He is not advertising his work he is educating us. Not all viewing lives in USA or could afford this surgery with out insurance which our insurance would not cover as it is out of country.
Years ago, spinal surgery was the last option before you committed to a wheel chair. In reality, the surgery only prolonged the inevitable, and a wheel chair, indeed, was your new device to get from point A to point B. I have been diagnosed with DDD, have had a hip replacement, and suffer from Sacroiliitis. As my disease has progressed, I have often thought of spinal surgery. A year ago I was told, "you are not at that point yet". I was dumb founded that my pain levels were not deemed severe enough for modern spinal surgery. Today, a year later, I spend my days consumed with pain management. I have lived with chronic pain since my mid 50s. I am 75 years old and believe I will never have a pain free day in my life again. I share these thoughts, because I am certain many of us with similar issues feel the same way. I enjoyed this video and thank you for explaining your techniques and the future of spinal surgery.

Hi, I've just done a major rewrite of this comment. (As of Thursday, Sept 21, 2023.)
I’m 73 with much the same spine history you have. Four years ago I had fusion surgery to free a trapped sciatic nerve root. It was a hugely traumatic operation, mainly because of the muscle tissue they had to cut through to get to my spine. I was laid up for six months following the surgery, and another six months when finally, I got back to normal. One part of me that will never be returned to normal is the supportive muscles that were severed to access my spine. I am living with the consequences.
After my successful double fusion fully healed, which took a year, my new normal was a very stiff lower back initially due to the metal rods and screws holding my vertebrae together while they fused, and then caused by the rigidity of the fused bones themselves. The fusion cured my pinched nerve, and my excellent surgeon cleaned up everything he could in the region, hence the double fusion of L4/L5 and L5/S1. But to no avail beyond solving the immediate crisis of that pinched sciatic nerve root.
Now the more subtle, and in many ways more insidious effects of Degenerative Disc Disease continue to progress as my spinal arthritis spreads, extending to a larger area of my spinel. This time from the inside, the bundle of spine inside my spinal column continues to be corrupted by arthritic bone spurs that clog my spinal canal. The result is the other kind of stenosis, the one that consists of pain in the spine itself, rather than referred to the hip and leg as happens with a pinched sciatic nerve. The terrible nerve pain has been replaced by a dull ache that turns into unbearable bone pain if I walk more than a few minutes.
My bone pain that gets worse by the month. Knowing now what I did not know then was that after my fusion was fully healed, I should immediately have begun pro-actively mitigating the unfortunate consequences of the underlying disease, arthritis of the spine. Had I engaged in a rigorous exercise program focused on the core support muscles, healing as much as possible the ones traumatized by surgery, along with losing 35 lb. of excess weight (another thing I'll blame on the pandemic), this drastic action might have prevented the second radiculopathy I experienced three years after my first -- this time affecting the vertebra just above the fused ones, exactly as the doctor in this video described. In one of the most dramatic experiences of my life, when I truly could not bear it any longer, and the docs told me my spine was too far gone to withstand another fusion, that second pinched nerve root somehow freed itself. Over three weeks the pain diminished by the day, not because of any action on my part or medication, but just my dumb Irish luck.
But due to the insidious nature of osteoarthritis of the spine, my luck was bound not to hold. Instead of finally being free, the manifestation of my arthritis changed from nerve pain to bone pain.
When people tell you it’s too early for spine surgery, they’re talking for bone pain. The trauma of fusion is not worth it, they say, and you should manage bone pain in different ways. I fully agree., Nerve pain can be so extreme it becomes intractable, and basically you are disabled and suffering seemingly beyond what makes life worth living. Sometimes the nerve frees itself and the pain goes away. But if you do not have the great good fortune I experienced, an urgent surgery may be required.
This video shows us an alternative approach to spine surgery. If this doctor does back surgery as well as he does communicating the condition to non-medical viewers, I'm going to track him down and if my insurance disagrees, camp out on this office doorstep. (Just kidding, but his half-hour video finally clarified the dual personalities of spinal arthritis, and suggested a new way of dealing with it while retaining the spine's intricate stabilization architecture. It's all news to me and I'm going to research ultrasonic microsurgery in depth, along with consulting my pain management team about its efficacy in cases like mine.
The reason for urgency is that after having survived two episodes of nerve pain, I am now afflicted with bone pain due to arthritic bone formations INSIDE my spinal canal. The pain is completely different from the agony of nerve pain, but it has made me unable to walk more than ten minutes. My battle is a long way from being over, but I am determined to fight it as long as I have the physical resources to do so.
Though I'm now a certified senior at 73, one of the few good habits from my youth has persisted: riding my racing bicycle on a tough 15-mile circuit here in the East Bay hills, ever other day. The internal strength from doing that since age 19 could be what might see me through. But then, truth be told, I slacked off a lot during the pandemic, and this could well have been what made me vulnerable to stenosis that began clogging the inside my lumbar spinal column.
I've accepted that ultimately, my health is my responsibility, something I can't delegate or wish away. At my advanced age I'm free from cancer and heart disease, a huge privilege I will never take for granted, and despite my stenosis I am still able to ride my bike 15 miles every other day. As it turns out, riding a race bike tucked over in the racing position (as opposed to sitting up vertically with all your weight on your butt), provided me the side benefit of my cycling exertion indirectly strengthening my core muscles, while at the same time keeping my body weight off my lumbar spine, so while riding I have zero spine pain, nor does pain set in afterwards..
As we've seen from this doctor's presentation, and as I discovered serendipitously by my unbearable nerve pain resolving on its own over a couple of weeks, we should not give in to stop-gap solutions like epidurals and pain meds unless the pain is intractable. (This morning I had an epidural, my first in years, and over a few hours discovered how massively my quality of life has been degraded, just by experiencing a few hours without pain.)
Through the coming winter I will double down and engage in a twice-daily floor exercise regimen to optimize whatever my core muscles are capable of to stabilize my spine. For a guide I'm using "The Back Mechanic" by Dr. Stuart McGill, whose focused techniques have saved the spines of many world-class athletes. I'll be riding my bike through the mild Northern California winter. And I've already lost 25 of the 35 excess pounds of belly fat (which I will also blame on the pandemic, though I know better) that could not be in a worse place, due to the added strain they put on my spine, especially when standing. My goal is by next Memorial Day to be significantly more mobile on foot than I am now, and at a lower pain level, without meds. I know the pain will never be eliminated, but if it can be brought to a level I can live with, and at the same time reduce the likelihood of another pinched nerve, I will consider my plan to be a success, and a starting point for further improvements.
One thing I could not bear would be to lie in bed over the upcoming winter, watching movies on my iPad, zoned out on pain meds, and knowing that even a trip to the bathroom would be an excruciating ordeal. I am very lucky not to have to live out this scenario -- many people are stuck with it -- and for that reason, knowing an activity-based option is available to me -- I could never forgive myself for not being able to walk my dog next Memorial Day. Because ultimately, my health is MY responsibility.

I heard of spacers & everyday there is new tech. Dr cut my pain meds so now Ive fallen & broke my nose then my shoulder & sternum & head. Dont know how I got to my cell phone so miracles happen & new techs. Dont give up I feel ur pain! Ortho Dr told me Id never be able to play tennis again by not being able to ever lift my arm & am clueless at everything else he said. Drs should never say NEVER! I kept having neuroma surgery on my foot but it was spinel stenosis. Just love how this Dr spks in layman terms. Keep the hope💜🙏💜
It started from jogging and it was pain down my left leg disabling me. Definitely having herniated disc had some to do with it and swimming although great was not helping. I kept cycling and 6 months after it was gone forever, my doctor said; keep peddling it healed you. Does it work for everyone ? All I can say is it worked for me. If you’re a cyclist who got hurt from jogging and of course can’t jog anymore because the pain got worst to the point it became lumbar stenosis hang on to cycling. Ride in safe places which are designed for cycling and give it time and don’t overdo it. My lumbar stenosis disappeared, a got better and currently do all 3 events ( swimming, cycling and jogging ) none competitively. Quality cardio an strengthening workouts ranging from 30 mins to 1 hour. I hope this helps someone out there.

I have been told I need a laminectomy but that because I am on suboxone for pain I have a very low tolerance to pain and this doc doesn’t want to take me off of that and put me on another pain med. Also, that only my legs will be better not my back. I will always have pain in my lower back. I had to leave my job and my quality of life at 60 yrs old is terrible. My 83 year old mother is more active than I am.

I Had a laminectomy L4/L5 L5/S1 in 1989 when I was 37. I was disappointed that it didn't make me 20 again and it hasn't been without some pain but I'm glad I had it done and I stayed active and now 71 and I still work and thank God for the great Doctors I had.

I had 2 failed surgery of Laminectomy
I just had the surgery for the samething. My pain started in Oct 2023 December it got so bad I had the surgery in Feb 2024. I'm 48yrs old and I feel so much better. Still a little numb in that area, at times. But u know it takes time

This is hands down the best CZcams video explaining what spinal stenosis is and the causes. I wish I had seen this video prior to my laminectomy in 2015. It helped but it creates issues that points towards inevitable fusion because you break the cable and turn back muscles into useless fat and scar tissue.

@matth2340 Do you have stenosis in both lumbar and cervical spine? Also, why the muscle degeneration? Unable to workout anymore or surgery created mobility issues. I have stenosis above c4 & c5 where a surgeon put in two discs with screws 9 months ago. Terrible pain on right occipital and deltoid area. Wondering why he didn’t do anything about the stenosis:( Appreciate any advice.

Just one thing missing from the video:How to stop the arthritis. It's really simple and anyone can do it because I did and have no back issues at the age of 73. Stop eating carbs/sugar/seed oils and only eat meat,water and salt. Do that for 90 days which will usually result in the pain on inflammation to go away. Then if you want to introduce some vegetables or fruit in small amounts and watch for bad effects by all means but keep a log of everything you eat. My lumbar pain went away with the 40 lbs of fat. My bmi went from 27/29 to 15/16. Not carrying around all that extra weight sure helped more than my back.

I am 81 with spinal stenosis and recently had a procedure called Vertiflex which is helping immensely. I can stand up straight comfortably. It may not be for everyone, but it is helping me.

I had severe spinal stenosis back in 2018. First two Drs wanted to do a four disc fusion because I had such terrible arthritic growth on both the inside of the canal and had foramina osteophytes on the exits pinching some of my nerves almost completely flat. I had this in four of the five sections of my lumbar and it caused extreme pain. Thankfully I was directed to a neurosurgeon who must have used this ultrasonic tool and it fixed my problem and I had no fusions whatsoever. I’m so thankful I was able to find this Dr. My life would have been ruined had I listened to the first two physicians.

Neurosurgeon in Evansville, Indiana This is were iIm from. Its a city of around 180,000 people. This Dr has an excellent reputation. He told me up front before he looked at my MRI that he would only work on me if he believed he could help me. I appreciated that right away.

I wish I saw this earlier, because my first surgery was a debucle, the second one Last month was successful, exactly like You describe. However the BACK pain continues many days after the surgery. It gets alliviated by Symbalta and lots of Tylenols !

Thank goodness I've seen this video. I am in the process of getting my back problems at L3,L4,L5 rectified surgically and the method described to me is the older method. I will insist that I do not receive this method but the newer ultrasonic treatment if at all possible in the UK, and if not available on the NHS system then I will have to consider paying for this treatment myself.
Additionally I can also see benefits to the new method in that it is less intrusive so recovery may be quicker. I can't thank you enough for putting this video on CZcams, making me aware of this treatment, I hope others see it particularly surgeons this side of the pond.

Thank you so much for this video, it really gave me a great deal of information I wasn't aware of. I had a Laminectomy and fusion on L4/5, in 2016, I've had the same pain every day since then, until today, that I had after the operation. I couldn't walk for the first hour after getting out of bed, every day, for one year. After 5 separate pain injections, the worst of the pain went away. Since 2017, I'm hunched over forward, when I walk, and I have slowly, over the last 7 years, lost the the ability to walk more than 60 feet. I have (had tested), nerve damage at L5 disc, and I have become a couch potatoe. I'm unable to take any pain medication, due to having had a gastric bypass 28 years ago. Let this be awarning to anyone considering having a Laminectomy done, to weigh, if your present condition might get worse.

Dr Robbins had a heart attack on stair master or treadmill next day. He was also a friend yet he didnt communicate as well as u do. I've seen 2 Great Danes get spacers & went back to normal. Knowledge is power so thx now I know more. Ive witnessed so many bad surgeries so u r such a blessing. Thank u from the bottom of my heart💜🙏💜 watched ur video 5×.

I can no longer walk my dog, ride or care for my horse, and am struggling to keep on top of my housework or garden. Like Brian Goulding I’m in the UK and have never really been informed as to what my problem really is. This was so understandable and visual . Would love to be able to do this procedure as I love travelling and my back problem curtails most of my activities. I do grit my teeth and do things in pain but oh! how enjoyable it would be without that pain. Thank you Dr Cantor for the explanation. Who knows? Maybe one day!
Is this available in UK as a private patient as my pain so severe it is life changing and depressing but I have severe osteoporosis and experienced 8 spinal #s lumbar and thoracic over last 3 years. I am now on Evenity injections for osteoporosis that will be for one year only. I am 67 and was a very active person who feels totally debilitated and Neuro Consultant says spinal surgery to much of a risk. Dr. Cantor, thank you for a most informative session. I have been struggling with stenosis since April and after a couple of injections on each side of the L4, L5 I am once again mobile. The pain was so severe and debilitating that I was ready for the surgery or anything that would alleviate the discomfort and to get me off the walker. I'm glad the direct application of anti inflammatory medication worked because it has given me the time and opportunity to find your video. I will definitely stay away from the Dremel type of intervention and will hold out for ultrasound if and whenever surgery becomes necessary. I am glad I found your information.

This was an excellent presentation, if only I had received information like this before. My back was fused in 2018 and I have rods and screws. I walk but after short distances I have to sit and rest. It takes a long time to get anywhere, sometimes I can walk further but often my legs feel like lead and back pain severe. After seeing this informative video I wonder if anything can be done now. We went over my MRI just like on the video and yes I was a canadate for a lumber decompression. I agreed to go ahead and boom I had an appointment. His office staff is very accommodating and goes to great lengths to keep you in the process. Emails and phone calls are answered. Had surgery May 2nd 2024. Everything is great so far. I can highly recommend Dr. Cantor and his team. Also the care at Holy Cross in Ft. Lauderdale was A1.

What a great video! I have spinal stenosis and bad sciatica running down both legs. I live in pain😢 A surgeon wants to cut into my back and put in screws and plates...not happening! I love sport and keep myself slim and active but rely on oodles of painkillers. I wish I could find a Dr Cantor in the UK. ❤

Wish I had known about ultrasonic technology and Dr. Cantor's techniques before I had a traditional open laminectomy (L4-L5) in Sept 2022. I'm over 8 months post-op and still struggling with much of the same pre-op pain. My post-op MRI and X-rays revealed a seroma (fluid buildup) below the laminectomy and some mild spondylolisthesis I didn't have pre-op. In March I had epidural block and fluid drawn out of seroma in March by a pain management MD. Scheduled for another epidural in 2 weeks. Recently started doing more PT. Back trouble sucks!
I had my lamenectomy in October 22. I got what the surgeon promised, relief, and for that I'm very thankful. After this video I can see my relief may be limited. Time to slow down I think. Good luck.

I've been a Certified Medical Assistant for 32 years in family practice but still have a difficult time understanding my condition. I'm 52 and have foraminal lumbar stenosis and spondylolisthesis. Four different neurosurgeons have recommended fusion L3-5. I'm so depressed for having to stop working. My last five jobs have fired me for not being able to work good enough. I've applied for disability. It's not good. I do not want surgery. I've never met a patient who has been happy with their spinal surgery 😢

As a retired physician i found your explanation really clear, congrats on your ability to convey things. The downsides of traditional surgical procedures seem pretty much worked out but what about problems and failure rates with ultrasonic techniques. My surgeon was very keen not to operate c5 level and he was right as after 6 months of, at times, severe discomfort things settled. How is this explained by your model of "bone junk"
I'd like to know same questions. Are there no other Docs in USA trained to perform this? Retired RN, lumbar surgeries wo hardware. 76 and I have no pain meds can't hardly walk, off friggin pain scale, etc. Drs usually are thrilled to teach others their pioneer technique. Sorry for rant..

@@jerryrobinson7856 my problem is money. Lost my business end of 2008. I ended up on Medicare, filing for disability.. I did it a few years before my time and my monthly check was a lot less. Both feet and lower legs are asleep, numb. I'm not in shape now to go through an anterior and posterior fusion from L3 to Sacrum. I would consider this ultrasound surgery if it clean enough stuff after 4 surgeries that left a lot of scar tissue, stenosis, left over disc material, new disc bulging, etc. I know Blue Cross ain't going to pay for me to go from Alabama to Florida. I've been thinking about sending my most recent MRI and report to Dr Cantor. I'm taking a wild quess this surgery is around 20k or more for Dr alone. Then all the other charges associated with hospital, anesthesia, etc.
What types and how many surgeries have you had? Have you had this type of surgery somewhere? Many of these videos are 8 or so years old. A couple of newer. I have not been able to find any info on this other than Dr Cantor. My friggin brain can't take this BS much longer and been going to dark side. With both legs, hip replacement, neck, hands and fingers etc make me think how many years do I have left. They have gone by so damn fast and I'll be 77 next year! I don't want to die any time soon. Last pain Dr let me go after 2 years because I was still taking a legal Rx, Klonopin, which I was put on around 87 or 89. He and last 2 never said too much and my piss never showed anything to dismiss me. I'm a long time user of cannabis. As far as I know they never checked for it. I get sent to another pain clinic not too long ago. I knew they would do a drug screen and THC was found, duh!! Got call telling me my lab work was abnormal and I had to go back and see the psychologist. Short version is I was told my urine had THC, I said so what. It's illegal, well test but don't check. I really was wound
up and started being an ass and cussed his ass out. It's 2023 and Alabama have not figured out how to get their medical marijuana dispensaries up and running. But, we will not be able to get bud or vapes! 1 route to get it is via suppository. WTF!!! 😥 Let me hear some of your history if you don't mind. If not here, maybe email. ☮️

I’m in this exact scenario right now. All the symptoms you described I’ve lived by for years but this week it really showed its colors. This video is great at explaining what I think I’m up against. I’m just worried about my previous surgery in 1997 when I had a discectomy on 4-5. I’m meeting the surgeon in 2 weeks so I have time to get the MRI. I’m trying to put on a very positive attitude but I’d be lying if I said I’m not concerned.

If I could speak with you before sending my MRI's etc ? From being schedule for a back fusion and had to wait due to COVID-19 it was on hold for two years. Wow!! I don't know if I have spinal stenosis, but I understand it. This is one of the most amazing presentations on human anatomy that I have ever seen. What a teacher! I did not understand what causes arthritis either. But I have an understanding of that as well. I will be watching more of your videos, Dr. Cantor. I will be starting with physical therapy tomorrow, hoping that takes care of the pain I am struggling with.

Excellent presentation. What about the asymmetry of the incision into the lamina? Are there any long term studies on how that might cause problems in the long term just like the full laminectomies caused degenerative problems eventually. IT would be a perfect procedure if this were not there. This concerns me. Thoughts.

I am 66 years old and have been suffering with lumbar pain for many years. I finally decided to do something about it. I went to the doctor and she sent me for 15 sessions of physical therapy before any imagining had been done. I came out of the physical therapy feeling worse. When I sit or lie down the pain goes away but standing for a short amount of time is very painful. What is even worse is I have been diagnosed with early stage Parkinson's Disease and atrial fibrillation. I'm not ready to give up yet. I go to Parkinson's exercise classes which are exercises mostly seated so I don't get the pain there at least. I have been searching in Austin, TX, where I live and haven't had much luck. I finally got my PCP to give me a referral to a spine surgeon which she did. He asked me some questions, checked my reflexes, and had me walk up and down a hallway. He also wanted to send me for physical therapy until I told him the outcome of my last 15 sessions of PT. Then he suggested I get steroid injections which I'm hesitant to do. I would love to find someone in Austin, TX or the neighboring areas. I do not want to have the old procedures due to the spinal damage that can occur over time with fused joints.
Short of finding an local ultrasound provider I may find myself coming to see you, Dr. Cantor to see what you can do for me.

Same here! I endured my 1st cervical ablation 3 months ago and I'm worse off now. My p.m. Dr already scheduled a 2nd one in September 😢 then wants to do my lumbar.. I'm terrified if I tell him no that he'll dismiss me as a patient. I have RSD/CRPS full body and my spine feels like it's crumbling daily😢

This appears to be an amazing break-through procedure. Dr., I have a question. How do you access the area of the vertebra to remove the "scar tissue/junk material" when the vertebra are so closely "stacked" on each other in the spine? Is the entire spinal cord "surround" accessible from the back of the vertebra? Thank you! I'm thinking this may be a life altering procedure for me.

So, i had a lower lamenectomy to treat severe stenosis a year ago. No rods and no fusion and also no mention of possible troubles down the road. I got the relief i so desired but now i see i need to start being easier on my back. I'm 73, still working as a market gardener and appreciate the warning. Wish this video had popped up among the many i watched pre-op. My surgeon was great but using older technology. Wish i had known of this option as it seems far and away a better option. Id even expressed concern about the loss of bone but didnt hear any thing about loss of stability. Thanks

This would not stop popping up on my feed, and I’ve had spine problems since I was about 14. My lumbar feels SO compressed, I can’t stand up more than 10 minutes. Now my spine has entirely fused together with bone spurs, and I have Lyme, which may be the reason it fused with arthritis. I also have hyperlordosis, which is not postural. When I’m not in pain from mold toxicity, autoimmune disorders and Lyme, my back is on fire. I don’t want surgery if I can help it, but this is horrible!!
Oh! I can’t sit either! I have to lie down or be in a recliner. WTH? No injections ever helped.

Superb! Last year I had L3, 4 and 5 fused. I have stenosis and arthritis as well. My latest x ray shows everything is nice and tight into the bone, which is a relief, but I am about to have some cortisone epidurals due to some sudden ongoing pain which may be a result of some Pilates moves..... it especially hurts getting up from a sitting position... (getting old certainly sucks!)🙄

I have lumbar stenosis In C3 C4 C5
And also cervical stenosis at the base of my brain stem and spinal coran meet. Inoperable at my age I'm 80. So I am going paralized from neckdown Can barely use arms. Shufflefeet. Headaches. Balance off and unbearable pain from neck to feet. Hands drop everything and hurt badly. I can't pick up 3 lbs.

Wow! Amazing. Ultrasonic should be the only allowed procedure. I'm 45 and am being told I'll need cervical and lumbar fusions. I'm so scared as I've heard more horrible stories than positive. 😢
DONT FUSE! NEVER FUSE! Thatsnold medicine like blood letting. You'll lose ROM. And screw and pedicles will have no anchor once osteoarthritis sets in and loosen.
I have 8 different issues. Just left a small town hospital Orthopedics doc clinic. They can't help me. He told me my facet arthritis can't be that bad because I'm only 44 and it doesn't set it in till about 60.
I plan to write a book on how I've been treated since the pain began. I WAS healthy when it began. I've been treated very bad because of pain. My ex BF saw the bulging disc on the MRI in 2014 and said. "I thought you were faking it." I asked him why. "I don't know, attention."
So I asked him you think I'd rather have attention than money? What?! I have lost so many jobs, respect and credibility because working makes it worse. SSDI keeps denying me. Medications have ruined my endocrine system. Physical therapy hurts. Idk who to trust anymore.
But I shouldn't have been treated like a burden from SSDI since paying into it from 1995 to now. It's weird that you can't have access to funds that are there for up to 3 years.

I am on medicare and medicaid and am also hesitant about surgery..same reasons. But at least now I know what I want and that is a big deal. The Ultrasonic surgery at least weeds out the surgeons I don't want. This was very helpful.
Disc replacement in Germany using lp-esp disc should be your first option, 2 levels here no regrets!!

Thank you for this very informative video Dr. Cantor. I am dealing with severe spinal stenosis. I had an MRI in 2012 and the surgeon wanted to fuse L3,L4 and L5. I was terrified to have this done so I gritted my teeth for 10 years. It was getting much worse and I had seen videos of new techniques that were much less invasive so in 2022 went to that same surgeon and had another MRI. Again he wanted to fuse L3, L4 and L5. I saw a video about Radio Frequency Ablation and tried that to no avail. The doctor at that pain clinic referred me to an Orthopedic surgeon who based on the MRI from 2022 wants to fuse T12, L1, L2, L3 and L4. He wants me to get another MRI which I am doing. I don't want to be in pain for the rest of my life but I don't want to make a bad situation worse. These two surgeons make me even more afraid because they want to fuse different vertebrae so I feel like one or both of them will put me through major surgery and not fix the problem. Dr. Cantor do you know of any surgeons in the Columbus, Ohio area that do the procedure you are doing?
