This could easily happen here in Iowa where I live. People speed and change lanes all the time, even though there are w2ell marked speed zones and continuous white lines painted on the roadway, supposedly to keep the lanes separate thru the work zone. There may have been a third car that pushed the one into the other, but if she were changing on her own, well, she wouldn't want me on her jury.
There is no traffic enforcement on 695 at all anymore. It's like a race way and people will tailgate you even if you are going 75-80 mph. I bet she was tailgating someone in the middle lane then tried to cut the Volkswagen driver off in the passing lane but clipped his front fender and lost control going over 80-90 mph.

This is how I have my living for the last 40 years. I drive a quad axel dump truck and I do the dump truck work milling and paving in Charlotte and hear sometimes we are behind Jersey walls but about 80-90% of the time we only have those sturdy 🙄plastic barrels or cones and on our loop 485 they have construction zone speed @70 mph. That is total bull shit! In my 40 year and still active term I see guys get killed on a regular basis. People do not pay attention to what is going on in front or behind them and everybody has to be first. Not only do the brain dead traffic engineers that have no idea what they are talking about will go on tv or the radio and tell people to ride in the closed lane to the lane closure to try to merge and that is the very thing that causes most of the accidents because of the impatience of our motoring public. In reality we put lane closure signs two or three miles out ahead of the work Zone we do that so everybody has time to merge before you get to the lane closure. If people would would merge into the open lane as soon as safely possible we would not I repeat we would not have bottle necks like we do. If all traffic were in line in the open lane when you enter the work zone we could roll smoothly with no back up but everybody has to be first so they ride in the closed lane to the closure and the moving traffic has to stop to let them in line and that is why we have bottle necks and hot tempers and that is what gets people killed, I have seen it happen many many times I was narrowly missed in the late 80’s by a drunk driver while I was sitting in the paver I just handed my ticket to a young man just 23 years old with a wife and two kids as he walked away from my truck to the truck in front of me a drunk driver crashed through the plastic barrels side swiped the truck in front of me and hit the young man from behind, he never saw it coming. He was hung under the car and dragged a 1/4 before they got the drunk stopped. It was much like seeing a deer splattered except for it was a friend that I worked with every day. People slow down pay attention to what you are doing and no matter what you hear to the contrary merge as soon as safely possible get in line and drive safely as our signs say let them work and let them live.
The opening in those jersey barriers should not have been exposed to traffic like that. The left lane should have been closed while that gap was opened in their work zone. Somebody messed up and I hope the families of those who were killed see justice. I’ve worked on the highway on both the north and southbound sides of where this accident happened and I can tell you this should have been easily avoided. What a horrible tragedy. Condolences to everyone effected.

It's going to start occurring with more frequency as people who already are angry and frustrated are forced to go through increasingly higher numbers of construction zones, with little work actually being done. As a truck driver, it's almost as if, regardless of which state I'm in, it's just one long construction zone. I will see zones where they have an entire lane shut down for 10 miles, forcing people to drive 35-45 mph, just to finally get up there and see two guys standing in the median, leaning on a shovel, playing on their phones. Zero roadwork being done. Then, 2 days later going back through, and it's moved an additional 5 miles. Causes unnecessary traffic slow ups and frustration. They are doing it to themselves, and I simply have no more sympathy

I’ve worked in road construction for years. I understand the frustration of being stuck in traffic, but in most cases the traffic is caused by inattentive drivers.
