- Here are a couple of vlogs in a series about how I learned and Peritoneal Dialysis (PD) Manual Method At Home (CAPD). This one is about connecting. CAPD is often good when you have a day where you are not able to do your full cycler treatment for some reason or if you travel like me, some shorter trips are just much easier to travel without having to bring your cycler (CCPD) with you. I’m on my first day alone on manuels. There is not a 24/7 PD nurse on call on weekends to help and I get stuck/confused when I complete dwelling step and it’s time to drain. So, after dwelling I have been draining into a separate “Y set up” drain bag each time (3 times a day) not the Ultrabag . I know the “Y” bag should only be used for last manuel drain each day.
If you are using the same drain connection for the Y setup all day long, then you risk possible contamination and peritonitis if the connection on the Y is not properly sterilized before connecting each time to your transfer set. This is why they would have you use that for the last drain by some clinics. I would also recommend you check out the link to the FB support group for Peritoneal Dialysis Support Group where we have moderators and other PD patients and care partners to help with these things 24/7, although your clinic is suppose still have someone on call 24/7 on the weekends as well.

You have been a great help I have just started dialysis on PD a few days ago and I have not yet received my machine I am doing everything manual which is 4 times A-day 30 minutes at a time I mostly using the green and yellow bags but I'm feeling full all day and wonder when I stop the machine on the night time will I still feel that fullness

WHAT IS DWELL TIME? IS THAT HOW LONG THE FLIUD IS IN YOU? YOU DON'T ALWAYS HAVE FLUID IN YOU? MY WIFE WILL BE DOING THIS AFTER THE CATH IS PLACE. WE HAVE NOT BEEN TRAIN UNTIL AFTER THE CATH. NOW I AM CONFUSED. I WAS THINKING THE FLUID IS IN AND OUT 4 TIMES A DAY CONTINUES. I GETTING CONFUSED WITH MANUELS AND MACHINE AT NIGHT. THIS VIDEO IS ON MANUELS.
@@ellenlee1 correct, the dwell time is the amount of time that the solution is in your peritoneal cavity after the fill phase of an exchange. Also, correct that there may be periods in the day where some patients have no PD solution dwelling -- most common are day time, if they are using the cycler at night. It will vary from patient to patient based on their individual treatment prescription. Great questions!
@@KidneyJourneys YES I UNDERSTAND THAT, IF YOU ARE ON THE MACHINE IN THE MORNING YOU DISCONNECT THEN OF COURSE YOU HAVE NO FLUID IN YOU. IF YOU NEED ADDITIONAL DWELL IN THE AFTERNOON AND SAY YOU DRAIN AT NIGHT INTO A NEW DRAIN BAG, WHAT DO YOU DO WITH NEW FLUID THAT CAME WITH THE DRAIN BAG AND FLUID COMBINATION. YOU WOULD NOT USE THAT FLUID BECAUSE YOU WILL NEED THE BIG CYCLER BAGS OF FLUID. SINCE I AM ONLY DOING MANUALS. IF THERE IS A TIME I DON'T HAVE FLUID IN ME. THEN THERE IS A TIME WHEN I DRAIN OUT WITHOUT PUTTING NEW FLUID IN ME. THE SAME WOULD HAPPEN THE DRAIN BAG WOULD BE USED AND THE NEW FLUID WOULD NOT. SO ARE THERE DRAIN BAGS ONLY? THAT'S WHY I AM NEW , NOT TRAIN YET, THESE ARE MY QUESTIONS. I ALWAYS THOUGHT IT WAS ALWAYS IN WITH FLUID AT THE START THEN FLUID WILL ALWAYS BE IN ME. I WOULD DO 4 EXCHANGES IN 24 HRS . EACH WITH A DWELL TIME OF 5-6 HOURS GIVING TIME TO DO THE EXCHANGE. WHAT IS THIS 1 HOUR DWELL TIME IN DOING MANUALS? THEN IT WOULD BE SO EASY.

In the meantime, since you are already on PD and doing manuals, I would encourage you to join one of the recommended support groups and we can chat further on there about how the peritoneal membrane works and the Peritoneal Equilibrium Test (PET) that is typically done either during training or within the first few months of starting PD to tailor your treatment prescription. You can also hit "subscribe" and the Notification bell on this channel to get the notification when the new videos are uploaded.
Not everyone will have 5-6 hours dwells per exchange. For a lot of folks in our support groups, their dwell times range b/t 2-4 hours, mine was no more than 90 minutes per exchange. and I will discuss the reasons in an upcoming video. So yes, there may be times for you or others that there are hours in your day where you do not have solution dwelling in addition to what you mentioned for manual (CAPD). It will depend on your individual treatment prescription.
where do you get those blue pads? what are they called?
Thank you for the question. The are called blue underpads. I get mine from my home dialysis clinics during montly visits with other ancillary supplies. Some clinics allows patients to order though their approved supplier with other supplies and have them shipped. But they are also available for purchase on Amazon or other Medical Suppliers online or stores. They come in different sizes.
I don't see anything more current than 2 years ago, almost 3 years ago now. Anything more up to date? Or did you get a transplant ? Would love to hear about that if you did.
Thank you for watching and great question. Yes, I did receive a deceased donor transplant back on Dec. 2019, so have been recovery, getting through Pandemic and focusing on other aspects of life as well with more travels with work, etc. I still speak nationally for various non-profits and continued to run different support groups. May do a live stream one day to try and asnwer some questions live on the channel until I figure out how to edit again. :)

What about flushing the lines to prevent air getting into you?
You just want to make sure when you prime the solution in the lines that there are no air bubbles before connecting to your transfer set to fill. For manual CAPD there should not be, since the solution fills to the drain bag in the closed system so once you see some solution from the fill bag in the drain bag, it' usually take care of. They will go over the priming process during your training for PD.
I am recently on hemodialysin dialysis and want to change to PD then proceed travel to Asia. Do you have any advice or any experience on getting supplies while living in example Thailand for a month Vietnam for a month or the philippines? How long prior to travel with that PD insertion surgery need time?

typically it takes 2-4 weeks for the exit site to heal properly, but some folks may need more time and also training time can be up to 12-15 sessions, depending on how your home dialysis clinic trains, their staffing and scheduling availability. While PD makes travel still capable, there are different regulations in different countries,so you will need to talk to the supplier to make sure they can get you the items you need based on your own treatment prescription in the various countries.. You should also talk to your home dialysis clinic and nephrologist about your travel plans as they still typically will need to assess you and run labs monthly while you are on PD, refill your prescriptions, etc. So planning on Asia travel sequentially likely will involve some planning, coordination and lead time.

What if the solution doesnt flow into the patient tube that is attached to the stomach?
Great question Nana. Often this is due to something that may be blocking the PD Catheter like fibrin or omentum. Typically the first step is to contact your PD clinic/ PD nurse and they will likely have you go into the clinic and attempt a flush or pull of any fibrin via a syringe (not something to do on your own at home). If that does not work, they the PD Nurse will work with your nephrologist to order a scan and possibly re-positioning day surgical procedure for them to try other ways of clearing the blockage or possibly replace the PD catheter with a different one (another day surgical procedure).
any recommendations for warming solution for capd?
If you use a styrofoam or other cooler, wrap the heating blanket around one of the solution bags and put a bag below and one above and let them set during the day or over night. They do also sell portable warmers for CAPD solution bags, but they cost appx $350 USD the last time I looked. Heat multiple bags before you need to use them. I was fine not heating them when I do CAPD (Manuals) and it did not bother me at room temperature.
Your heating pad on med or high?
@@kolotiletongamoa9654 I personally did not need my solution bags to be heated. It's kept indoors so they were at room temperature. If you need it heated faster use high - test out how much time for your own heating pad. If left heating all day, even the low / medium should work. Put a new bag in when you take one out to use and rotate them first in first use.
one hour dwells? How long is your whole cycle when you're on the machine? my dwells are about 2 hours I think.
Thanks for posting this very good question. My total treatment time on PD was around 8 1/2 hours per night over 6 exchanges. Each person will have different dwell times per exchange based on the results of their Peritoneal Equilibrium Test (PET). Some folks may have more effective treatments with longer dwells and some would do better with more exchanges on shorter dwells.
my total treatment was 8 1/2 hours each day. the last year was about 9.5 hours as the Amia Cycler and my needs worked a bit different. I slept through most of it or was watching videos moderating support groups on FB. Links in the description.
@@KidneyJourneys Next Friday I go from Manuel to Amia. What is your take on the Amia? Why was your treatment time longer on Amia?
@@howardmraz3336 The total time to setup and take down on the Amia is a bit longer due to the computerized step through screens and the system shut off programming. on the home Choice, you can end the treatment and turn off the cycler must faster. The Amia completely drain off any solution not used and then goes through programmed shut off. But I usually just disconnect while it’s doing it’s shut down and make coffee, breakfast, shower, etc. Also, by the time I was on the Amia, my health required a bit more dialysis to meet clearances, thus my treatment Rx total treatment time was increased a bit.
@@howardmraz3336 Sorry for the delay in replying. Just seeing this now. the Home Choice Pro was easier to manage changes to programming on my own as my original PD nurse taught me how to make those changes. On the AMIA, it's easier to learn and use for the average person, but the setup and take down takes a bit longer than Home Choice. AMIA was also much easier to travel with as it's more compact and lighter of a cycler.

I'm not quite sure of your question, if you could clarify.... Do you mean the cost of doing PD; how much time? or something else?
Why is there to exit points in only one catheter
