When I lived in Thailand, I once walked down the stairs in the middle of the night to get a glass of water. It was pitch black, so I reached out for the banister and unexpectedly placed my hand on top of another hand. Except it wasn't another hand. It was a huntsman spider. My guess is that 2 containers were used to make the ice cube. There is a hand sized depression frozen into the top of the block of ice.
The top of the frozen block then becomes the botton, leaving a rim of ice around the edges to hide your hand.
You just sit the block of ice onto your hand, the thin section allows you to see the hand through the other side.
The ice was only sitting on his hand and wrist, check out the technique used on the flip-to-smash.
