I have rheumatoid arthritis but finding someone else who does who has also had breast implants is difficult !       @@barrettplasticsurgery absolutely! I have a consult on monday and like to be prepared. So I have been taking notes. I was getting frustrated at the amount of shady or incomplete information out there. This video really helped bring me peace of mind and confidence going into the process. I do have a question, with submuscular placement, does a weightlifter have to be careful not to build up the chest muscles after implants? (Being fully recovered, of course.) I wonder if the pecs would strain against the implant with muscle growth/tension and cause the shape of the breast to deform?

    Good to see the explant shift or choosing against implants. There is a lifetime of future surgeries ahead with breast implants that isn't discussed... aged implant exchanges, capsular contractures, weighted implant breast lifts, plus numbness, etc ... not to mention the serious BII health issues!!

    I also thought capsular contracture was the body's own defence against foreign object such as breast implants. Also the way they used to break it up was to message the breast hard as to break down the contracture and the silicone as that would get hard too. Now I am hearing it's contamination too.

    Wow. This video was SO HELPFUL. It answered all my questions and then some. I am scheduled for surgery here in my hometown and she has fantastic reviews and seems like a great doctor based off consult (she uses Keller Funnel!) I am just so impressed with your approach and all the extra things you do to make this operation comfortable. I have only had a consult and my pre-op surgery is in two weeks! Is it appropriate to ask to have local anesthetic injected as well as the antibiotic solution? Thank you so much!!!

      yes, you should discuss all your concerns prior to surgery. That is one of the main reasons why preoperative examinations exist.

    Dr B. Are you open to making a podcast on elite athletes with implants. For those women who are still feminine but slightly muscular body types but maybe a little more cut and like to do HIIT on a regular basis? I’m worried about going dual plane or sub muscular with my level of training!

    I love all of his videos and used his most recent breast augmentation post op video and felt amazing after only a week. I had my first breast augmentation after birthing 3 children in 2008 with saline implants and the second 2014 because one of them deflated. I had my third, most recently, in 2022 with a breast lift and silicone implants which I absolutely love. When you’re a thin woman and breast-feed, your skin stretches out. I stretched out so much and without any fat, I just had sagging skin for boobies. Silicone, are by far, much more realistic feeling and they feel much better on my body too.

    Are saline implants the better option if going above 800cc.?
    My wife is looking to have her saline implants redone/upgraded as they have been in 14 years.
    Looking to go abit bigger 1000 to 1200cc preferably through the nipple again as to not add scaring.
    Can the capsule be removed through the nipple along with the old implant .
    Or would silicone be the better option and to go in from under boob through the fold.

      finding the right Dr. is extremely challenging especially with the number of online platforms and resources available. There a lot of great Dr's available. Make sure he/she is a board-certified plastic surgeon. Definitely check to make sure your choice specializes in the procedure you want and spend the time to read through patient testimonials. There is a number of different sites where you can check for reviews (Realself, yelp, google, etc.).
      You can always pick up the phone and call around also, find an office you feel comfortable with and set up a few consultations.

    Hi! I am having breast augmentation surgery in December and I am incredibly excited. I am going for areola incision with a 400 cc silicone implant under the muscle. This is the most informative video I have watched. Thank you for that. I have realized that maybe I didn't ask all the questions I should have. Like, does he do a multi layer closure and does he use a funnel with all the sanitary steps to avoid capsular contracture. My question to you is, Is it ok to ask these questions or would you as a Dr. take that offensively? I don't want him to feel like I don't trust him. I would just like to be fully informed.     I'm getting ready to have my breast augmentation in September and I do think I picked a great surgeon. I really want to achieve a very natural look and I figured I would want a moderate profile so I could have that. I really like the upside down heart shape and I really don't like when they look like 2 round balls sitting on your chest. However, I have literally no breast tissue on the top so my surgeon said I had to do high profile otherwise they would just look long, or something like that. And then he said he would have me not wear a bra so that I could still achieve the look I want. Does this sound right? Like I said I know I picked a good surgeon and I do value his opinion, but I guess I'm just trying to double check that this can still allow me to achieve a natural look. Thanks so much for any input!

    Why sailne bag is full of the air? Its not like that when its actually placed. Without the air, sailne bag feels way more jiggly then silicone

    I've wanted an augmentation since I was a teenager due to asymmetry, I believe was caused by an injury when I was 16.     Do you ever perform a sub fascia placement? I just got my BA surgery 8 days ago and got this placement and currently my lats are so swollen. Please get back to me love your videos and this is such a great video very informative

      I almost always place implants submuscular. It lowers the rate of capsular contracture significantly. In addition, it looks much more natural because the muscle provides covering over the implant so its not as round on the top. I've also noticed the implants drop less over time when they are protected under the muscle.

    I had gastric bypass and lost over 150 pounds. I was told by plastic surgeon that they could do a lift but would require me to back for a 2nd surgery for implants. When I asked the reason I was told necrosis of the nipple.

      I recommend my patients to replace implants after ten years in order to reduce the risk of complications such as implant rupture.

      Breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient.
      When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.
      Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.
      Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.

      the cost of surgery depends on a lot of factors (type of surgery, number of treated areas, anesthesia fee, surgeon, facility fee, city, etc.). It would be the best to visit or call plastic surgery offices and ask for a price.

    Now if you used saline tear drop and filled it more would that be a better option for tear drop. I am 300 cc but as you know the bag sits under the muscle in its housing so some is hidden and doesn't look as big. I also noticed on all my friends that once rhe breast settles it becomes a little smaller. I guess all the swelling lol.

    I think this is very informative.. however, moderate profile does not look good on everyone. I’m confused by the one profile fits all approach. What about those that desire a certain cc but can only attain that higher cc with a more narrow implant.

    So if I wanted new breast I could go though the same opening? As I have no Capsuler contracture and bottoming out because I have mussels that were cut for housing. I have seen going though the nipples and I heard that you lose sensitivity and it never lines up and the whole idea is to look good naked. Dr dolskey in Bala Cynwyd PA did my second surgery. I only lost faith when I took my friend there and he suggested silicone as I mentioned at that time it was more liquid and not gummy. He was not a fan of silicone but that changed. He did a fantastic job. I would get surprised looks when they would find out it was inhansed. I guess since they are round I would get them tear drop. And it's been with me for 15 years + knock wood.

    Question can you get capture contracture after 5 years of your breast implant procedure?

      it is possible that capsular contracture occur after 5 years. Capsular contracture affects people approximately one in 20.  No one knows exactly what causes it and what can prevent it completely. It's important for your doctor to use a minimal incision and a Keller funnel as a sleeve to protect the implant from touching anything before goes into the breast pocket.  Singulair has been shown to prevent it

    Because it was so much muscle taken out I was in extreme pain when laying down or getting up. I finally learned to hold my breasts and roll to the side and then get up.

    Can you recommend any surgeons in London? Your techniques seem so thorough and the two people I have seen so far dont compare. Too far for me to travel to you though, especially in Covid 😑

      finding the right Dr. is extremely challenging especially with the number of online platforms and resources available. There a lot of great Dr's available. Make sure he/she is a board-certified plastic surgeon. Definitely check to make sure your choice specializes in the procedure you want and spend the time to read through patient testimonials. There is a number of different sites where you can check for reviews (Realself, yelp, google, etc.).
      You can always pick up the phone and call around also, find an office you feel comfortable with and set up a few consultations.

    I am watching videos on implant because one day I would like to go tear drop. I have saline implant under the muscle. They look and feel natural. There are different bags some better than others. But I don't know about gummy bears. When I had mine done it was liquid silicone and once it ruptured it would go all over and very hard to contain. My first left implant ruptured and because it was saline it folded over itself and my body absorbed it. I got my second one and went much larger which fit my body better. Again saline.
    My nipples have feeling and your right about the top pockets as it make for a natural look.
    The saline implant you are showing has air in it which make it look completely unappealing where that should have been given a fair chance as well. I am still watching though.

    I loved this thank u so much I learned a lot but I have to disagree about capsular contracture is due to contamination I got capsular contracture and I have autoimmune issues so I just think I got it because my body formed a capsule to reject a foreign object entering the body. I also developed keloid scar from c section so I’m not a good candidate for surgery but I’d love to get some gummy bear implants

      Ibuprofen isn't a blood thinner. You're thinking of aspirin.

    i'm going into a breast augmentation consultation tomorrow. i keep hearing about double bubble and the implants moving to the arm pit. can i ask the dr if he can sew the pockets before inserting the implants? also i hear from other women that you are supposed to go 100cc larger than the size you want if it going under the muscle

      Breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient.
      When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.
      Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.
      Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.
      You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

    I had a breast lift 3 months ago. The doctor chose 325cc moderate implants. My results are a lot lower than expected- the bottom of my breast is at my elbow and I require a push up to get the look I want. In addition my bra size is now 32H. I really want to be able to wear tops without a bra. What type of revision should I invest in? Complete breast lift revision? Implant exchange? Fat transfer to the top of breast?

      I understand your concern. However, without a proper assessment, it would be difficult to determine what went wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

    Hello. Very interesting podcast on CZcams I just lucked into… thank you. Do you offer any podcasts that address 15+ year old saline implants, and what to expect with either re-doing them, doing a lift, or, most especially, total removal? What is to be expected of the breast tissue after total removal of under the muscle implants? Can the breasts ever go back to looking the way they were prior primary surgery? What age women are deemed too old and too high risk for surgery? 👵🏻

      it is hard to tell for sure without an examination. You breasts will probably not look like they did prior to surgery 15 years ago. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

    A little worried that i am having 325cc high profile textured implants at the end of the month. How high is the chance of bottoming out?

      Bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions, which is why I try not to use the inframammary approach to any breast surgery.
      Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies.

    I am currently going through reconstruction after mastectomy and I have 390cc I wanted to fill 30 or 60cc's more but my PS said it wont make a difference. Is that true? And do PS get offended if their patients seek a second opinion

    I don't know what happened to my reply about giving saline a fair chance and not fill it with air to make it look undesirable. I have saline. It's under the muscle where you can see how to build housing for the bags it won't bottom out. Also my saline is very soft and well done. When it leaked it was absorbed by my body and the bag folded on itself. I went and got a 300 cc and did the whole thing again but bigger.
    If the pocket is done right you won't see the bag. Also going through the nipple I have never seen it line up and how do you see how much muscle your taking out? In such a small space? And what about the nerves around the areola? I rather look good naked than having no scar but ashamed to take my top off. But now that your calling the silicone gummy bears does that mean that it's thicker it doesn't spill if it should rupture? Also do you do saline tear drops because if the silicone moves around wouldn't the saline bag filled pretty good stay in place. I chose you as I liked some of the things you were saying but I got no response to a lot of my questions and some was taken off.

      The different types of implant offer different pros and cons. For instance, the pros of silicone implants are that they look and feel more natural and ripple less.
      On the other hand, the pros of saline implants are that they do not require as large of an incision as silicone, less expensive, they have less incidence of capsular contracture (scarring around the implant), one can tell immediately if there is a rupture, the size can be slightly adjusted, and they can be used for women less than 22 years of age.
