding is about six months after you have completely stopped breastfeeding. You can speak and visit a board-certified plastic surgeon to further discuss your preferences and the options that are available for you, especially if you are looking forward to having another child.

    Thank you for explaining this. Not only did I not know about this but this is a good alternative to a breast lift. I'm a little too fearful of ever getting a BBL because of the high death rate so I'm glad to know about this procedure to take a bit off certain areas and fill in some areas on my chest.

      I don't think it would give you a breast lift that's a different procedure. Fat transfer to the breast will mostly give a fuller look to the breast shape you already have. If you have sagginess then you'd need a breast lift and maybe implants/fat to get back the fullness you once had before your breast started sagging.

      any major weight changes would affect the grafted areas. That is the reason you need to reach goal weight prior to surgery.

    Im getting an explant due to BII. Was never informed of fat transfers, originally! I had a lift and implants due to extreme asymmetry and fat loss in one breast. Im now getring my old, contracted implants out and fat transfer and lift as well as a tummy tuck (lots of extra tummy skin and diastasis recti)

      fat viability depends a lot of surgeon technique. The fat has to be placed back into the breasts as soon as possible so that it has a greater chance of survival. It has to be placed carefully so that not too much fat is placed in only one area.

    I've been considering fat grafting to the breast after breastfeeding and I think it will be what I choose to do as far as augmentation, my question however; is that my son prefers contact naps on my chest, will the added pressure/weight on the grafted area affect the final result?

    Have a third fat transfer next week. After BC 17 years ago, had implants removed due to significant capsular contracture. (Clinical trail participant for gummy implants). Had minimum fat transfer with new implants 220 cc. Severe capsular contractor within 4 weeks! Had them removed and just decided on fat transfer. Implants do not agree with me. No longer con-caved, but realize the best results will be nearly flat chested and wear a padded bra. I won’t do more than three because I find it considerable painful/uncomfortable and don’t like the downtime. But feel it was worth it. I can feel my heart beat again and my chest is warm if that make any sense.

    I was personally going to do implants plus fat transfer to breasts to get the biggest & most natural results. I’ve seen this done a lot in Korea & their breast implant results are impressive

      I did it y'all I got surgery! I'm about a month out now and my breasts are making great progress. As for the fat transfer, I have volume in my breasts that exists in places it's never existed before.. especially on the sides of my breasts, I have never had side boob before!! As well as volume between and on the inner sides of my breasts, before my sternum bone was super aparent and obvious but now there's some meat there finally! Once my breasts drop and soften a bit more I can tell I will finally have cleavage I've never had before.. my surgeon injected fat in three different places on each breast, one on the side, the tops, and the inner part near my sternum. The fat has created such an improvement in my breasts along with the implant, I would 10/10 recommend!

    Dr. Barrett, I recently had a rupture of breast implant. 🤦🏾‍♀️🤦🏾‍♀️🤦🏾‍♀️Therefore I had a removal and also brest lift.I truly miss the volume. I no longer desire implants.     I just had my 1st fat grafting along with tissue expander to implants and so far so good - two weeks post surgery. I do wish more doctors took the time to let the patient know AHEAD of time what they are in for as far as fat grafting. and what to do following the procedure as far as massages/lymph node massage. I like Dr. Barrett's approach and explanations!

    My surgeon did composite including implants. I was lean before so it was a challenge to find fat and he went with flanks and inner thighs. Man the lipo sites were sore after but all bruising faded. Boobs still in drop and fluff stage and I have a lot of upper pole fullness and visible slim down in hips and thighs . He said he only took 300 grams which is interesting since it looks like I lost an inch or so from those areas.

      finding the right Dr. is extremely challenging especially with the number of online platforms and resources available. There a lot of great Dr's available. Make sure he/she is a board-certified plastic surgeon. Definitely check to make sure your choice specializes in the procedure you want and spend the time to read through patient testimonials. There is a number of different sites where you can check for reviews (Realself, yelp, google, etc.).
      You can always pick up the phone and call around also, find an office you feel comfortable with and set up a few consultations.

    I have a question concerning the harvest of the fat cells. How much body fat percentage is needed to harvest an appropriate amount of cell? I'm 160cm tall and weigh 55kg, I'd consider my figure as "sporty" but I don't have a lot of fat tissue to work with...

    What about stem cell enriched fat transplant? I read a few years years ago that fat transfer often had a very lumpy results due to reabsorption of the fat by the subjects body. I then read that they were using stem cell enriched fat to prevent reabsorption. The gist was the Drs harvested twice as much fat as was needed for the transplant. Next the Drs harvested all of the stem cells from half of the fat they removed and placed those stem cells in the fat they didn't harvest stem cells from. They then used that stem cell enriched fat for the transplant. The fat they harvested the stem cells from was discarded and not used. Is this not the best method? Is it not being done now? Any further information on the subject?

    How about replacing old implants and having fat grafting at the same time? In such a case, would you place the fat all over the breasts before putting the new implant in, or would you only place the fat around the implants?

      breast reduction is best suited for women who wish to decrease the size of their breasts. While there are different techniques used to perform this surgery, I prefer the vertical incision (lollipop pattern) breast reduction technique instead of the anchor scar pattern, which reduces more noticeable scarring than usual. I also use a SAFE liposuction technique to reduce additional fat around the stubborn axilla area and bra line. This way, my patients are achieving beautiful results with minimal incisions which reduces downtime and delivers an overall better breast shape long term.

    Also he is absolutely 100 % accurate about the pain from the liposuction harvesting site. I have had 9 breast surgeries in 4 years from mastectomy to failed sub pectoral implant reconstruction (gummy bear implants ruptured - and where recalled by manufacturer) followed by the procedures listed below. By far the most discomfort has been the liposuction from the flank - I feel like the proximity to the spinal cord makes that area far more sensitive than other areas
    I am a 42 year old female with the BRCA 1 genetic mutation and have had a a bilateral prophylactic mastectomy.
    March 2021 DIEP Flap reconstruction
    June 2021 lipofilling harvested from flank to upper pole of breast
    I understand that, as with any cosmetic procedure, there is no way of predicting how much fat will survive post lipofilling.
    However my question is: "IS it possible for the fat to migrate towards the axillar and bra strap area post op? "

    I’m going to get fat transfer for my breast. Can I get a full lipo surgery during the fat transfer procedure ? So a lot of the fat is thrown away and not used for the fat transfer? I feel like if I’m doing the surgery I might as well pay a bit more and get both procedures done.

    You said that the procedure is done carefully like planting seeds, but there‘s a video I just saw on youtube where they showed everything and it looked so harsh and painful and just not any kind of gentle.. but the breasts looked great afterwards... is it really like that?

      Fat viability depends a lot of surgeon technique. The fat has to be placed back into the breasts as soon as possible so that it has a greater chance of survival. It has to be placed carefully so that not too much fat is placed in only one area.
