    Love that you saw the Clooney meme 😂 Everyone in the corporate media has been beyond dishonest about the obvious decline of Biden, then have the audacity to act surprised.     I feel like the entire country is trying to have that difficult conversation with their parents when it is time to give up the keys to the car.     I am 81 and in excellent health. Your NBA analogy is spot on! If I got up in front of the world stage and performed like Biden no one would have to send me in for a mental evaluation. Everyone would know from my weak performance(s) that I would not be qualified to continue holding that job. Biden’s mental state is irrelevant. His day to day performance is…and it’s abysmal.

    My 80 yo Father In Law has Parkinson’s and is definitely becoming impaired and he’s significantly more cogent than Biden. It doesn’t take an MD/PhD to see a Parkinson’s patient start to slip. You don’t have to shake a man’s hand to see him decline, we all have a lifetime of seeing the elderly decline.

      I home cared for my father for 5 years and last 3yrs palliative bed ridden, from advanced parkinsons and dementia... what Dr V Prasad is talking about is doctors on both sides making anecdotal and visual diagnosis isn't helpful or ethical from doctors who should know better.... there's an obvious and good reason why Biden is avoiding a cognitive test and that is because, once you fail this test, it's legally binding and you nolonger have mental capacity to make decisions even on your own health and finances, never mind being the US President... so he would have to immediately resign and step aside for Kamala even before the next election! My father failed his cognitive test before he setup legal 'power of attorney' for medical and financial decision making, meaning under the eyes of the law he didn't even have capacity to give capacity to his own family to act on his behalf, and this meant his own wife and children had no legal say in my father's care and we had to ask paid strangers from the state / government permission for anything and everything... as all decisioning making defaults to the government, not family unless you setup it all up with your lawyers before you loose mental capacity. But the problem is alot of people are like Biden and don't want to admit anything is wrong, either out of fear or stubbornness, until it's too late to act,

So, in his increasingly frequent addled states, he doesn't remember that consciously, but he still senses it subconsciously, like a toddler seeing a scary shadow in the window at night. . . if the toddler lives in northern Alaska, that shadow might actually BE a bear.
      Biden inherently knows he's too frail anymore to fight Putin.

    Thank you Vinay, excellent commentary, thank you for continuing to call out those irresponsible physicians and keep this important question before the public! Parkinsonism is not Parkinson's disease, it is a set of observable signs that lead one to consider the possibility of Parkinson's disease. He certainly exhibits signs consistent with Parkinsonism by observation alone (and if you shook his hand and did not detect cogwheel rigidity, or if he did not exhibit micrographia for example, would that change your overall impression of the man? I would say No based on observation alone). The observable signs (festinating gait, mask facies, intermittent confusion, speech abnormalities among others) may be caused by Parkinson's disease and/or other neurological movement disorders. These are manifest and plain to see.
    Having said that, I completely agree that without a history and physical exam, without considering past medical, social, and family histories, without considering the current medications the person is on, it would be irresponsible at the least (if not misleading) to pronounce some sort of formal diagnosis. Perhaps with laboratory and imaging studies to provide further data an alternative diagnosis could be arrived at. You are right to distinguish the diagnostic aspect as completely separate from the evaluation of the degree of physical and mental acuity needed to perform the duties of the Presidency-- and I deeply share your concerns as to whether he has capacity to perform those duties in the outstanding manner that this country and the US public needs and deserves.

    Vinay, it’s a clinical diagnosis made by observation. The neurologist was 100% spot on. I just diagnosed a patient of mine by observing her gait, listening to her speech, and looking at her stone facies. Neurologist concurred and began treating her.

    So well said! There is something in American culture that romanticizes the "common man," the kind of guy "you want to sit down and have a beer with," that devolves into Philistinism. I'm an American who spent half my childhood in Asia where people were widely applauded for their educational achievements. No one was marginalized for being a "nerd" or called such a name. The desire for an American President to fit in with the crowd can lead to selecting mediocre people. Clinton and Biden were exceptions to this rule, but Biden is just a pathetic specimen. I hope he's replaced very soon.

      Masklike facies, bradykinesia, fenestrating gait are all observable and c/w Parkinsonism though the diagnosis of Parkinson’s disease would be a reach. All he needs a cognitive exam not neurological exam. Really don’t care about his reflexes and sensory exam which reportedly are not a problem

    I think you have to ask yourself (and maybe this requires someone old enough to remember) how doctors were trained to recognize the symptoms of Parkinsons. They watched videos showing what the symptoms look like in an actual person suffering from the disease. Obviously, it's not the only test, but arguably enough to form a preliminary medical opinion. But I agree with the idea that a cognitive exam is not required to determine if someone up to the task of running an entire nation. It's clear he is not, maybe he could run an ice cream truck (up and until he eats thru all the inventory).

      No doubt. If I was a world leader, I would be very frustrated having to repeat myself, and spend all the energy required to interpret Biden through the mistakes. At this point, he represents an impediment to whatever his putative goals are.

    This is the K.C. Shuffle: They know that we know that the table is tilted.
    Put that together with what Clooney said: "...The short ramp to election day would be a benefit for us, not a danger." I heard someone call it the "honeymoon" strategy: I met this guy just the other day, don't know much about him but I love this guy and he is here to save the country. Six months later: I never saw that side, not sure how I feel about it now...
    In other words this is not a medical issue for the American people, it's a political issue and looks like a pretty dirty one at that. Best not start thinking about how far back this began.

    It's interesting. We've seen similar drops in cognitive and physical capacity in my uncle who has Parkinsons the last three years. Each corresponded with a covid vaccine. First two AZ he had significant brain fog and he was juiuusssttt starting to improve after the 6 months when he went for a pfzr booster shot and has simply worsened significantly since.
    Biden has had about 6 MRNA from what I can tell. And each time there's a new drop in condition. Food for thought.

    So I’m a medico in Australia (GP of 35+ years ) & seem to remember Dr Robert Malone talking during the pandemic of a harmonisation of media, big tech, government and the medical authorities . From memory he didn’t allude to how or why. Looking in from outside & with a medical background I’m with you Dr Prasad- something is not right and it doesn’t really matter what because fit for purpose is clearly something your president lacks. Politics have nothing to do with it and neither does his main opponent. All the best with your governance( we have enough problems here with our out of touch and ineffectual ruling class)…

    I remember seeing President Biden back in 2021 at a town hall meeting. The President rigidly held his arms so that his forearms formed a right angle to his upper arms. At the time I thought there was something wrong with Biden.

    I wonder how many anticholinergics his doctors prescribed to him over the years. What do you think? Of all the patients with dementia, how many got that way from taking prescriptions their doctors insisted they needed? The guy slinging drugs on the street will tell you that his product will screw you up. I find myself educating doctors about anticholinergics and why they shouldn't be the first line of defense for minor complaints, especially for people my age, way too often. I shouldn't have to do that. But you know, neither the drug slinger nor I were privileged enough to be indoctrinated by Big Pharma via a medical degree either, so there is that.

    If a considertable amount of time, in a presidential debate (or press conference), involves discussing the President's bedtime, we're in a really weird space. A crisis effecting the entire world can arise at 4 AM EST. If a President is largely a figurehead than, I suppose, it doesn't matter. If, as we're told, the President is the "leader of the free world" and is actually in charge then it matters that the guy can only work six hours a day. As far as a diagnosis or a particular pathology, who knows? The bottom line for myself is the following oddity: We're told Trump is an existential threat to democracy and that the house is on fire (that's another debate) but Biden - this cadaver-like creature who struggles to walk and speak - provides the best chance for Democrats to defeat Trump, and, lead the country. My question is: What's going with the DNC? Full disclosure: I have never liked Biden, I've always thought he was a war monger and on the take, and I can't stand Trump. So here I sit...as I'm told my vote would "matter". I just don't give a shit anymore. It all feels like some sort of crazy, dystopian theater.

    Yes, he was not pushed in different domains. He knows those foreign policy issues inside and out, but it is new knowledge that he is going to have trouble with and he was not asked those questions.

    I love how you quote the HIPPA privacy rules regarding that incompetent dictator running this country.     The question of medical diagnosis vs higher standard is the difference between voter opinion on whether he SHOULD be nominated or elected and whether it is illegal to allow him to run or serve in office.
    That is why the focus is on the medical diagnosis. They are trying to avoid the competency question that could affect voter opinion.

    Shouldn't your point also be directed towards those in line to be president? The VP and the speaker. I saw that neurologist on NBC and had similar thoughts. But I like his notion of Biden taking the 4 hr test he mentioned that addresses what you were hoping the press would do with questions and much more. Weird times here in America.

    There is one way in which I think a neurological exam would potentially be useful, which isn't diagnosis, it's prognosis. What's gonna happen over the next 4 years? Does he have a degenerative disorder or could he sustain this (albeit terrible) level of performance over time? In 4 years will he remember his children's names? Will he be able to walk up on a stage unassisted? But it's still horrendous that it's a conversation we're even having. People have drawn comparisons to Reagan and Thatcher's issues, but Biden is clearly so much further along in his deterioration than they ever were while in office. It would be a mercy to have him drop out of the race so he can get the care he obviously needs

    People were pointing out the mental decline back in 2020. He's just gone at this point. He should be perparing for the end instead of "running" the country.
    But what was it Diane Finestien, waited until a week before signing over her estate then passed a week later. Still in office.
    The joke that government is an old folks home is sad but true. And points the a broader concern, if these people are not needed, why are we wasting money on them?
    They should be providing US a service, not US providing them a retirement home.
