It’s a sort of myth that smelling something burning is a sign of a stroke, but it’s any unexplained smell that isn’t really there that can indicate that someone’s experiencing a stroke.

The part about white cloaks really encapsulates a big part of the song's message. He knows that his comedy can theoretically do good in some aspects of the world's problems, but when it comes down to it, he feels helpless to help prevent tragedy, and he feels like it's self-important and tone deaf to try to use comedy to help.
Of course you would run! He knows that, of course. He's just not sure how to help without coming across as another white savior, and without sacrificing more than he is willing to spare.
