    The Clarke's sign or Patellar Grind Test has long been used in the assessment of patellofemoral pain or chondormalacia patellae but it appears that the test has a low clinical value and should no longer be used in practice.

    I have had an MRI to confirm grade 4 CMP on the left and grade 2 CMP on the right. Never had this test performed throughout the diagnosis process but as a physio student I thought I'd give it a try. It was very painful on the right, then extremely painful on the left. Wasn't expecting the pain to be so intense

    May it‘s important to mention that the downward pressure on the Patella should not be that high.. otherwise the test is always positive..

      You want to fixate the base of the patella. What do you suggest to do differently?
      The literature suggests otherwise, namely that the test rather has a bad sensitivity (lot of false negatives compared to relatively low true positives)

      Physiotutors I meant to say that if you put too much pressure on anybody’s patella, they will always have pain when activating the quad. Otherwise everybody in my PT class would have Chondromalazia patellae :D

    I just did this test in myself and it hurts a lot. I haven’t been able to play soccer for 2 months. I can’t run, squatting hurts, and I’m just depressed right now.

    Kunt u mij alstublieft helpen ik ben bijna 16 en heb al ongeveer een half jaar dit in het begin viel het best mee hoeveel pijn ik had maar de laatste tijd heb ik echt het gevoel dat ik dood ga echt serieus ik krijg meestal s’avonds heel veel pijn dat ik bijna niet kan opstaan en zitten soms huil ik heel de nacht door en slaap ik terwijl ik huil ik was naar de huisarts geweest hij zegt het gaat over of je moet sporten maar sporten doet nog meer pijn en de pijn ligt in / random mijn knie schijf en als ik daar zoveel pijn heb gaat het zelfs verder naar onder mijn voeten daar heb ik dan ook veel mijn alsof iemands e bijt en ook heel raar s’avonds krijg ik ook heel veel pijn aan mijn vingers (knokkels/duim) en heupen en rug sorry voor mijn lange verhaal maar ik heb echt hulp nodig ik ben echt bang en ik ga volgende week weer een afspraak maken want door die vorige heb ik nu nog veel meer pijn! 🥺😭😭😭🙏

      Hey Bilal, i understand that noisy knees can cause you to be worried. However usually knee cracking is nothing to be concerned about and actually pretty normal.

      That was the underlying theory but the correlation between alignment and complaints is not there

    What is the solution weightlifters? I went through over a month of physio for PFPS and the pain returns as I resume training, my physio referred me to an ortho because he thinks it's chondromalacia. Still not lifting the weights I could before the injury, I am hoping it does not require surgery!

      A month is NO time at all to work on PFPS. Rather give it 3-6 month, especially considering that you want to lift heavy weights again. One of the most crucial points is to adapt your load and to very slowly build up the intensity and volume of your training.
      Keep working on your hip and quadriceps and increase volume during weightlifting as tolerated without pain.
      "Chondromalacia and malalignment are findings that almost always do not need to be "corrected" to relieve pain." (Post et al. 2017)

      Thank you! I should have clarified. My injury happened in late March and I went to PT for one month, but continued to do the corrective stretches (improving femoral IR) for another month with very conservative loads. I can lift lighter weights without pain, but just a minor tickling sensation across the patellar sometimes. I am increasing the loads and volume again slowly after 2 complete weeks off.

      You're welcome. PFPS is not an injury, it's a symptom complex and often caused by a sudden increase in load on the PF joint. Think we discussed earlier that improving internal rotation does not make much sense to us as only very little internal rotation is necessary for the squat.
      With PFPS, you should focus on the hip abductors and external rotators, as well as the quadriceps.
      For the rest, keep increasing your load gradually and give your body (and cartilage) time to adapt.

      Thanks again. It was a sudden buckling of the knee under a heavy but not max squat after multiple sets (160kg). At the eccentric portion above parallel I felt a sudden rubbing movement. I could train the next day fine but after enough volume my knee would be so bad I couldn't train at all after 15 minutes (could not air-squat without pain). After that, I had all the usual PFPS markers: theater signs, increased crepitus, pain going up/downstairs etc. but they are gone now and only come back for a few hours after light training.
      My PT told me he thought the asymmetry in IR in my left leg (less than 10 deg on the left) was responsible for a compensation pattern that maybe hurt the underlying cartilage, by excessively pushing out the knees. I turn out negative on this patellar grind test but he still thinks I should see an ortho if it does not improve. I will follow your advice and continue a gradual increase in loading and strengthening hip rotators! McConnell taping helps a little bit. Should I place a priority on front squats to emphasize the VMO? I thought the strength-asymmetry between lateral/medial heads theory was now discredited.

      No worries. It's hard to give good advice without seeing you in person. We'd suggest to even lower the weight until you don't have pain after training at all anymore and slowly increase from there. Your cartilage needs time to recover.
