Total Motion Release Technique for Babies: Upper Trunk and Lower Trunk

  • TMR or Total Motion Release is a technique that was created by Tom Dalonzo-Baker and modified for pediatrics by Susan Blum. The total motion release technique demonstrated in this video is for an upright positional release of the upper trunk and lower trunk. This TMR or Total Motion Release Technique is often utilized in children who struggle with crawling or have a medical diagnosis of Torticollis or Plagiocephaly.
    Please note: this technique should only be utilized in conjunction with a Physical Therapy plan of care or under the direct supervision of a physical therapist.

      KingKingKing000, Thanks so much for the comment -- you are ABSOLUTELY RIGHT. The pelvis has SO MUCH to do with GI motility (movement of food through the belly) as well as relaxing the pelvic muscles (we all have them, even babes!) that you may find this helpful in releasing gas, relaxing the nervous system, and relaxing the pelvic floor enough to decrease any tension there and/or helping food move through the system a little more efficiently! I know lots of parents are going to see this comment and give this technique a try simply because you took the time to share your win, so THANK YOU!! Lauren

    Hi! Awesome video, my son has a right side torticollis and I’m unsure which side rotation needs to be done. I’m in Australia and can’t find any therapist that has the TMR training to help us. Thanks in advance

      Hi Helene! There’s a few options: you can always book an online parent consultation and we can try to figure it out together with a quick screen in multiple directions. I am in the US but have a few parents in Australia that have found those extremely helpful.       The other option is to do both directions and test each to see which direction is easier. With R Torticollis typically a child would prefer L rotation of the neck, which can continue down into the upper trunk (but not always). I would try L arm under armpits first and then switch to R arm under armpits and see if one feels easier and then hold the easy one consistently for 2-5 min for the next week and see if it helps. With the TMR technique you ONLY do one side (the easy side) so that’s the big difference.

      @@liviamoraes1272 this is a different style of increasing mobility than traditional stretching. Traditional stretching increases length by pulling on muscle fibers to lengthen them and this is basically the opposite- increasing mobility by shortening the fibers, allowing them to neurologically relax, thus increasing range. Its great for babies because they cannot relax through the stretch feeling and often tense up making stretching less effective.
      Technically one could do this position using traditional stretching principles but I have found it to be much less effective than using the strain counterstrain or TMR framework.

    It seems counterintuitive to me.. like if they can stretch well on one side, I want to stretch their body even further on the side they’re already good at stretching??? Seems like I should be stretching the tight side instead🤔 if it’s being done gently I don’t get how it could cause micro damage🤔

      Hi Fabius! It IS counterintuitive, it is another route to improve mobility but I totally get why it is confusing. Its an osteopathic method that is the opposite of traditional stretching. I have a few other videos using this method that are on the channel if you go to the playlist and find tmr or total motion release, this will help fill in the gaps with more explanation of this style of mobility, since its new to you.
      I would recommend trying it out, if you do the opposite you’re not doing anything wrong, its just traditional stretching vs TMR release. I find that the TMR style has quicker results than traditional stretching but I dont villianize traditional stretching because I use it in other styles like the front of the neck. But I like TMR a ton for the trunk and use it almost exclusively.
      The microdamage can come from stretching the muscle fibers against tension or resistance, its again not bad but just a different style of gaining motion that can be less effective in infants because they do not know their body is safe when you tension it like adults do, this can decrease effectiveness. Again, either way is fine, theyre just using different frameworks to get to the same end goal. Choose which style works best for you, if youre not getting the results you want you can try the alternate option and see if that is better. Good luck.
