THANK YOU very much I appreciated the fact that you didn't change your accent, this is not intending to bash ( excuse the lack for a better term) instead I was able to concentrate better on the topic instead of getting distracted by the pronunciations.. it's way more familiar and topic focused. I really appreciate it. more power to you. thanks for the tips, well explained. Clear and simple.

Thank you for this video. Why should evaluation date and time be the following day while surgery is scheduled at 18:30 of same day? Is the evaluation not also pre operative?
    Please clarify me

The evaluation written and explained in this video is for today, not tomorrow.

Hi po. Nurse Audrey, how to make a plan for a community patient? Especially the aim of care? Re-evaluation date? Example is nursing problem is poor nutrition r/t poor appetite. Thank you

Hi. If it's a community scenario, check your scenario if it's requiring you to do 2 care plans suitable for the next 24 hours, a week, a month, etc; that's where you'll based your re-evaluation date.
      Example:
Identified Main Nursing Need/Problem: [ name of patient ] has poor nutrition related to poor appetite (The root cause of your nursing problem here is the "poor appetite" that should be addressed in your aims of care.
Aims of care: [ name of patient ] 's appetite will improve / [ name of patient] 's will be provided with more nutritional food choices to improve his/her appetite
Re-evaluation date [ re-evaluation is the RE-EVALUATION of your CARE PLAN not the frequency of your NEWS2/PHQ/GCS score ]:
      if for 24hours:
[ Name of patient ]'s care plan will be re-evaluated tomorrow, [ insert tomorrow's date and time on your scenario ] or if clinical condition changes
      Hope this helps.
