I expected the plot twist to be the kid being alright, but I didn't expect a *second* plot twist

That's what cyanide and happiness is all about. It gives you an expected plot twist, and then *another* that you can't guess.
Actually there were 3 plot twists:
SPOILERS
1) the boy’s wish for death
2) the boy surviving and being cured
3) the boy being killed anyway
They KNEW you predicted the first twist, so they put in a second twist
They always surprise us
Imagine the backlash if he didn’t fulfill the kid’s wish. Would ruin the integrity of the foundation.

It was never hooked up to him in the first place. The doctor was a wrestling referee just pretending to be a doctor so he got a fake heart monitor to make it seem like he knew what he was doing.
