How will you know if it's in the implementation? Planning is the second station after assessment. Not all patients who are anxious requires anxiolytics unless it's prescribed. If it's not relevant, then there is no need to write it

@@alexisjeanbascon994 That's correct. However, that is the medication hx and not the drug chart. It may/may not be prescribed during hospitalisation or not needed at all. Remember to focus more on nursing interventions which means non-pharmacologic ways of dealing with anxiety. Best of luck
