Please i have got questions, must all the identified nursing problems have an evidenced factor? Again, you mentioned that medication for anxiety should depend on thr scenario, and you may not know if patient will be prescribed anxiolytics or sedatives except in the implementation stage, is it ok then not to write that part of adminstering drug

You may write that but make sure not to include it on the evaluation letter or SBAR if it never happened.

Hello, if I write 6 or self 7 nursing interventions how many self self should I write?
Is it compulsory to write self care? Can I write only interventions?
If I need to write self care, does it need to correspond with each interventions?

Thanks for the video, but I'm a bit confused between reassessment and re-evaluation? Aren't we supposed to write the date of re-evaluation of the care plan and not the time of re-assessment of obs? Please clarify?
