    TA violated their own procedures as well, since there were only 5 cars with functioning brakes. There are supposed to be TWO cars with functioning brakes for every car without functioning brakes. Also, the TSS on the scene was to be supervising the move up front, not operating the bad order train.

      Why they cutting brakes for front 5 cars? Like figure out which brakes and car then operate from the second car

      @@williamerazo3921 The fleet was modified into 5 car units. All the operating controls were removed from the other cabs.

      You are partially correct. The procedure was proper as our equipment are 5 car sets. So, if you have a problem with a car in the first set and have to operate from the 6th car, you must cut out the brakes on all five cars.
      I agree with you on the fact that the TSS should have been up front flagging and not in the 6th car operating. The fact still remains. If you lose positive communication during an “Other than head end move”, you must stop the train.

    The MTA needs to launch a new campaign targeted towards teens similar to the “Ride Inside, Stay Alive” campaign but advising them not to -fuck- around with the emergency brakes.

      I agree something should be done about the constant pulling of the EBV, however, I doubt an announcement in the stations/on the trains would stop these kids.

      30 years and I don't remember that much surfing going on, at least not in the IND/BMT. Will admit the evolution of the EBV has kinda sucked. The wire replaced the rope(We had miles of rope, as it had multiple uses), but getting to the valve is much more difficult .

    As an employee here in Chicago, pushing defective trains isn't unusual. Along with radios, we have buzzer signals, hand and flashlight signals to alert the pusher and the flagger

      Problem is, in NYC, when you sectionalize a train, as was done here, you're connected iron to iron only. There's no buzzer communication, so there's radio only. IIRC, in Chicago, the trains have only electric brakes and no air.

      Whoa! Went to Chicago once, rode the Blue line to Wrigley. Didn't get nosey enough to check a cab. You say their equipment doesn't use air brakes?

    When that derailment happened we had another one a week later on the F train between W 8 St and Neptune Ave, it was a slow speed derailment and no one was injured.

    Was a C/R. @pbatommy has it right about the five car string. Who's operating is irrelevant. 96st. Tower(or whatever super tower runs that interlocking) is in trouble as they should never allowed the light train to even enter the station. A red home ball would have affected the in-station approach signals, possibly tripping the sixth car before the first can leave the station. I do believe that the regs say not to allow a light train unless it can clear the station. Other problem: If I remember, on older equipment, a cut cord meant nothing. Open the cab door, reach up, and pull the metal valve the cord is connected back to the closed position. My friend works at PATH, used to work with me at NYCTA, and we came to the same conclusion with your great report. I was a TWU rep in the day, and I can see myself defending the Tower Operator.

    This demonstrates how none of these incidents happen in isolation, and they all are the result of multiple things going wrong at the same time. From some idiot pulling the emergency cords and cutting them off, to the subsequent rerouting of other 1 trains, to the cut out brakes not stopping the train when it passed the red signal, to the breakdown in communication between the flagger and the operator, all of these things had to go wrong for the incident to occur.

    The tss was right in operating the train and controlling the speed. The moment he did not hear the flagger, he
    SHOULD HAVE STOPPED THE TRAIN. Imade this move endless times as a motorman and Tss in the 30years I worked there.

      Unnecessary and impractical. The nearest yard with a diesel is too far away. The “other than head end” move is very safe and effective when done correctly.

    So the flagman instructed a train to pass a red signal. The first statement from the drivers’s union was “management” caused this.
    But it sounds like the flagman was the original driver.
