  • This video discusses motor and sensory dysfunction resulting from hemisection of spinal cord (Brown Squared Syndrome).
    Spinal cord hemisection causes injury of motor and sensory tracts in the posterior, lateral and anterior columns of the same side in addition to the dorsal and ventral horns.
    - Tracts in posterior column: Gracile and cuneate tracts that carry touch and proprioception from same side to the opposite cerebral hemisphere from lower and upper extremities respectively. Keep in mind that Gracile and cuneate tracts cross to reach to the opposite cerebral hemisphere in medulla. Injury of gracile and cuneate tracts in hemisection causes ipsilateral loss of touch and sense of position (proprioception)
    - Tracts in lateral column:
    1) Corticospinal tract (injury of which causes upper motor paralysis of the same side of spinal cord injury). Keep in mind that corticospinal tract descends from the opposite cerebral hemisphere and crosses in medulla. Upper motor neuron paralysis is characterized by hyper-reflexia and positive Babinski reflex.
    2) lateral spinothalamic tract, bringing pain and temperature sensation from opposite side of the body and 2 segments below the level of injury (injury of which causes loss of pain and temperature in the opposite side of spinal cord injury and starting 1-2 segments below the level of injury),
    3) spinocerebellar tracts, carrying proprioception to cerebellum (injury of them causes ataxia in the same side, however this ataxia is masked by paralysis of the muscles due to injury of corticospinal tract)
    4) Injury of dorsal horn/root causes ipsilateral loss of sensation in the skin area (dermatomes) supplied by the same injured spinal cord segment (at the level of injury)
    5) Injury of the ventral horn/root, causes lower motor neuron paralysis of muscles supplied by the same injured spinal cord segment.

    14:00 why not loss of pain and temperature 2levels below the lesion sense it’s STT ?
