    Hey Rob, I really appreciate how thorough you are in your assessments and research. Every time I watch one of your videos or read one of your posts, I always walk away with new information that helps me make a more informed decision. Thank you for being such a reliable/trustable leader in the space.
    Here's the protocol I've been on for the last 4 months:
    AM:
    1. 15 minute scalp massage
    2. If Friday: Full top of scalp derma needling session
    3. Apply 1cc (1mL) of topical
    PM:
    1. 15 minute scalp massage
    2. Apply 1cc (1mL) of topical
    Topical:
    1. Finasteride: 0.01%
    2. Minoxidil: 6%
    3. Biotin: 0.2%
    4. Caffeine citrate: 0.05%
    I'm pretty sure I'm seeing results, but it's hard to tell since the baby hairs I see now - at the temple region - are not showing up well on camera, and I can't remember if I'd been seeing them since the beginning of this protocol or if they are new. Getting a 4 month follow up trichogram scan done in about 10 days to know for sure! I'll add my progress photos to Perfect Hair Health!

    great video - you have the most informative and teachable knowledge made available for the masses and it is appreciated! a couple of videos back (i think) mentioned i had been mixing my own topical caffeine solution w/purified water (at .005g/100mL, .05mg/mL - OD) with successful visual results. shortly after, increased it to BID and added my own mixing of retinyl palmitate (at .5g/100mL, 5mg/mL) with currently continued stable visual success. so it is good to see you mention it in this video - understand the difference between the two and without having to have a prescription and also for long term use the palmitate hopefully should not be damaging but hopefully be helpful (dosing based on cosmetics research). these are not the only therapies used, also included is: nutrition, microneedling, and oral (really sublingual the entire time) finasteride and minoxidil. appreciate your channel and the well thought out information, thank you for sharing! (p.s. 20:55 the holding back of a laugh at the "fabled biotin" hahaha)

    Rob, since we are not professionals and you support techniques like scalp massages, and you demonstrate doing them in a very forceful way, I think a video teaching how not to damage your arteries (especially the temporal and the occipital) would be very useful and reassure people abou if scalp massages are safe or not.

      Scalp inflammation is the result of internal inflammation and/or external inflammation. The external inflammation is cured when you start taking showers with purified water and not being exposed constantly to microbes or toxins in your daily basis.
      The internal inflammation you cure it changing your diet to a autoimmune diet due to the fact that you have a disruptive microbiome and leaky gut, so in this case only eliminating ultraprocessed foods may not resolve the whole problem because you could have a sensitivity to a food that is usually healthy.
      You'll need to keep eating like that for months, slowly adding lots of fiber (50g fiber a day), omega 3, vitamin d3 5k iu and probiotics foods / supplements and you'll be inflammation free in your whole body also in your scalp
      Extra points: meditate, laugh, smile, walk min. 9k steps, do cardio min 3 per week

    Hi rob im 17 years old dealing with hair loss (not receding just thinning)I’ve really only tried your massages for a week but have seen insane stabilization/ small growth it’s borderline shocking I have to mention that I am using an Essential oil (Rosemary) and have been for around 3-4 months and with the essential oil I’ve only seen very very slightly thickening but lots of shedding so I guess they really go together well huh 😀 anyways lots of love and thank you for your research and dedication god bless
    Edit: pls note my diet and workout are not very good and have not been since young if anything wayyy under so I hope that by addressing these issues this year prioritizing things such as Gut health and Exercise I hope to see more results . As for the messages I’m only really massaging for 5-15mins a day so I hope I can gather myself together and reach 1 hours ! My hair care is trash water+essential oil (ikr wth) I’ll do something about it this year

    I'm done,.... More than done. Coming May, i'm going to see one of the best hairtransplant doctors in the world, in Bangkok. I tried the following:
    - Foams for 3yrs
    - PrP x 3
    - Had my scalp examined with a microscope and even got piece cut out for testing
    - 4 micropigmentation sessions
    - Combined finasteride and minox in a topical spray 2yrs
    - Bought the Derminator 2.0 and started stabbing my scalp with 2.5mm needle depth 1yr
    - Now i switched to Oral Min at 0.25 and Fin at 1mg, last 2 months
    - MK677 for benefits 1yr
    I did stop the hairloss and the inflammation pain, i do TrT with Gel now and i see stabilization, just no regrowth imo.
    Done pauzing my life, or dressing myself so i can wear a hat xD Might document my travels and give more details on the doctor if anyone is interested

      good on your for trying all of these things, and for so long, this must have really tried your patience. out of curiosity, did you incorporate any dietary changes, and/or scalp massages? i hope everything goes well with your transplant if you end up going that route.

    1 question about your final suggestion. You still recommend 0.25% topical finasteride, but the study showed that like 10x less has almost the same results, so shouldnt it be 0.025% in the conclusion? Thanks

    My current solution: 5% spironolactone, 7.5% minoxidil, 0.01% finasteride, 0.01% tretinoin (1ml applied twice daily). Also microneedling with 1.5mm dermaroller once weekly. Hoping to finally get some results.

      @@anirudh011Some of this I took from the guide @PerfectHairHealth uploaded last month. The studies I found with topical spironolactone used 5% solution 1ml applied twice daily, so I kept that the same. With minoxidil, I found another study where efficacy stopped going up around 7.5%, possibly due to solubility in propylene glycol maxes out at 7.5%. The finasteride is so I get 0.1mg exposure / application or 0.2mg per / day.

    Sorry Rob i threw in the towel. And got a transplant last year. Best decision i made. That was after 4 yeara of massage therapy loose scalp, and dermarolling with minoxidil.
    Now i use topical caffeine and laser therapy. My hair results have been amazing.
    No finesteride. But use saw palmetto and pumpkin seed twice a day.

    I had some serious forehead increases (ha)....then I changed my diet - no sugar, no grains, no carbs - animal based --- this isn't an info-mercial.
    My hair has since (over 2yrs now) started to grow back AND hey, I lost 78lbs in the process.
    I am happy to have this channel as a resource to base some other ideas on...keep in mind, I used to shave my head (voluntarily for 10years)...it was when I decided to grow it back, that I realized it wasn't like it 'was'. I am 53 years old - and because I try to keep the natural approach, I have found some other things that MAY be contributing to my hair growth.
    I made a mixture of organic pure castor oil, lemon juice, rosemary oil. I have zero knowledge on if/why this is working...but it seems to be adding to some fullness over the last few months. (just be cautious it doesn't get in your eyes). I wear a bandana to bed to avoid the contact. I just started to research the value of castor oil and I went down a rabbit hole after that.
    Anyway - just thought I'd share...to be honest, I am probably the least caring guy here, about my hair growing back - I can shave it all off tomorrow and be fine - the wife prefers it shaved anyway...but MAYBE there is some value to that mixture. Has anyone else tried topical natural products with success? Maybe a mixture juggle could be a breakthrough

      @@Crushin123 If your diet improved your hair, then the cause (or at least some of it) was something other than male pattern. Lots of things can cause hair loss: nutrient deficiency, thyroid/hormone, stress, etc. You could have male pattern in addition to another cause. Your diet might have taken care of the other cause or it might have improved the hair you have left, but there's almost no chance diet impacted male pattern.

    I never heard the news about Fish Oil. I use it everyday. It was reported in the news recently that it could help reduce the chances of dementia and never mentioned the heart risk. My 95 year old mother has been taking it daily for years.

    What about RCP mate? Like the Revivhair treatment? Many claim that hairs wont fall off if you stop unlike minoxidil. I am interested in knowing the regrowth effect of RCP as i get sides from even 5% minoxidil.

    Hey, great video and I always wait for you to upload something related to these chemicals and other possible solutions.
    But I was searching google for the side effects of finastride. One study showed it reduces the risk of prostate cancer but google said it increases the risk. So contradicting, and I'm so confused.
    Could you please clarify the side effects or any potential risks ??

    I'm a male and I lost 30-40% my hair out of stress in exam 20 years ago when I was a kid but my hair didn't grow back. No recession on the hairline and the pattern is more like female pattern hair loss. The loss is stable for 15years, but in the recent 5 years, it is slowly getting worse (still no recession on hairline). Tried 1mg finasteride a few years ago for almost 2 years, but that has no effect on me (no hair re-grow). My dad and his brothers show signs of AGA at their late 50s. Am I too late for trying to re-grow my hair since it was a long time ago and all I can do is to at least try to stop it getting worse?
    Would you mind sharing your thoughts on my case? It would be much appreciated.🙏

    My age is 19 now but my hair st fall out since age of 17, but in age of 17 my hair is fall out like from the roots but its not get thinning then I use to worry about it then go to the dermatologist they give me some sort of oils kits with shampoo i applied for that about six months nothing changed again check to another dermatologist the just give a minoxidil and said apply it for 6 months,they don't say about its side effects I just applied for a month then stoped when i know about the side effects then again go to another dermatologist they recommend PRP after the 5 session my hairloss not stop then check the testosterone levels that also normal and then my family's side none of us have a balding what is the reason for my hair loss

    Can i switch to topical combination of mionoxidle 5% and finasteride 0.1%
    Is it effiective for blocking dht 0.1?

    Hello everyone, I'm doing a lot of research on potential treatments for androgenetic alopecia. I see that many people think that the new treatments in development will achieve nothing and others who affirm "that there is no better time to have alopecia"... I read that by 2030-2035 the problem could be solved. Do you think this is realistic ? Or is this still a fallacious promise ?

      Hey there! The recommendations in this video are basically for Bryan to consider switching to oral finasteride, provided that his serum DHT levels are as suppressed as we suspect. There is a possibility that he’s still getting some side effect reduction from his topical, even despite the high dose of finasteride, depending on his circulating finasteride levels (related but different from serum DHT). But for those looking to maximize finasteride localization to the scalp, we still recommend the lower dose topical formulation you cited.

    I use Hims topical, it’s Finasteride 0.3%, 1ml dosage per day. That would also be above the required dosage size, correct?
    It calls for four sprays once a day, wondering if I can reduce it.

      If you have a measuring cup for medicine - even something like children’s Motrin - we can figure out your exposure by having you spray 4 times into the cup and seeing where you land on mL lines. Or, just count how many sprays it takes to get to 1 mL. Then I can do the math for you.

    I'm curious why you didn't recommend LIPOSOMAL topical finasteride? There's a Milan University study that shows it's systemic absorption is 1/18 compared to oral.

      We've tried to interview Dr. Chiara Insalaco about her (non-peer reviewed) data for years! No luck. At this point, I'm skeptical that the liposomal formulation is advantageous. Liposomes are actually poorer carriers for finasteride vs. ethanol. My suspicion is that liposomes probably confer little benefit, and that the localization effects are probably due to this alongside a lower dilution of topical finasteride.

What are your thoughts on companies like Xyon which use a special/patented liposomal formulation that they have conducted their own research on?       I think it’s more so about her not wanting to discuss the data. To my understanding, she has since moved from liposomal to silica formulations of topical finasteride. RE: XYON- I’m skeptical of just how much localization is taking place. I’d like to see serum tests to confirm none of the dutasteride vehicle is going systemic. If there’s a detectable amount, this might hide just how much dutasteride is going systemic and getting transported elsewhere in the body.

      I currently see liposomal delivery vehicles as (more or less) a marketing trend. There are certainly instances where ingredient penetration might benefit from liposomal delivery. But the application to hair growth products is far less than what marketers often suggest.

    I’ve tried anything and everything. Drugs, massages, natural etc. everything that is pharmaceutical gives me sides. Even the .0005 finasteride topical. I even dropped it to every other day and still got sides. Went into everything with a great attitude too. Not sure how it’s even possible I can be that sensitive. It’s so frustrating and exhausting at this point. Ready to give up.

      @@nmc5657 dht is a hormone. It doesn't work like that. Any male that has gone through puberty makes higher amounts of dht than kids/females. Which is why men lose hair, called androgenic alopecia. Thats like saying that if he de-ages himself enough, he will stop making testosterone- this will not happen.

      @@MrPeaps yes it's a hormone, but there's a time (post-puberty) that 99.99% of men are producing DHT yet not losing hair - why doesn't he "reverse age" to that point in his life?
      to be clear, i'm not disputing what causes hair loss, and fully advocate people taking care of themselves and even offering themselves up to the public as health experiments, but i think this "reverse-aging" stuff is a tad ridiculous, especially as it relates to his hair loss. i can "reverse-age" many things by popping multiple pharmaceuticals.

    Can I ask why we are going backwards with this hair growth stuff....... I mean of course topical solutions of dutasteride and minoxidil are going to be somewhat effective, but he also had a hair transplant and does prp....... But more importantly topical finasteride slash dutasteride are all increasingly toxic, we've known this for the last 20 years what are we going back for this so what all the massages and DT therapies don't work?????

      Plenty of people still making money on hair loss and very few getting results that matter. It won’t get solved until the pharma companies come up with something better or it’s dealt with at the genetic level. Meanwhile we spend a fortune on things with side effects and hours massaging and rolling to little avail. I think the OP is advancing the cause and making good sense out of what’s available. The take away is it’s hit or miss, including finding a good dermatologist that will see you anytime soon, and perhaps transplants are still the best solution since they have seemingly improved.

      I think he’s doing PRP / exosomes / dutasteride as mesotherapy. For what it’s worth, I think autologous therapies (exosomes, PRP, PRP + Acell, stem cells) are no more effective than microneedling. Mesotherapy dutasteride might be worth it, especially for some individuals. We’ll have videos about this in the future!

    Your comments regarding minoxidil 10% daily being equivalent to minxoidil 5% 2x daily are incorrect and dangerous.
    Total drug exposure is not the whole story. It is also highly concentration dependent. Same thing applies to other topicals such as finasteride.

      I know there’s heterogeneity in study outcomes measuring hair growth from topical minoxidil solutions greater than 5%, but I’ve also looked into these studies. I feel the one study suggesting superiority of 5% vs. 10% is a bit misleading; those results might very well be explainable through stronger hair cycle synchronization occurring in the higher-strength groups. Since that study only ran 36 weeks, we’d need a longer time horizon to tease that out. Moreover, the other studies seem to show added benefit from higher-strength solutions of minoxidil, particularly for non-responders to 5%. This is echoed by oral minoxidil dosing studies comparing response rates. All of this (in my eyes) suggests a dose-dependent effect from both topical and oral formulations, though some extrapolated models suggest diminishing returns after 7.5% for the topical (which haven’t yet been tested adequately).
      As far as safety - you’re totally right that with increasing drug exposure comes increasing risk. When I say consider increasing the dose, I’m not also saying doing so won’t change the safety profile vs. twice-daily 5% minoxidil. Bryan will need to decide what he is/isn’t comfortable trying. But as most of the side effects from higher-strength minoxidil relate to skin irritation, and given the ability to manage this side effect with other ingredients / carriers, I think he can take most of that risk off the table.

      I agree with your points.
      However, perhaps I wasn't clear with what I was trying to say.
      From the textbook I referenced:
      "The hairgrowth effects of Minoxidil are concentration-dependent. It does not matter how much active substance is in the solution; only the end concentration matters."
      So minoxidil 10% 1x daily vs 5% 2x daily, yes even though they both will contain the same amount of active drug, their efficacies can be vastly different.
      I'm not saying it works exactly like the following analogy, but it's something to consider:
      100 L of 10% HCL
      vs
      1000 L of 1% HCL.
      So both of these have the same amount of active substance, HCL, but they will have vastly different abilities to clean a driveway for example. No matter how much volume of 1% HCL I have, it will never be strong enough to be a good cleaner. You need a strong enough concentration.
