After trying at least 7 different kinds of MTB shoes, I could never find a pair that worked for my feet; I'm a size 44 Euro, standard width, but could never find a pair that didn't make my forefoot and toes numb, until the Lakes. I have the 238's and the 218's and both of them are uber-comfortable and work amazing with my Crank Brothers eggbeaters and Candys.

I got the road version of these shoes few months ago, the CX238 and I exactly had the same problem with sizing. The forefoot was wide enough and very comfortable. However, the hind-foot area was too wide for me and on a moderate effort my heels would slide out. I ended up selling the shoes because of this issue.
I followed their online sizing guid for the shoe and I would say I got the right size, length and shoe width at forefoot area were correct and comfortable.
