    While watching this I had a AH-Ha moment. It started adding up. I see so many versions of keto and carnivore on these channels with all different requirements for this many or that many carbs or carnivore with fruit and honey and raw milk and yet these people almost always are super fit. I wanna be like that but if I eat too many carbs or add fruit or honey the pounds will come back on. I see now from watching your podcast here that I am not 2 -3 years into being fat adapted. My insulin is not being suppressed yet so of course I shouldn't be adding in the carbs yet, like fruit, even though they can because they are already there (fat adapted, insulin suppressed, 2-3 years into keto) I wish I had known this about a year ago. So now I'm going to stick to "as close to zero total carbs" without going over 20 total carbs per day. Thank YOU so much for this podcast. It's going into my AAAVeryBest podcast saved group as I type this.

    This timeframe makes so much sense. Thanks for the perspective. I'm 3 months carnivore or ketovore and am so excited to someday fix my resistance. Its a blessing that it can be reversed.

    Right now, I'm zero carbs because while my A1c has finally reached 5.3, I'm still determined to try to reach 5.0 (or lower) with your brilliant guidance. 🥰

      To say 0 carbs is a dream. Protein makes glucose. The liver likes to have a good storage of glucose for those famine days. Fat is the only 0 carbs macro, but again the liver can make glucose out of fat too in an emergency. The majority of the foods we eat, even if wholesome, have some amount of carbs; eggs have up to one carb. I am keto with two meals a day, never overweight, but I was prediabetic (which has resolved) as my genes have misbehaved as I get old. I have practiced moderation all my life, but these diabetic genes caused me to reevaluate my lifestyle, and I am now very strict since moderation did not work. Good job with your A1C; mine is 5.4 also aiming to get lower.

      You may think you are zero carbs but the body needs 1 teaspoon of sugar in 8 pints / 5 litres of blood to function.

    I appreciate how your stance on carbohydrates is evolving and becoming more nuanced. I'm 56 years old, lean, I surf everyday and supplement with weight training. My liver fat is only 1.4%. So for me, 100 to 150 g of carbohydrates is very much a low-carb diet. It's almost a ketogenic diet actually because my energy needs are so high and my metabolic health so good.
    But for somebody who is obese and diabetic, they clearly cannot be messing around with that level of carbohydrates.
    I agree 100% that counting carbohydrates from vegetables is absurd. All vegetables should be considered a freebie.

    Excited about the fiber video .. I can’t really tolerate any at all! I I now understand why I always tended to be constipated , even when I tried all the high fiber stuff ! I don’t eat fiber at all anymore , zero issues !

      Same here, Kim. It was a total revelation to learn I can survive without fibre. I tried stuffing myself with it for decades, as my GPs continuously advised me to eat more fibre. As a vegetarian, I really could not find ways to up my intake. I learned from Paul Mason that some folk respond to zero fibre and it changed my world.

    This explains what I do now that I'm getting to the end of the very strict carb diet. I believe I can make good choices knowing I can eat two meals and do it correct.     Thank you for giving a realistic timeframe to fix insulin resistance. This is something that is rarely stated and needs to be for people working at fixing it. This is not an overnight fix for many who travelled too far down the resistance road!

    Been mostly carnivore for 2 years, still drink black coffee. Definitely not ready to add back any carbs. Maybe in the future I'll be able to eat some berries or some greens. But I'd like to see more about why they are necessary, if at all.

      They are not. What's necessary is the stimulation of insulin in order to ensure your metabolic function is not impaired by being dormant. In other words, your pancreas is functioning normally.
      You may be doing this as a carnivore through gluconeogenesis.

      I'm not buying his "insulin suppression" theory until I find some credible research papers on the subject. Protein in sufficient amounts provides a decent stimulus of insulin. And I'd like to know what "non starchy" vegetables Dr Cywes proposes to obtain 100 plus carbs daily.

    Looking forward to the fiber video! I was an OMAD girl for 3 years and stopped counting macros after I reached my goal weight. Turns out I have vitamin and mineral deficiencies! And leaky gut. Pay attention to these details he mentions because you can talk yourself right into being unhealthy. There’s a RIGHT way to do it!

    This is the first of your videos that I have watched that raises more questions than it answers. You said in an earlier video that the liver produces all the carbs needed for those functions that require carbs for fuel, and that we really do not need to take in those carbs from an external source. Now you are saying that after a couple of years of being carnivore, our pancreas has stopped or significantly reduced insulin production, and that we need to reintroduce some carbs to get insulin production up again. That does not sound like a very good balance among our internal organs. After all, we are getting some carbs from the meat, and what about the carbs manufactured by the liver? If we are actually carnivore, that should be enough. Of course, in the beginning of this video, you are saying that humans are not pure carnivores, and perhaps they never were. Given our dentation, I would agree with that; we are omnivores, but with a preference for meat, and enough so, that our bodies are in fat burning mode. For those of us who are carb addicts, and at the beginning of our journeys, the future at the 2-year mark does not bode well if we have to throw a significant number of carbs back into our diets to get our insulin level back to where it should be. If that is the case, then there is something very important that is missing in this approach to getting the body healthly again.

      Please be kinder to yourself. It's human to crave addictuve carbs, and it's human to turn to carbs for comfort when we feel strong emotions (anger, grief, loneliness, frustration, etc.).
      I can't tell you what will work for you, but I'm 66 and have found that two main things are critical:
      * a meat-based diet
      * noticing my feelings and discriminating between true hunger versus eating to fill a different need.
      I wish you the best! Be kind to yourself, not punishing. 🥰

      Don't let reports of people doing this 'perfectly' and/or having amazing results quickly discourage you. Most of us fall of the wagon occasionally. Sometimes to the point of severe setbacks.
      Don't give up on yourself!
      What has helped me in those times, is just ignoring calories and weight and all that and putting all my mental energy into eating the right things for my body. Even if I still stuff myself senseless. Because with meat and eggs and fat...that usually stops within days and my appetite starts to self regulate again. Do not try to change your diet and eat less/skip meals, at the same time. I know it's tempting, because of that surge of motivation at the beginning of a diet...but it can come back and bite you in the ass, really quickly. Eat that meat and that butter and drench your veggies in olive oil, until your appetite starts going down naturally. Your body will calm down and stop making you stuff you face very quickly, if it doesn't feel threatened/deprived anymore. You can't win this fight with willpower, you gotta treat your body more like a kid or pet, then a prisoner of war, doing forced labor for you. There have to be rules, yes, but also love and kindness. Not just hostility, threats and punishments.
      Be nice to yourself. Start with the attitude: 'Hey body, what do YOU need, to be happy and cooperate with me, here?'
      I think, many of us, who maybe didn't have a picture perfect childhood, just never really, fully matured all of our character from child to adult. There a still remnants of a scared/insecure/lonely little kid inside and that makes us act out and self sabotage in times of stress. And if that's the case, you gotta acknowledge and embrace that part of yourself and make it feel safe and loved.
      It'll stop throwing tantrums at every opportunity then.
      Well, that's my 2cents and conclusions, after 2 decades worth of diets, exercise obsession, binge eating and self hate.
      If your binge eating is a mental/emotional issue, the change has to start in your head. Eliminating drug like carbs
      helps, sure, but it's not all there is to it, for some people.

      I was intrigued that he views vegetables as free; that makes counting net carbs and limiting them to 30 or less feasible for me, not to mention way less to log into Carb Manager-yay! Are we still to count carbs for starchy vegetables according to him??

    I admire your humility as you learn new things instead of doubling down like the internet gum flappers. It scares me that carbs will eventually become necessary again because I know I cannot handle them psychologically. Can you do a video that discusses how one can eat carbs in a way to not trigger addiction when the point of insulin suppression is reached? How does this not blow a major hole into the addiction model?

      That is a good question.
      However, carbs are in every whole food you eat except pure fat. So you are already eating it, hence the limitation. Their is no such thing as a zero carb diet unless you only eat fat.

    120-130 gram of carbs to me, as an addict, sounds like a ridiculous amount. Any amount that allows me to eat a sandwich is waaay to much. A true addict can only abstain, not moderate

      I am a confirmed addict. I seriously restrict all carbs to be in ketosis. If I ever am metabolically okay to add carbs, I must nonetheless NEVER eat sugar, flour, wheat, grains or other refined, white stuff. Sugar and flour are my nemesis and a line I cannot cross without going off the rails. That’s my bright line.

    It was eye-opening to hear the topic presented comprehensibly. I'm 4 years on keto. I definitely feel metabolically healthy now (should get some blood tests done - I have no idea what the results will look like). I often consume my vegetables raw and blended. I imagine I absorb more nutrients this way. Lately, I was thinking about introducing some carbs into my diet.

      @@saren6538 Have done any measurements to support this statement? I haven't done any. I don't even measure ketones. Mostly, because I'm lazy and I don't quite need to be strict about the diet because I have a BMI around 22. But going just by how I feel, blended veggies keep the blood sugar lower than boiled veggies. I blend veggies because otherwise I spent 45 minutes chewing each meal, which is annoying. I am a slow eater and I chew meticulously. :-)

      @@jiriwiesner yes ive blended food ate it and tested my blood sugar 2 hours later - blood sugar gave a reading, then next day repeated it eating the same food and not blending it - blood sugar gave a higher reading. ive done it with 3 different types of food. as i said the blender breaks teh fibre (which meshes your intestines to slow the sugar spike) .... in fact this is common knowledge now theres a youtube video somewhere as well ill try dig it out... lot of people i know .know this also

    keto since 2.2022. carnivore since 1/2023. Blood sugar has stabilized, neuropathy subdued for the most part. Looking forward to complete lack of carb cravings and normalized Insulin production.

    I have been eating only whole foods and no grains/starch for the last year counting carbs as "net". As I only eat vegetable carbs with my fat and protein, from what you have said these are 'free carbs' and dont need to be counted so actually my net carb count would be correct or even too high if i have understand you correctly. So taking this to its full meaning, a dinner with meat, cauliflower broccoli and sprouts (no potato) will have a count of zero veggie carbs as far as the body is concerned.
    From the many reviewers I have seen here there seems to be a one size fits all model that's not helpful, meaning, there are two diverse types of carb eater. One getting carbs that predominately come from processed foods, and the other by carefully selecting whole foods only but with cruciferous and other veggie sources. I dont think these groups should compared the same when counting cards to achieve a daily target of 20g. they need to be looked at separately.
    Given your carbs statement are 'free' I believe my net carb is nearer the correct rather than total carbs I eat.

    I gave myself permission to eat below 100 carbs at first, then 50, now 30. The thing is, the only way keto eliminates my hunger is if I go below 30. So, doing the math, 50 carbs should allow me to lose weight but without the hunger suppression, it's pointless. I know so many big people who don't want to do keto and make excuses, but if they could experience this lack of hunger for a day, they would be hooked.

    Doctor, you talking allot about carbohydrates and starches, but never about resistance starches, which they say that when you deep freeze any kind of bread, rice or even potatoes for two days and then reheating it becomes résistance starch, in which our body does not absorbs it and your insulin does not spike? Could you talk about resistance starch?

    I am a 76yo type two. I was rushed to MUSC in Charleston, SC in October 2023. I remember nothing fro 3 days. I was told my A1C was 11. Since they time, my CGM shows a 90 day GMI of 9.2. I discovered Dr. Ekberg in January 2024 and have been following his 2mad since I discovered you channel the first of March. for the last three weeks I have basically done omad. We eat dinner around 4/5pm daily. I only consume 2 cans of sardines until the next dinnertime. My blood sugar has been above 200 to 300 every day. I know that I have to continue this way of eating for some time to come. Do you have another suggestions for me?

    Interesting on insulin sensitivity taking 2 years. It took me approximately 6 months with insulin score of a 3.

    Thank you for your wonderful advice. I've never suffered from sickness or allergies 🙏. I'm now 67 and looking for optimum health to continue improving my marathon times. I see so many marathoners relying on huge carb input to fuel runs, but ..... I see this as unhealthy. I started keto a few months ago then decided to do more carnivore and stopped the kale 🤮. I'm on OMAD. I run around 13 miles most days.
    Should I reintroduce more veggies into my diet?? I seem to survive all the crazy variations in diet info. I currently have a fridge stuffed with papaya after watching Paul Saladino!!
    So .... How do I decide the RIGHT best and healthiest athletes lifestyle?? Thank you. 😊

    I was low carb until 3 months ago. I messed up with carbs,icecream and alcohol. I had a video conference with you on 11 Oct 2022. I am now low carb, no alcohol and sweets since 19 Oct 2022. Only consuming ghee, pressed coconut oil and butter and off course meat. I also consume leafy vegetables like cauliflower and brocolli.Please advise when should have my next blood work done. I live in Melbourne Australia.

    I have always counted Total Carbohydrates when counting them, the whole “net carb” idea seemed like a lie to me from the start or a cheat and when you are motivated and serious you don’t want to cheat yourself!

      Hi Nicole, you can ask you GP for a HOMA-IR which is the insulin resistance test. Also, there is a doctor's directory in Australia at the Low Carb Down Under website along with plenty of presentations for further research on their CZcams channel.

      @@betheboss.online Just an FYI, Homa-IR isn't its own test, per se. It's a calculation based on other tests. If I remember right, you need to have fasting insulin and fasting glucose taken at the same time. Plug those into an equation (I don't know it without looking that up) to get your Homa-IR score. You might have already discovered this by now, but it's a topic that I've seen confuse people, so I thought I'd clarify a bit.

      I'm guessing that he thinks we should be spiking it twice a day once we're "cured" of our insulin resistance. I haven't heard this idea of "insulin suppression" mentioned anywhere else. Perhaps it's just another name for the "metabolic flexibility" some others speak of.

    Dr. Cywes, are you familiar with Dr. Pradip Jamnadas? He's a cardiologist in Orlando that has broken out of the matrix. He may not be quite as far along as you are, but he's pretty awake and I suspect you two overlap on 90% of your beliefs and practices.
    He has a CZcams channel with viral videos on Fasting and Sugar/Processed Food addiction. He may still use statins on occasion but he doesn't push them on anyone. He's all about fixing the diet.
    A family friend went to him after another cardiologist recommended a cardio ablation. Jamnadas told him to hold off on the surgery and try to fix his problem with diet first. Not sure how it all turned out, but Jamnadas had high confidence his problem could be solved with diet.
    You guys should team up!

      Also: In one of his recent CZcams interviews he said he plans to build a lifestyle clinic onto his cardiology practice, so he can devote more attention to diet and food addiction. I'm not certain of the time frame but I think it was late this year or early next year.
      I don't speak for him in any way, but I just feel that you could play a valuable role, even if it's just as an executive advisor.

    So the ultimate key is insulin resistance or sensitivity. If you are insulin resistant, then to reset your body to a healthy relationship with carbohydrates, you need to go ultra low carb. Give your body a chance to heal and your cells a chance to lower their defences erected because of years of being assaulted with way more sugar than they could ever use. If you are insulin sensitive, or once you become insulin sensitive again, then you can consume more carbs, because in a metabolically healthy state, your body can make use of a moderate carbohydrate input. Just don't overwhelm it all over again with metric tons of savoury snacks and sweets. And when you are insulin sensitive, then you get the full benefit of all the myriad `processes in the body in which inulin plays a part, because your cells aren't doing everything they can to block insulin out to avoid being overloaded with sugar.

      I watched a Paul Saladino video the other day where he talks about his re-introduction of carbs to his diet after years of eating carnivore. What Rob said in this video makes sense of what Paul discovered re his own body. The carnivore diet healed Paul's own insulin resistance, and with insulin sensitivity properly re-established, he can now healthily consume regular carbohydrates (in the form of fruit and honey) while maintaining peak metabolic health. Very glad I clicked on this one.

    I really admire your work Dr C. That said, women do need carbohydrates to make and maintain hormones, especially progesterone. Women should not cut all carbs during their luteal phase. Can lead to amenorrhea.

    Have reduced my insulin levels by half, however they are still high. Sugars are high too. Have constant inflammation markers, high cortisol. From this video, I'm aiming to go low carb, less than 20g a day.

    My carb management app just gave me a minus carbohydrate total after I added the amount of exercise and fibre I added into the mix. I had to reread my food list to find out my total carbs before the app deducted them to net.
    The avocado carb was highly reduced merely by its own fibre. Interesting.

    22:13 I have started lowering carbohydrates since mid November 2024 and stopped taking statins. Now after 3 months my total cholesterol is 297 and my LDL 211.I have no cardiovascular problems but my doctor insists on a diet low in fats and taking statins.

    I'm having difficulty wrapping my head around the fiber comment. I take raw psyllium husk a couple of 3 times a month. I've never seen my ketones shut down because of it. What a Great Colon cleanse...it goes like a golf ball & comes out like a water melon 😁. I guess I'm old school in that I look at fiber & subtract it from the total. I am following your recommendations & am very thankful to you for reaching us veggies for the most part are free 👍🏼👍🏼👍🏼 I'm just choosing to look at fiber as a plus when it comes to keto.

    Confusing. Count total carbs but don't count vegetable? Protein does stimulate insulin release, and in my experience, raises blood sugar, so why would anyone have to add carbs to trigger insulin?

      @@robertcywes2966 In my own experience of rising blood sugar and gaining 15 pounds on a high protein, almost carnivore, diet; and what I learned in nursing school, yes, protein does indeed cause an insulin release. I realize that I have quite a bit of metabolic damage, so I may not be typical, but I've had to reduce protein to even begin gaining control of blood sugar and weight. I'm not finding any studies that show otherwise.

      Agree on confusion… If you put a type one diabetic on a keto diet, they use less insulin but they still have to bolus for protein about 4 hours after protein meal

      @@keylanoslokj1806 No, that’s not what she’s saying. See Dr Richard Bernstein, also on YT… type 1 diabetic and doctor, who also advocates very low carb, higher protein diet. Type 1 makes no insulin, therefore always has to bolus insulin; but able to use much less insulin with a careful meal plan and exercise.

    I had a cerebellar stroke 4 months ago. The blood pressure was over 220/120. I am on blood pressure medication. Going to try Carnivore after a 2 year break. My addiction to sugar is absolutely insane. I get really sick with joint pain when I don't eat sugar for 3 days. Have electrolytes and everything in the book to start. In all seriousness. There should have been clinics for carbohydrate addicts. This is no joke. No Doctors in my Country do belive in Carb addiction. My friends started to Cry when i startet Ketovore for 4 years ago. They where afraid that i gonna die. I quit Ketovore 2 years ago. The i regain my weight. After to years of going back to the standard diett. Everybody say. «Aaahh you got a Stroke because of Keto. You damage your liver and will die young if you go back to Keto». I dont Care got to use my willpower to go back on Ketovore.

    Thank you for this. Although I’m not positive if I’m insulin sensitive again because my doc won’t test my insulin my blood markers are perfect now. It took me three years for my A1C to come down from 5.6 to 5.1. Weight is stable. Cravings are minimal.

    If it takes 2 years to become insulin sensitive! Wow, then how is it people get A1C lowered meaning lower blood sugar. I can be in ketosis and still have a BS of 129. I was carnivore 3 months. Blood sugars would not drop. High blood sugars above 100 cause the damage right? I was so frustrated. My fasting insulin was 16.1 after 3 months. No baseline before that. My a1c dropped from 7.0 to 6.1. 22:18 This information may have saved me some disappointment. Maybe too much protein based on this information. I am grateful for the information.

    1:50 let me say.. I can't even maintain weight above about 50g/day carbs and some varieties seem to lower that threshold. I really thrive on 20g or less.

    Theres a dr telling post menopausal women to do 1 high carb day a week! I'm struggling on ketovore (less that 20g carbs a day, 60-80g protein and 100-150g fat).

    Does anyone know where you can get an Insulin Resistant test done privately in the U.K.? Our GP (Doctor) will not agree to doing this test, as probably he wants to keep my husband on numerous tablets for his high blood pressure. We need more Functional Medicine doctors here in the U.K. instead of pill pushers, who are not interested in curing people, just masking symptoms with increasing medications, which bring them in money. It’s an awful approach to so called health care.
