More about me as a Cosmetic Dermatologist:
Hi my name is Dr. Stefani Kappel and I’m a board certified Dermatologist! I’ve been practicing dermatology for 15 years and have been obsessed with skin my entire life.
Authenticity and honesty is my priority. Unlike many dermatologists on CZcams I do not accept any paid partnerships. I never have and I never will. Everything I post from skincare to in office procedures are ones that I truly believe in.
After obtaining my medical degree I completed a residency in a dermatology at UCLA. I took my first job as full time faculty at UCLA as professor of Dermatology.
It was then my love for teaching expanded and I began to educate thru social media. I post educational videos for my patients and for “skin nerds” like me who want to take their understanding of skin care to the next level.
Thank you for joining my CZcams Derm family 💙
I am also the creator and formulator of MDAiRE Skincare

Dr. Stefanie, I turned 70 today and I found you. I have been doing procedures for twenty years and will share. MY thoughts with you and this community There is a life style a person must maintain to achieve better results. Drink water, eat good food, exercise, use sunscreen, check out the person you will have working on you. See what she or he looks like. Check out the clients in the waiting room to see what they look like. Don’t go for a cheap price or fly by night institutes in a strip mall. A good doctor will cost money. Be prepared to spend.
On the other hand you don’t have to spend a fortune on luxury skincare. I incorporate Nivea into my daily skincare routine every day. Lastly be very realistic with your results. Even the best doctor can’t make you look forty years younger. Now I am off to Pilates.❤
ABSOLUTELY 1000%
🙌I agree too!! What lasers do you do or procedures? Skincare ?
Maria, i did Thermage on my face, chest, tummy, legs with disappointing results. 70 years old is too old for the thermage in my case. I focus on clean skin, I wash my face every day with a mild forming cleanser, followed by an eye serum, face serum, mixed with argon oil. I finish up with a little Nivea and sunscreen, even in winter. I do not die my hair anymore, but I do have a cut every two months to keep the shape I do manicure and pedicure and I don’t leave my house without lipstick. So the new goal for me in this stage of my life is to look as current and fresh as I can dress appropriately but fashionably I don’t want to look desperate. in a few weeks, I will have micro blading on my eyebrows. I would really like to know what other men and women of a certain age do to look current, fashionable and appropriate please answer. Thank you.
@@janetkalo7520lovely info my dear. I am 60.years of age and I don't look above 40. I am with no fillers or plastic surgery. I workout daily doing strength training and pillates, and for my face I follow FACEROBIC program by Ms,. Peta check her out in CZcams it's a daily routine of face exercises, exercising the muscles of the face the same way as we do the muscles of the body. It definitely reverse the aging of the face. Have been doing facerobics 8 years now and my face is firm and tight. No sagginess, no wrinkles on my face, Since you are concern with your appearance I believe in working out your face muscles❤
I thought you didnt like Thermage for neck? Please cover more in depth.
I love how detailed oriented, non-biased, and concise all your information is! Thank you so much for sharing!
I appreciate your use of technical terms & your unbiased opinion. Researching these topics can frustrating because most resources are oversimplified & laden with sales pitches.
I wish more videos would give approximate prices, or price ranges for these cosmetic procedures. A lot of people want to know.
This video was more than I expected. Thank you for the wealth of information. I had no idea where to start until I watched this video. Well done
i believe everything you say because when i look at you here compared to you on your oldest videos, your neck look has insanely improved! went from looking 40 to 20. amazing. i didn't know you can do that much improvement on the neck as i kept hearing the saying 'the neck don't lie (when it comes to age)'. also you don't look overfilled anymore, i really love where you're going with your own treatments, great skill
Brava, Stefani. You really know your stuff! And you have a talent for teaching: I have learned so much from this particular video as my neck is my biggest problem. You did a great (and very exhaustive) master class on this vexing topic. Many of your colleagues here on you tube touch on these topics for neck rejuvenation, but yours has been the most informative. I really appreciate your breadth of knowledge on this topic, and I like your delivery, which is fast paced. I find a faster pace helps me absorb large amounts of information, and in this video there was a lot to absorb, Thank you for your thoroughness, and I have to say, you are quite brilliant!
Thank you! I turn 60 this year and started noticing neck elasticity and lower jawline a bit but not quite ready for plastic surgery. I have been wearing sunscreen on my face and neck since my early 30s. I have had Clear and Brilliant, Botox, and Ultherapy in regards to fillers only lip fillers. Most people think I am in my 40s and that comes from woman younger than me. So I agree Sunscreen for sure but regular ongoing “maintenance” is a great way to keep looking fresh and youthful. I appreciate your non-biased professional overview of the various treatments. ❤
Thank you so much Michelle!!
Well, I’d also add genetics. I’m 49, using all treatments and creams on the planet. And my neck looks quite bad already 🤷♀️

Is there anyway to address and treat vertical lines on the neck? Thank you
wow what a great informative video! you are so thorough! Love that! Thank you! you post such good video's!
This is such an education that you are giving us this is such an education that you are giving us so that we can find the best provider for the the process that will give us the results we want
Sunscreen on neck is very important. I didn’t know when I was younger 😢 and now I’m paying for it…
I had thermage treatment in my mid 50's basically to my face and some down my neck. I believe I was just in the hands of the wrong operator. That was the biggest waste of money I ever spent. It was very painful and no results. I am 66 years old now. The operator was once a facial reconstructive doctor, so one would have thought he knew how to operate the machine. He literally talked me into having this done. Sometime after the consult, when my appointment came, he said, "now what are you in here for?" LOL. Then proceeded to tell me my skin looked nice. That's when I should have jumped out of the chair and ran! Months later, I went to one of the girls there for a facial. At the desk there was another lady he was trying to talk into having the thermage done. He looked at me and said, tell this lady how nice it turned out. First of all, that was breaking my privacy by telling a stranger what I had done, and secondly, I told her it was a waste of money and did not work 🤣. I do believe after watching this video. it depends on who is operating the machine. Sometimes we have to learn from our mistakes. Great video.
I have never heard Thermage was “all that”….period
@@GoOutside321 A total waste of money!
I had thermage too and went to a poor practicioner. Had a huge headache for days and felt it was a waste of money. Cancelled the next appointment, tried to get a refund as I had to buy a package, and the doctors office lost my record???????
@@kaepeach7588 Lot your records! I'm sure they remembered taking your money! You should have been compensated!
Good for you for telling the other lady that it was a waste of money. You may have saved her looks as well as thousands of pounds.

Hi. I have been interested in skin care since my teens. I used Retin-A for acne before it was discovered for wrinkle prevention. It has worked wonders for my skin.
I am interested in products for my neck. It looks pretty good. However, I do have slight laxity underneath my chin. I would like to tighten and maintain that area.
As a lover and user of Sculptra it was disappointing to hear it is your least favored option for use on the neck . I do trust my provider but will proceed with caution.
I actually believe my neck is benefiting from the Sculptra used along my jawline. It's looking better for sure. Maybe a combination of different at home products and the Sculptra.🤔
Thank you for all the info you share.
Wow! Very helpful. My issue to being a slim 47 yr old is short neck/chin skin with short platysma bands. I have a nice jaw but it gets ignored due to the turkey neck.
You are the best, thank you for sharing all your knowledge, it helps me so much to choose the best treatments ❤
Wow! That was a Master class…. thank you!!!! 👏
You're so welcome!!!
I like thermage. It’s one of the only energy based skin tightening procedures I can do safely over my tattoos.
Just what I was looking for…great info. Thank you for the detailed descriptions of each.
Sure would love tips on HOW to find a good cosmetic dermatologist. I’m in west TN. Any tips?
Absolutely love your channel! You’re one of the only doctors on CZcams I trust. Quick question: How many pulses of Thermage would you recommend for the full face without the eyes and how many pulses for the neck?
900 pulses. And thank you for your support!!
i tried thermage on my face, neck, chest, tummy and legs. the procedure was very painful , extremely expensive and i saw little if any results. as time goes by, i realized that i needed more drastic procedures more often with not great results. so it’s time for me to set new goals. i will continue monthly facials and maybe pcp. i am not interested in surgery. i am 70. i would like to know what other mature people do to look their best.
@@DrStefaniKappelWhat depth and duration is safe for the face, to NOT melt facial fat pads? And what duration to the neck, if the goal is to tighten lax skin, and target a bit of excess fat in submentum?
Thank you for the great information and advice you bring on your channel. Take care.
Thank you!! So glad you find it helpful :)
Excellent presentation thank you so much Debra Bader
I am very interested in the client, Bella. I’m not sure if I spelled that right procedure I’ve had a double chin my whole life when I was really skinny and now I’m a little overweight. I’m also almost 68 I live in Florida do you have an office anywhere close thank you Debra Bader.
I'd love to know your thoughts on opus plasma and how it compares to thermage technology. I'm a fan of ellacor for loose neck. Had 2 so far but it will take more. So far suble changes but more then microneedling and ultherapy combined. Thank you for this video!
11:30, they will destroy the dermal plexus doc. Therefore I do advise my patient prior to doing a facelift (and those who did energy-based devices for years) to go for a hyperbaric chamber before the surgery. Very nice and informative video. Thanks
Can you explain if Arazlo is a retinoid? How does it work? Would you recommend it? Thanks! Love your videos! ❤
Thanks for sharing all your knowledge and expertise. 👏🏼👏🏼👏🏼👏🏼👏🏼
Thank you so much. Can you review at home devices we can use between professional treatments
Yes I'll post a video in the future on this subject but unfortunately most don't work and they can be expensive with not much return in investment 😐
Just found your channel. I'm looking forward to watching more of your videos 😊
Yay! Thank you!
Thank you, Doctor! This is the video I needed! 60 year old here looking for neck help!
I love your videos so much. You always provide so much good information
Glad you like them!
I love your channel and learn so much from you. If I lived in CA I would be in your office weekly. I love in Dallas, Texas and unable to find anyone that can do what you do. Even when I go to a fancy place and they are over pricing everything the results are not there. What Doctor do you recommend in Dallas Texas. It's sad 😢😭 to spend money to find out I've been played with a clinic taking money 💰 and not educated to do what they advertise they can do.
This is very encouraging information ! Thank you
You are so welcome!
Hello Stefani , I've noticed a significant improvement in your neck compared to your videos from five years ago. Could you please consider making a video to inform us about what you did for your neck improvement?
Thanks again for an excellent video. Clear and comprehensive. Lots to ponder. I think my neck is in pretty good condition, despite my decades - my long thick hair might have helped. Wish I was near you. Always nerve racking finding good providers for laser etc .
It is! And I'm happy that I could help!
@@DrStefaniKappeldo you do referral’s for Central & No. California?
Love all your videos!
Hello Dr.Stefani … I’m reaching out because I am interested in getting thermage. When I called the dr office in my area they suggested exilles or tixel. I automatically went to your website to see if these were some of the services you provided and you Dont. To be honest I Dont recall if you have mentioned them in one of your prior videos. Nonetheless; can you provide your opinion on them. It was informed to me that Tixel is the service that’s replacing thermage. Is this also true? Thanks Dr. Stefani for providing so much necessary info to take care of ourselves ❤
Thank you for about her very informative video!
Truly superb explanation of different techniques !! Can one also have non surgical after having surgery ??
Thank you!!!
I really enjoyed this video!! I’ve lost 50 pounds and now my face doesn’t look so good which is so disappointing!! I’ve had threading done on my jawline and Botox in my neck but still very unhappy with my neck. I’m thinking thermage is my next option. Btw I’m 50. And you are SO easy to listen to. Relaxing lol
Thank you Dr Kappel!
Do you do Thermage on Fitz 5 skin? I wonder if the heat causes hyperpigmentation. Out of all the ones you covered, that one sounds the most interesting to me, but not sure if it might cause more dark spots than I already have. I have general loss of plumpness in my neck due to being a thin person mid-50s. Not really any wrinkles, just lax and seeming to have no subcutaneous fat in that one area of my body. Overall BMI is fine at 21, so don't want to have to gain wait all over just to try to plump up my neck.
I had dysphagia after Botox in my platysmus muscles--it was terrifying to feel like I couldn't swallow! Not for me. Thank you for presenting all of these other options for turkey neck!
Always inject less than 20 units and do not inject too deep. As long as an injector follows these guidelines dysphasia shouldn’t occur. I’ve never had that complication in my 20 years of practice.
@@DrStefaniKappel thanks! I have been too afraid to try it again, but if the neck bands become more prominent, I will consider it. Your information is wonderful!❤
Hi dr Stephanie you are so pretty and thanks for making these informative videos can you please talk about how to control saggy jowls and how to best fill under eye hollows
Dr Stefani what is your view on profhilo ?Or skinboosters? IN Europe they are used to a great extent.
Dr. Kappel would you please tell us the work you have had done and what you do to maintain your skin quality?
I've done thermage & lasers consistently since my 20s, as well as high quality skincare and photo protection every single day since my 20s as well ❤️
Very informative video ..i feel I have learned so much just in this video. I just had Ultherapy treatment..and with my treatment..i will receive 3 Elixis tteatments..my question is can Elixis tteatments..cause facial fat loss..?
I agree with what you said about surgeons trying to push people away from these treatments and into surgery. I used to read the Real Self website (where people ask questions and surgeons answer them), and every question about beauty treatments is just answered by the surgeon saying to get surgery instead. I've seen questions where a 30-year-old wants plumper lips and he tells her to get a face lift. Nothing but a bunch of vultures.
Loved your videos thank for all your information I will love to be able to make an appointment with you but I live in Nevraska.
Excellent information. Could you please share what you do for your undereyes to keep them so youthful? What treatments. I never would have believed your age. Thought you were in your late 20s. I have issues with this area and am adverse to filling tear troughs.
Thank you! I've been lasering my under eyes since I was 22 and I've used eye serums my entire life. I think thats the biggest thing thats made the most difference.
@@DrStefaniKappel what types of lasers are good for under eyes?
@@DrStefaniKappel I've never seen or heard of lasers that can plump up the under eye like that! It looks like your skin is an inch thick under your eyes, have you not ever used filler or skin boosters in that area? My skin is like rice paper under my eyes and want to thicken it, but lasers have never worked for me:(
Love ur channel!! Wish u could link some reccos on what u spoke a about with skincare that’s not too expensive;) ❤ u look amazing!!!🎉❤😮

You are wonderful! Thank you for sharing your knowledge! Have you heard of or have experience with a Jet Plasma procedure?
thanks for all that useful info would you say the Nira Pro Laser is good for the neck area and did you mention Helliocare the spf? contains one of the new ingredients x
I love your videos and have done a lot of research on various procedures. The only area I think you're lacking is doing more unbiased research on microneedling, especially when paired with radiofrequency. As you've pointed out: thermage gets a bad rap because many people have had their fat melted. Similarly many people have had scarring or no results with microneedling. I see a wonderful NP who can do medical microneedling on me and I have not gotten radiofrequency done as I went to her freaking out about it melting my fat. She talked me through the issues you've mentioned with energy devices and user error. I've never gotten any treatments done until now at the age of 38 as I haven't had any signs of aging until recently--a bit of unnoticeable (to most people) jowling and neck laxity. So far I've noticed in just two treatments of face and neck, a lift. Like I'm happy with my results. I do not swell much or get very red. But that said, it does do a similar thing on a cellular level by kick starting fibroblasts into producing collagen and elastin. I don't think it's good for moderate and severe laxity, or wrinkles, etc. but for those of us with mild signs of aging, it may be an option. It is pretty painful but no down time. At least for me which is very important. I do not want to be walking around red and bruised. I'm quite prone to bruising. That said--I would LOVE to get THermage done but I do not know how to find a reliable and skilled expert. I've read so many horror stories I'm terrified. I weighed the pros and cons of both collagen induction therapy aka microneedling and the thermage energy based approach and found myself being able to find a reputable NP for microneedling. I'm wondering if you have a list of practitioners nation wide you trust? I live in NY so seeing you would not be possible. You do know your stuff and your face tells me you know what you are doing. It's hard to tell. As not all MD board certified cosmetic dermatologists have started at a young enough age to really get such amazing results like yourself.
So I guess, how do you know if the person who is performing any heat based/energy based treatment on you will not F up your face? lol To put it bluntly. What are tips?
I appreciate your ethos. I am 68, and have wrinkles above my mouth. What can I do please?
Thank you for the video. I have a question. I am 68 and my neck is very wrinkled and I have OPMD. I would like to have a procedure done. Is that possible with my OPMD?
Really interesting about situps etc causing increase in size of platimas?sp muscles.
My neck is def showing my age. I started that RF frequency treatment but I don’t think it showed a difference ? I bought a home device ( NuFace) and I apply tightening cream to my neck and vit c and sun block. I’m not sure if I’m going to get a neck lift. My mom has the same neck. I think it’s genetics. Neither of my sisters have this issue but I also notice I am a side sleeper and my neck folds while I’m sleeping !
Before and after is great but I would like to see post treatment pics and healing pics and time of healing
Please, what is the name or model make of the newest Thermage device so I know what to look for in a treatment. provider.
Do any facial exercises help with platysma banding on the neck?
what do you think of excillus laser for the lower face and neck?
😊And one more question… I’m a professional violinist and have played this instrument since I was four years old. I’m almost 70 now and of course, my neck skin and tissues have very much been affected by holding this instrument under my left jaw. It’s held almost under the angle of the jaw. And it is also very supported by the clavicle and triceps. The instrument itself is not very heavy-maybe 1.5 lbs.-but I’m holding it for literally hours at a time. I teach of course and also perform so my neck gets a lot of workout. The chin rest (misnamed, it should be called a jaw rest), pushes the skin/tissues under the jaw forward so that you end up looking like you have a balloon under your neck. Is this an area I should just forget about doing anything to bc I’m not stopping violin anytime soon.🥴
I feel in your case that is just apart of you and actually really special. Its like surgeons always having tough skin on their hands from scrubbing for so many years. I say just leave it be because anything you do may compromise they way you hold or feel when playing the instrument. Quality of life always comes first and I say that as a cosmetic dermatologist where cosmetic outcome is usually my priority. Yours is a special exception. Congratulations on your successful career s a violinist! That is amazing
@@DrStefaniKappel You are just so sweet! 💖Thank you so much for your kind words. I will just accept that part of me as it is.🥰
Don't stop violin! A musical talent is one of the greatest gifts. As much as I fight aging, I would definitely give up a good looking neck in favor of being able to play beautiful music.
@@dammar117 No worries! I’m not giving up my instrument any time soon! 🥰 My own teachers taught well into their 70’s and even 80’s. I’m still playing and teaching and will continue to do so for the rest of my life. I can’t even imagine my life without the violin and orchestral music in it.💖🎻🎶💖 Thank you for your lovely encouragement!💖💕💖

Hi doctor. Would radiesse also work on the neck from weight loss and for creppy skin?
The neck skin lacks sebaceous glands? How do I get occasional pimples there with no sebaceous glands?
Dr.Stephanie, this is an old video so not sure if you still recognize comments? But, I had the fraxel done when it first came out and I believe the plastic surgeon was new at operating it and possibly had settings wrong but, don’t know for sure. I had tremendous pain during the procedure and had numbing cream on for an hour in advance. I ended up with major swelling that lasted 5 days. I looked like a sumo wrestler. That was several years ago, but, I thought it was a lunch hour procedure not , a big recovery time. In any case I wondered if it was an allergic reaction or operator error. I went to some one reputable that I still respect because mistakes happen . I just never finished treatment after having that unexpected painful and slow recovery. Any thoughts on this?
What’s your thoughts on the ingredient of Acetyl Hexapeptide 8 causing drooping of the skin in some individuals?because it happened to me after using a skincare product as directed for weeks then it all went South literally! The area between my eyebrows the skin was sagging and drooping!
The amount should never be enough to cause a droop. That's terrifying! That's what assays and clinical trials are for.

Hi Dr. Stefani! I love learning from all your content… ❓❓❓I have a question about a treatment called EMFACE -I am considering this for more lift. It is described as It’s one of a kind facial laser treatment which targets muscles and skin simultaneously to tighten, lift, and tone the face and helps restore the facial contours all without pain, downtime or the use of needles.
EMFACE™ uses Synchronized RF and HIFES™ technology .
The buzz is that it’s showing good results so far… It’s relatively new. Wondering what your thoughts are ? Your opinion is so important to me. Thank you!
I love EmSculpt but not EmFace. Botox is one of the best antiaging treatments we have. It works by softening muscle movement to relax wrinkles. So doing a procedure that goes against that is counterproductive, making wrinkles and etched in lines worse.
@@DrStefaniKappel thank you! 😊 would you recommend radio frequency instead?
Thank you Dr Kappel.Very interesting. I always learn a lot from your channel. I would love to start using Mdaire vit c serum as it contains ascorbic acid but I was wondering if I could use it with a stem cell/ growth factor serum. I was told that the low ph of ascorbic acid would effect the efficiency of the stem cell serum. Any advice would be appreciated.
I purchased her vit c serum and it’s really good. My similar question is if I can use it with hydroquinone 6% and, if so, which order would I apply?
Yes of course! The delivery systems of the Vit C are innovative and allow the product to rapidly absorb within minutes. I'd put the sc/gf serum on after and there shouldn't be any problem.
Does Dr Kappel offer a growth serum? Or, does anyone know which one she recommends? I’d love a video on GF!
@Dr Kabbel. What do you think about those exercises that can be done to the platysma muscle as well as using a towel soaked in Epsom salt to slap the neck repeatedly to promote collagen synthesis. Can those be used as maintenance in between Botox or Thermage treatments? What do you think about Ulthera versus Thermage for the neck to tighten and to stimulate collagen. Thank you. I'm 55, the inside of my arms are 80 😒 and it happened so fast
@@josie4peace so like put it on our arms all over or in the trouble spot? Is differin gel good?
Thanks for the informative video! Will Thermage work on neck laxity due to weight loss? Or does that require a surgical lift? ❤
Thermage, about 12 years ago, didn't do a thing for my neck or face except leave a few burn marks. Luckily I won it in a drawing so did not have to pay for it but I guess I'm wondering if it should have been done more than once.
@@sable7114 I’m sorry that happened to you, but thanks for sharing your experience.
@@jessicanoir6217 thank you Jessica and you're welcome!
Thank you !
What if it is loose skin mostly in the double chin area? All the women in my family are thin and have this droopy chin. The derm I saw recommended microneedling to produce collagen but will that work if that is just my anatomy? I have always had it but am 34 and really hate it
