  • The Usmle Step 1 has changed from a numerical grading system, to a pass fail system. In this video, we discuss whether these Usmle step 1 pass fail changes are beneficial or harmful to medical students (MD, DO and IMG).

      Dear Dr Challa, I am a first year med student. I am planning to take my step 1 in June 2022. I heard the ECFMG would be having a seperate list for accreditation of schools from 2023 or 2022. Can i get an ECFMG registration/ token number before December 2021 as my school is registered in the world directory of medical schools as of now and take my step 1 whenever i wish to that is in June 2022? Or am i supposed to register myself only before taking the exam? Please reply.

    If I take my step 1 exam as an IMG in late 2021 (before it changes to pass/fail). but I apply to residency when I finish medical school will the residency programs consider my step 1 score or they will just look if I passed or not because by that time all of the American graduates will have a pass/fail score.
    -basically I am planning to get a high score to stand out as an IMG but I can't find an answer if my step 1 score will be relevant or not by that time. any help is very much appreciated!.

      So everything is merely speculation at this point, because, no matter how the organization wants the system to be, we will only find everything out 1-2 years after the pass/fail system has been implemented after looking at the data reports.
      However, if you take your test before January 2022, your Step 1 numerical score can be seen by the program directors until 2025. So if you take your step 1 and get a high score, it will definitely work in your favor if you apply before 2025.
      This can be a double edged sword though. If you take your exam before 2022 and get a low score, the program will be able to see that too. So be careful and take the test only when you are confident and your NBME scores have been consistently high.

      @@DrChalla thank you, i have looked everywhere for. can you please share any source that mentioned the numeric score can be seen by pd until 2025. this means a lot to me thank you again doc

      Hazazi Hazazi Thank you for bringing this to my attention. After your comment, I’ve done some extensive research and realized that this 2025 was just a suggestion that “may” happen. However, as of now, it’s unclear when the program will receive the scores. Therefore, still focus on the getting an acceptable score and follow the same tips I’ve mentioned above. Sorry for the misinterpretation and thank you again for alerting me. I will contact all the people whom I advised this to and I will address this in a future video.

    Hi... Do I still study for step 1 vigorously, I mean like the way you've mentioned it in your videos, even if its a pass fail.
    Also I have been planning to give it in December 2021, but if I'm not well prepared, I'll be going for the pass fail system. Should that effect the way I'm preparing for it?

    Sir, my only concern being about even though step 1 is been changed to pass/fail system in 2022 , will there be a distinguishing between the students who had passed( I mean pass is pass remains the same). That is the scores attained by them in step 1 for getting passed will be visible to the programme directors or its just and just pass and fail

    i wanted to ask that do you know that if step 1 is going to be pass fail on 26jan 2022 so if we give our test on 5 jan so will we get three digit score or pass fail considering the fact that it takes 3-4 weeks for results to come

    Dr Challa, this was a much needed important video, so Thankyou. I am planning to give the step 1 in late 2020 as an IMG. So, if everything goes as plan then my matching should be in 2022. I read your replies to the same query but still was unsure of how I would be graded in 2022 during the matching round? will I have a upper hand if I score more and have less contacts and everything then other students?

      If you score more than other applicants, even if you apply after 2022, programs can see your score till 2025. So a good score, even when it’s not supposed to matter, will always tip the program director in your favor. I’ve heard many people thinking that they can just pass because their score won’t matter after 2022, but I don’t think that’s true. Although the systems changing, if your score is visible, the program director would still slightly be inclined towards the candidates with a higher score. This is my thought on the issue. Again, this is just speculation at this point, but, if I were a program director, this is how I would think. Hope this helps :)

    Hi! Quick question, Do you think this will cause programs to look at an application more holistically or will they immediately filter based on step 2 score? Asking for Imgs. Just because medical schools in my home country have some weird scoring systems

      This is a question that I've been asked alot. I will be making a separate video about this because this depends on alot of factors. Assuming you are a Non-us img:-
      1. First off, taking your exam before 2021 is a good idea provided you are confident you will get a good score. If you take your exam in 2021 and get a low score, it can possibly negatively impact your chances.
      2. This change will force the program directors to look into your resume a bit more. So if you have alot of rotations, research, volunteer work, etc then, you would be a strong candidate and you can take your test after 2022. However, if you are like most IMGs and you don't have much on your resume, then you should either start focusing on these things as soon as possible or focus on crushing step 1 (which is easier said than done because the determinants of your score is multifactorial)
      3. If you are an American citizen, then it doesn't really matter. American citizens always will have a little bit of preference over non-us imgs
      4. At the end of the day, contacts is everything. If you know someone that can help you get interviews, or you yourself are good friends with American faculty that can push for you to get interviews, then these strings would be easier to pull after the pass/fail system has been implemented.
      There are some more small factors to take into consideration, and I will go more into detail my next video. However, this is a very person specific question because there are many determining factors

    Great video! I have a question though. If I take step 1 before the change, let's say, on october 2021, before the official change. But take my other steps after the change, on 2022. Which system will evaluate me? Pass/fail or score?

      Good question! The numerical score that is given to you can be considered until 2025. Even though these changes are taking place in 2022, the people who took their exam before that will still have a valid numerical score. Its uncertain how the program is going to filter out the applicants, however the score is visible to them until 2025.

    Are there any step 2 resources that might be relevant to use in my first year of medical school?

      I'm not sure, but if your medical school system is similar to ours in India, then first year mainly covers 3 subjects namely Anatomy, Physiology and Biochemistry whereas step 2 covers other subjects: - Medicine, Surgery, Gynecology, Pediatrics, Psychiatry
      Your first year probably has more relevance to Step 1 more than step 2. And if that's the case, you could go with Kaplan. Kaplan teaches its material subject-wise and they are very comprehensive and give you a great understanding of the concepts!

      I failed my USMLE exam 2 times first was 2020 second was 2021 and I lost money on review questions and also wasted a lot of time studying which never helped me .

      I'm not sure about the process of becoming a pa or a nurse. However, i'm sure that the qualifying test for those might be different. Therefore, i'm sure that even if you don't match, it shouldn't be a problem to pursue a different career.

    Sir, As an Indian MBBS student who will be completely by 2022-23 how can I increase my observation and get clinical rotation in USA?
    And won't the PASS/FAIL system make it difficult IMG to get into desired residency?

      You can apply whenever you'd like. However when you apply, you will have to choose a 3 month eligibility period (In your case it can be Oct-Nov-Dec, Nov-Dec-Jan or Dec-Jan-Feb). If you are unable to apply take your test in these 3 months, your eligibility will be extended to the next 3 months for a small fee. However, if you don't write it in these 6 months, you will have to pay the entire fee again to re-apply.
