      Iv got basically the same story I just turned 36 and Iv also been dealing with pain about probably 10 years but id say it was about 6 or 7 years ago when I really started to question what the hells going on…. Anyways about 4 years ago I finally got to a foot doctor and he basically introduced himself took a look and feel of my feet, a x ray and then started talking surgery and chipping away cartilage and possible fusion If that didn’t work. I thought he was nuts but I hadn’t done any research myself yet unsure of what it was. Anyways I left there thinking well the pains not unbearable so as long as it doesn’t get worse. Well it’s worse and I see you wrote that comment 3 years ago so do you have a success story you can share?

    • @markaspland8788
      @markaspland8788 Před 4 měsíci

      No success story here. I don't know if i'm used to the pain or what, but I don't seem to be in much pain now, but I am less active. If i play sports and i really put pressure on the join it hurts, almost feels like it is ripping off. If anyone catches this early, you can tug on your toes quite a bit during the day which helps. I was still told to do this every day to help with the pain. Although painful tugging on it. I'm at 20% flex in my left toes and a little better in my right. I'm sure it is slowly messing up my ankles and knees, possibly more, but no problems yet. I'm waiting for stem cell therapy treatment prices to drop to hopefully regrow the cartilage, but maybe it is too late. I had my doctor say all the same stuff to me in Canada. They wanted $6000.00 a joint to scrap it down. @@chriswhitworth7133

    thank you for all these videos about big toe joint issues... I think I have stage 3 turf toe )and probably those little bones are now very big bones too), as at times I would really like to just cut off my toe! but one thing you haven't mentioned that I am sure aggravated my long standing condition is wearing sandals, slides. clogs, flip flops over the years WITHOUT BACKS. So to hold the shoe on my foot i was always flexing, clenching my big toe. This probably did not cause the problem but it definitely aggravates it. Now at 56 I can only wear shoes with backs and I have to tape my toe pretty much every day and all my athletic shoes have carbon fiber soles. Golf is another killer sport for the left big toe even with stiff golf shoes. I think you should talk more about proper footwear besides athletic shoes. I was never a big high heel wearer as that would be an obvious problem but these flat shoes are no good too! Again thank you for all your great information!

    I got mine over many years of overuse, though my surgeon said it can be genetic. Sure enough, my sister, brothers and my mother have it also. I have it in both feet. I first noticed the pain in my early 20s. In my late 30s I started to complain to my doctor. He said go as long as you can before getting surgery. One dr. suggested having it fused, another said no way, it will screw up my gait. Well, having been in the Army, they train you to go forever. Finally in my early 50s I couldn’t take it anymore.I had cheilectomy done one foot at a time. Got great relief, but the one toe never got as much ROM back as the other one. That one is almost fused now, and a little painful. I’m wondering if I should have the surgery again. I’m glad I had it done 10 years ago, but the recovery was a beast.

    The information in your video is super helpful! I’ve recently had toe fusion surgery after many years of dealing with toe issues. Going through the process of running and getting movement again. The carbon fibre insert wasn’t helpful post surgery for me, at least at that point but trying an orthotic now and seeing improvement. Has been such a long process for my scenario. Again, Thank you for providing such excellent information.

  • @benhirsch9488

    Not sure what the actual diagnosis of my issue would be. I was going down steps, barefoot and fast. Big toe caught folded under foot and I felt it as I stepped on it under the foot. Didn't know if I should keep it moving or immobilize. In hindsight I should have immobilized it for when it had to bear weight (walking), & then gently flexed it every evening w/o weight. Now (years later) it is about twice the size of the other big toe's joint and has extra bone on top of the metatarso phalangeal joint. Like a bunion but on the top of the foot. Almost feels like I dislocated something & it never popped back into place.

    Are the end suggestions such as stretching, shoes implants, etc. Helpful only in relieving pain? Is ny understanding correct that if you have hallux limitus, the only way to get mobility back is through surgery? I love working out but haven't been able to properly lunge or do any movements that require 90 degree bending of the toes, so I'm wondering, is surgery the only way to get thay mobility back? Or is it never possible to get the mobility back, even with surgery?

    • @DrTomBiernacki
      @DrTomBiernacki  Před 3 lety +2

      In most cases non invasive treatments won't correct the spur, but they take a lot of pressure off of it. Even with surgery though it is not a perfect solution, that is where a discussion of pros and cons with your doctor may be the best bet

    I have had big toe joint surgery twice, have been told twice that it can not be done again. It took about 14 years for the pain to return after the first surgery & has only taken about 2 or 3 years for the pain to return after the 2nd surgery. Realy would like to avoid fusion of the big toe. As far as what caused this, 3 things. 1) I ran distance when young. 2) Took karate lessons when young, where the instructor had us barefoot kicking the floor of a basketball court to "build up" calluses on the bottom of the big toe joint. (only years later did I think that wrong, who is going to stop in the middle of a fight & take their shoes off in order to kick? STUPID !!!!!!) 3) I had a job where I was on my feet all day climbing ladders etc. Just before I got a desk job I saw a surgeon who said he could fix me up, but that I would need a desk job for six weeks to recover. There was no way that employer was going to do that for me, so I used pellet packs that I would heat in a microwave & wrap around the big toe joint. I tried all the over the counter pain meds, they didn't even remove any pain at 3X the normal dosage. Changed employers got a desk job which allowed me to postpone the surgery for 6 years. 15 years later had the surgery a second time. Now whenever a high pressure front is moving in my toe feels like it is in a vice. I have been using CBD oil topically which does help, but of course is only temporary. That's my story.

    I just saw a podiatrist after a year and a quarter of big toe pain and was diagnosed with hallux limitus but i’m confused. Im a big runner who wore crappy shoes many years but the pain started after someone stepped on my big toe playing basketball. Not thinking it was serious I ran a lot after the injury and the toe pain came back. It got very bad after 2 weeks of running 5 days a week. After it was intermittent. So I’d think my foot was healing on its own after a lifetime of getting sprains and self healing foot injuries. So now seeing I have spurs I’m wondering if you see lots of injury precipitated spur growth, like how you mentioned the 5% of people who get hallux rigidus from broken toes. can spurs develop in a year or so as bad as mine did if my severe sprain a year ago caused them? Im somewhat suspicious I had spur growth for years from like I said bad shoes and lots of running. So does that mean my injury really wasnt related to my condition and I was due?
    Your channel is very appreciated. Lots of folks out here who take hits to morale realizing their feet will never be the same

      Try hoka shoes! I’ve bought the bondi 7’s. It’s there newest shoe.. i couldn’t walk for a couple months at all... I’ve worn these and they allow you to atleast walk... it’s a cover up from the real problem but they work because what happens is your toe can’t push off properly and these shoes have a rocker built into them which allows your to toe push off. They are pretty sick but ugly as hell . Hope this helps. Don’t buy inserts and please don’t get surgery.. there’s always a way to heal!

      Thanks for the great response, Soraninja!
      The only definitive way to know is to get an X-ray to confirm the osteoarthritis. At 31, unless there was a significant injury, it might be a stiff or swollen joint (this is probably the more common issue.) If you are in Michigan, we would love to help more Doing many of these things can cause significant improvement.!
      If possible, we would appreciate a quick & easy rating from you, and a good rating makes a BIG difference for us!

    This was very helpful, but then you left a hole in the information. First you said to see if you can bend it. Then you said see if you could be at 90° and then if you can’t bend it at all so they can’t just be 90° or not at all. If you can still bend it, what is it called? Can’t leave that option out of their, doctor.

    • @DrTomBiernacki
      @DrTomBiernacki  Před měsícem

      There are 4 stages of joint disease= 4th stage is essentially 0 degrees motion or under 5 degrees or so, 0 degrees is full range of motion (there are more factors than just bending), but in generally its a grading scale. In between is referred to as hallux limitus, the 4th stage becomes hallux rigidus.

    Surgery big toe fusion?

  • @CheaperEngineer
    @CheaperEngineer Před 2 lety

    Root cause is NOT over-use, but footwear which does not allow toes to spread naturally (laterally). This leads to sesamoid misalignment, extensor hallucis longus muscle tightening, bone growth back towards ankle, and then joint binding.

      Usually it can decrease the inflammation, but in some cases it doesn't fix the underlying biomechanical issues and the long term prognosis could continue deteriorating. At the end of the day an injection is a short term pain reliever. Not specific to you, but in general long term results are by a long term biomechanical solution. Hope you can find some help!
