    I was recently tagged and had sent a clip from an interview with Dr. Karan Rajan seemingly bad-mouthing chest compressions. Folks in my DMs and in my comments know how strongly I advocate for CPR, and they were eager to watch me attack this clip and fact-check Dr. Rajan. The thing is... I can't, because nothing he said was incorrect. That doesn't mean I'm not without my outrage though, as I believe this experience is emblematic of TikTok outrage culture and my core issues with that platform: the algorithm and audience is designed to highlight anger, divisiveness, and is breeding a culture I'm not enjoying being a part of. This has led me to wonder... should I stay?

      I’ve already requested this in a previous video but would love to see you two collaborate to discuss differences between the American and British health services. I have a huge respect for both of you and think you both make social media a better place in terms of education and compassion but absolutely respect the decision to walk away from an app that causes so much unnecessary drama and negativity.

    I don’t understand how this ever became an issue in the first place. Dr. Karan said that the success rate for CPR was much higher in healthy, strong, younger adults, but with elderly people it’s much lower and even if it works it could just straight up be non-beneficial. You’ve said before that often times CPR breaks ribs, and I thought to myself “of course something with a high likelihood of causing injury in the process of saving someone would be less beneficial to an elderly person with a weakened ability to heal!” And his explanation also made sense because of course no heartbeat would starve the brain and cause brain damage! It doesn’t make sense how people misinterpreted this as “CPR bad” because that’s not what it is, it’s about how CPR *can* save a life but it’s also not a miracle worker!

      This!! You can be sued for causing injuries during CPR, that's why you *should* be certified if you're going to do it spontaneously.

      I 100% agree with this, but the people who originally complained probably won't listen because they're the same type of people who constantly look for stuff to be mad about.

      Social media is making our patience worst. You can see that in cases like this when people hear and immediately respond instead or hearing, listening, thinking, researching and responding. They leave that part to the people their responding too. Like all those comments, they could have easly got the answer to their questions by simply thinking and researching but no, they chose to leave that work to Doc.

      The people who are “failing” to understand that CPR is a great thing, even if it isn’t always successful or causes other damage are missing the point for a simple reason: they don’t care about understanding. They want to cause strife and make useless arguments for clout or because they’re spiteful people, rather than spend time to get informed. They don’t care about being informed; they care about provoking people.

      ​@@RolaiEckolothat depends on the country. In Germany you can be sued for not helping, but you will not be sued for anything you do wrong - unless you are a doctor and you were negligent.

    I had to research DNR and CPR when I was a caregiver for my nearly 90 year old father. I was shocked to learn that in older people, ones over 60, the results of CPR are limited and sometimes horrible. What Dr Rajan said is absolutely correct. People are oftentimes brought back to life by CPR, only to languish for some time in the hospital and still end up dying while in the hospital. I determined right then that after I turned 60 I would fill out a DNR form for myself. I am 68 years old and still feel the same. Whatever a person decides on this matter, it should be respected. Bodily autonomy is very important.

      This is true. In fact there are a number of factors that come into play. If anyone is interested, there is a tool we use to support our decision making (support being a crucial term there - we do not JUST use this tool) called the Miracle2 score that stratifies the impact of various factors on survival after a cardiac arrest.

      Yup in old patients even if we revive them, they usually stay on ventilator support for 3-4 days and pass away. You're just hurting them with unnecessary invasive procedures.

      I think we have to be careful about who we blame here-while we absolutely need to be critical of what we watch and how we perceive it…I actually think it is social media’s fault. They’ve created this drug that we’re all addicted to and it makes us more irritable and reactionary. Not to mention young kids are the main target. We need to place more culpability on apps than on viewers.

      That's what I came on to say. It's hard to be truly careful on short form. Dr. Mike, you can spend your time and mental energy trying to right a sinking ship, but you don't have to. You have SO MUCH going on in your life, so much involvement in other social media, work life, side projects, entire new professional paths (hello boxing!), a wonderful giant dog...YOU don't need TikTok.

    I gave CPR to my dad... Unsuccessfully (it's his birthday today). When they don't spring to life, like they do in the movies, you feel like you've failed!! I found out years later that the chances of CPR working are generally very small! And there is nothing more I could've done. So I get what he's saying. And at NO point did he say don't bother! Would I do it again, knowing the success rate? HELL YES! Because even a tiny percentage of success is better than 100% of nothing. And it could just save someone's life.

      My mum works for a health center and they had a fair day where all sorts of people come in to see the stalls. My mum asked me to come and help with the cafe stall but while I was there, I also got myself a CPR certification. Never know if I might need it.

      It's something that actually makes me feel *more* confident, as essentially without CPR they're extremely unlikely to survive, so you might as well try even if you're really unsure! You're not going to make it worse, and anything is better than nothing here.

      I got news for you, cpr doesn’t save your life, adrenaline and defibrillator does, just doing cpr doesn’t help, just significantly increase chance of survival when you do treat, but I’m sure you called an ambulance and defibrillator didn’t save him and you did the right choice.

    Yes, Dr Mike, you should leave TikTok. It's the appropriate time, & it is the best move you can make to try to protect medical integrity on that platform. It has done all it could for you. Your career will stand in its' own. You could write a book on this very topic, & how it is affecting the medical field, & people will eat it right up!!     Thank you for championing "do your research before you respond". I hope we can get more people on this boat. Unfortunately, this isn't a new "tiktok" problem. It's been an issue since the 90s when everyone first got email and started forwarding "send this to everyone you know" junk.

    Brilliant video Dr Mike. I cannot stomach TikTok & have never signed up to it. I'm prudent about which platforms I give my personal info to & TikTok just does not make the cut in terms of privacy assurances or quality of content so it gets the big 'nope' for me. I don't think your online presence or impact would be damaged by cancelling your TikTok acct, in fact I think it shows sound judgement given the direction the app is going.
    P.S. My Mum saved an older man's life who's heart stopped in a public pool through CPR & I learnt it as part of my teaching career - vital skill that imho all adults should know.

    CPR saved my little girl. She has Myasthenia Gravis. At the time we didnt know and she went into a crisis moment and stopped breathing. Your voice was in my head "chest compressions, chest compressions" i got her back just as the first responders arrived. Id had CPR training before but having that mantra in my head shaved off seconds where panic normally takes over. Thank you Dr. Mike!

    Dr. Mike, this made me realize that I would love to hear you talk about hospice care. Two of my aunts died recently who were influential in pushing the hospice movement in our state.
    They were equally passionate in their opposition to the lives of the elderly and sick being treated with disregard by medical professionals who believed they would be better off dead.
    My family had an awful experience when my grandfather died, which caused one aunt to write an article about it. She received responses from readers who had similar experiences. That was the start of her advocacy, which led her to getting trained as a bioethicist.
    My other aunt, her sister, worked for change from within the system as a federal employee in a high position.
    I think there’s a lot of confusion over what the difference is between euthanasia (direct or through neglect) and things like DNRs and comfort care at the end of life. Knowing my aunts’ activism and having recently accompanied them both as they benefited from the hospice care they helped to put in place and hold to high standards, I find myself very interested in hearing your thoughts on the subject.
    Great video, as always! Love the lesson in critical-thinking.

      I never joined TikTok. I also left twitter and FB. My issue with TikTok is just what has been said here. People are constantly creating toxic drama (and abusing their pets) for the likes. FB, I realized I liked my friends less when there was zero mystery to their daily life, without any real connection other than getting sucked into whatever this weeks drama induced dumpster fire happened to be, and I said so long. I like a few channels on here, but just the same there are too many channels of people that think they know everything about something and I just want to shake my head and say “You know nothing Jon Snow, Dracarys”

    This hits really close to home. My father had a heart attack at a gas station and a good samaritan did CPR until help arrived. I know they saved his life and I'm thankful but he still ended up dying the next day because he had been without oxygen too long. It really woke me up to what a hailmary CPR is.

      When I was nursing we always put out a crash call if someone on the ward had a heart attack. Even with immediate 1st and 2nd aid (standard cpr by ward staff then intubation etc by crash team) most patients never recovered.
      This situation eventually led to the introduction of do not resuscitate conversations with patients and/or relatives.
      I have personally attended a collapsed man in his home. Even though I knew he was not going to make it I and another neighbour started cpr till the ambulance arrived. He died shortly after they got there.

    Man, even if all cases of CPR had an insanely low survival rate, I'd STILL do it, it's a free and immediately available form of resuscitation. A low chance of survival is better than no chance of survival (if CPR is right for you as an individual, that is).

      I bet most of those who were upset by this, have never and would never have bothered to learn CPR and be a good Samaritan in first place, but still find a way to think they've been lied to.

      I think you are misunderstanding this. The video discusses do not resuccitate forms for those with terminal illnesses or are of very old age. If someone in that category was to be brought back with CPR, it wouldnt benefit them, as instead of that person simply going to wherever your religion believes they go, they get brought back, they experience even more pain than before for no real benefit by bringing them back, and thats why some do not wish to be brought back.

He was talking about the misunderstanding surrounding the efficacy of CPR when it is appropriate, not the wider context of the original content. In fact, the parenthetical statement in his last sentence makes it fairly obvious that he understood that being resuscitated isn't the best outcome for everyone, thereby demonstrating his understanding of the concept of a DNR/POLST/MOLST and the need for such things to exist. For those who do not have a valid DNR or other valid instruction (e.g. a person with valid and current Powers of Attorney) to not resuscitate, early and high-quality CPR is a person's only chance for survival. Even though the overall success rate for getting ROSC is very low, that doesn't mean it's not worthwhile, as Plastic indicated. So long as you do not have a DNR or other such instruction in effect, an 8% chance of survival is certainly better than a 0% chance of survival.

      Survival rates from witnessed arrests, can be raised from 5% to 10% to be as high as 25%, as evidenced from the Netherlands, where there is a civilian network of trained first aiders, coordinated by the professional emergency services, which can be called upon if there is a high chance of survival. Those people are people already living in your local neighbourhood, and are able to arrive on on foot or by bike, before the ambulance or other professionals arrive, capable of taking over CPR from you to extend the amount of time where high quality CPR can be given, as well as bring an AED if one is available close-by. Multiple people get sent to the same call to ensure someone is on-site as quickly as possible.

    I put off watching this video because I love watching both your and Dr. Karan’s content and was worried this would make me dislike one or the other. Thank you for continuing to be level headed! Everything he said is actually true. CPR has a very low success rate. But that doesn’t mean there isn’t value in doing them in hopes it saves a life. And it’s so important to keep blood and oxygen, however minimal and slow, flowing to the brain while waiting to have access to better care/technology in the hospital. There was a really informative article about this and ECPR in the New York Times recently.

      That's a totally different scenario! You as a life guard are NOT a doctor and don't know the patient's health history. It's not up to you to decide! But a terminally ill patient who if they're lucky enough to survive the cpr, would likely live out thier days on a respirator DOES NOT benefit from cor. Hence why me, my mother and my terminally ill father decided to have a DNR! You absolutely CANNOT extrapolate your use of CPR on the beach or in the pool to ALL other settings. This black and white, over simplistic thinking is what is causing misinformation problems in the first place! God, try to use your critical thinking skills man.

    I've saved my grandmother following doctor mike's advice. On 22th she suffered with a heart attack I did the chest compressions and she came back we took her to the hospital. Sadly, I lost her on the 24th but I believe it was due to the chest compressions that all my relatives and family were able to see her once again. I will always be grateful to you Doctor Mike. Kudos.

I know him.” And let me just say, I super approve because I know you’re legit.     My mother saved my father with cpr after a heart attack, and since she started compressions right away my dad had a good outcome eventually. Hospital staff kept coming to meet her because she is such a badass.
    And yeah, the more people leaving TikTok the better.

One argument against leaving TikTok is, if we are not there, then it will be an absolute cesspool of rubbish which in a LOT of places around the world kids will have access too.     We signed a DNR for my brother who was terminally I’ll with a brain tumour. He passed peacefully & was unable to control any of his bodily functions at the time. Bringing him back for the sake of another week would of been cruel & self serving. You Dr’s have these extremely hard emotional conversations with families & do great work. The least we can afford the medical community is “context & fact checking before reaching or reacting”. Love to all you Dr’s & Nurses & all the hospital staff around the world.     I support you whether you want to keep your TikTok or leave it. I agree that there's a lot of stupid out there, but I think it is more of the fault of the algorithms used. Programming is not an exact science, as one who had to learn it in school, and I never was really good at it, though I did make good grades in it. The IT industry needs to get better at writing algorithms that protect especially young people's mental health. We need more scientifically provable fact out there and more fact-checking for these subjects are are science-based. Our mental health as a country is suffering because social media in general tends to be pretty toxic. That said, I'm so glad you're fighting the good fight and putting good info out there for us! And, I do believe that eventually, the process that selects the videos that we see the most of will improve. We are seeing a revolution in AI and in programming methods, and it has its growing pains. I do think TikTok needs to be acquired by another company, but I just hope it's one that will try to fix its most pressing issues. Good luck with your decision!

    As a licensed therapist in TN, I hear so many clients talk about how they use tiktok to 'cope' and can watch it for hours. We discuss other coping skills to use, it can be so unproductive. I personally refuse to download tiktok, I don't need another distraction in my day to day.

      than when I hit a blunt and enjoy exploring my thoughts to some music.
      then again I noticed how listening to only podcasts when on the move mentally exhausted me.

      A strategy I’ve used is based on the Blinkist app advert that is making the rounds. Replacing doom scrolling with micro learning… I actually got a history textbook on Western Civilization that I can turn to when I notice I’m just flipping through my phone and not consuming quality content for my brain. Like a few dance videos and some cute animal videos are good but not like a whole hour of them. So I’ll stop at a few, pick up my text book, just open to a random page, and all of a sudden I’m learning correct information on the Spartans. It gets me away from mindless phone usage and decreases overall screen time. (Edit to add: I doesn’t need to be a textbook, it can be short stories, or even a recipe book just something to actively engage your mind)

    As a recent psychology graduate, we talked about the toxicity of TikTok and the mass misinformation crisis going on within the app. I think that TikTok has fostered a massive dunnen Kruger effect with so many people in its community. So many people feeling overly confident to give medical and mental health advice or diagnosis that they aren’t qualified to give. Honestly a few of my friends have adopted this mindset as well and I’ve had to distance myself from them because of it. It’s unfortunate but Tik tok in particular is so toxic and creates biased feedback loops and echo chambers for its users. I don’t know if it’s going to get better. So if you deleted your account I wouldn’t blame you, the world would probably benefit from Tik tok dying out.

      See I agree with this, because of the amount of misinformation and pseudoscience on the app, but the problem is that there is clearly a demand for this type of content. Maybe we all leave tik tok, but we then still have youtube shorts and instagram reels, that are really feeding into the same problem. I wish therr was a way to hold creators of such apps accountsble to actually make sure they are not encouraging the spread of misinformation.

      Imo TikTok is the perfect storm of algorithm driven content with short form and therefore short attention span. Facebook, Twitter, CZcams and basically all the platforms use similar algorithms to feed and push content all with the same objective, keep you clicking, keep you on the platform for as long as possible. As a result it's very easy to basically inevitable that the majority of users end up in their own little echo chamber on all of these platforms.
      It's short form like TikTok and Twitter where imo attention spans reduce significantly so the content that'll stand the best chance of catching that shorter attention span is drama and outrage.
      Add the general attitude of all of us on the internet that it is somehow not IRL therefore zero consideration of consequence, very little thought beyond that knee jerk reaction that popped into our heads and well, I'm surprised the whole thing hasn't come crashing down long before now tbh.

    Thank you for sharing! This is a topic that has been on my mind for quite some time. While I agree that TikTok, like many platforms, can propagate misinformation, the issue extends beyond just one platform. Content creators often share their messages across various social media platforms, attracting trolls and exacerbating the problem.
    From the perspective of a content creator, I do appreciate TikTok's editing tools. However, having been barred from using it in the military and now at UT, I can't say I miss it when it's not available. Still, I find myself facing a moral dilemma. Knowing that some individuals are sharing harmful content or distorting positive messages, I question whether I have a responsibility to continue posting beneficial content and engaging with trolls.
    However, it's essential to prioritize one's mental and emotional well-being. Finding the balance between fulfilling professional obligations and maintaining personal health is crucial, particularly in the realm of holistic medicine. If TikTok becomes overwhelming, it's entirely acceptable to step back. There are other platforms where your content will be better received. Personally, I believe nurturing your CZcams channel's substantive content aligns more with the holistic approach, rather than being consumed by the chaos of TikTok.

    I was a big user of Tiktok and posted daily and had sponsors etc. I gained 80,000 followers, before I realized how toxic Tiktok had become and how addicted I was. I ended up going cold turkey and deleting the app. A year later, I realize that it's one of the best decisions I've made ❤️

      I realized tiktok was toxic when I saw a mother taking a video of her 4 y/o son, saying he was a girl, and then asking what gender he was. He repeated her. Everyone in the comments were congradulating him, but the few that said it was wrong were verbally attacked. (This comment isn't for political debate, it's just what I experienced. No hate against lgbtq, just the situation in this video and comments was so odd.)

    I like the idea of leaving the tik Tok and focusing the CZcams, because you don't HAVE to cut out the nuance and context to fit in the allotted time. With CZcams you can battle misinformation as you do by providing all of the accurate medical info in one convenient place! Love your videos!

    CPR saved my life! I had an out of hospital heart attack & cardiac arrest when out cycling, a bystander gave me CPR, another person called emergency services - next thing I knew I was in CCU. If it wasn’t for CPR I wouldn’t have survived. I was 58 years old when it happened. Im now fighting fit and back cycling like nothing happened. 100% support you Dr Mike 👏👏

      You were extremely fortunate, my step father was given CPR immediately as well but he was already gone and could not be resuscitated. CPR fails more than it saves, just because you were lucky that doesnt mean the vast majority are as fortunate.

      ​@@jackwhitbread4583He didn't say that. Success of many procedures is determined by many factors. In case of CPR the risks far outweight risks in majority of cases, almost all of them in out of healthcare arrests (as we cannot possibly presume DNRs).

    2 things:
    1, As a high school physics teacher, I find that the way my students interact with TikTok and similar apps does more harm than good. Personally, I have never used it. I agree 100% with what you say about nuance and how hard it can be to achieve that in such a short video. I respect you as a reputable educator and support which ever way you choose to go.
    2, Eleven years ago my dad had a heart attack driving home from work. The police who found him did CPR and brought him back. He was in the ICU for 1 week before having a second heart attack and succumbing. I am eternally grateful for the police and medical professionals' efforts RE. CPR and ICU supportive care. They gave me one more week with my dad to see him, come to terms with the fact I might lose him, and say everything I needed to say before he was gone for good. CPR is worth the attempt, you can't make the person any more dead by trying, and you just might get really really really lucky and give them a little more time, even if it is only one week more. Love you, Dad!

    Hey dr Mike, I think that your educational value on TikTok is important. That being said, I understand your wish to leave. Its a sinking ship that takes your time and enery and gives not a lot back. Please know that whatever you chose to do, your community that you have build on this platform will support you. The followers that you have on tiktok who are watching you for your content will find you on your other platforms. I support your decision on quiting or staying, staying would have the benefit that there is still accurate medical information on TikTok and you might accidentally influence users, but you can’t win the fight against all the wrong information on there. It’s a hard decision but whatever you will chose is right. Don’t wear yourself out over this

    I ODed at 16 years old and died. I'm only here because of the electrons in my brain were still firing off (according to the paramedics), that and chest compressions, EMTs were luckily down the street from my house. Dr. Karan is actually correct and so is Dr. Mike. My mind isn't as fresh as it used to be, I have trouble remembering things way more than I used to. I *feel* slower than I was before and it has severely affected me in my schooling, at work and yes even at home. Thanks for the great content Dr. Mike, and of course hats off to you Dr. Karan. Keep up yhe education.

    I’ve been a nurse for 30 years. TV and movies always show someone getting an AED shock immediately coming back to life. They don’t understand that it won’t restart a stopped heart. In all the times I have had to use an AED over the years I have never once been told “Shock Advised”. The last time I did CPR several years ago that happened in a room full of people in a dialysis unit in front of a doctor and as soon as I heard shock not advised I swore very loudly and creatively at the machine. This should be more talked about in CPR classes. Everyone has been conditioned their whole lives to think the AED will advise a shock, the first time it doesn’t happen is shocking, then gradually I hated waiting for the analysis to happen.

    I deleted tiktok a few months ago and my mind has felt a lot more clearer. It’s so toxic and creates so much anxiety. I know that you want to reach as many people as possible with medical knowledge. I think your energy may be best spent making content on other platforms where people will listen instead of just finding fault and creating drama

      Despite coming there because of a few content creators I watched on CZcams, and finding some other interesting stuff... I got bored on Tiktok very easily. Watching so many videos was tiring. Deleted it. Returned about a year later, watched stuff for a few weeks, then the political shenanigans in my country started and Tiktok basically made it impossible for people from here to watch foreign creators, even ones we had been following, their profiles just said "there are no videos on this page" and while I had found a couple people from my country that I'd watched, I still decided to delete it once and for all
      Feels nice. I can watch YT shorts, which somehow are still better targeted at me, and also alternate between shorts and normal length videos

    Deleting my TikTok was one of the best things I did for myself lately. It was starting to affect my motivation, mental health, and general well-being. I fully support leaving TikTok for these reasons as well as the reasons mentioned in the video. Tiktok's algorithm is designed to quickly figure out what triggers people, and bombard them with such content. It's plaguing our society, and it's gotta go.

    My dad is a general surgeon and mom was a head nurse for years, while we were watching a movie with a CPR scene where the victim 'woke up' and was fine, I asked them if that's accurate. They told me that it,s not correct and the actually mirrored what Dr. Karan said. They also said that CPR should be performed anyway because it may and can help; even if a patient is in very critical condition a surgeon wouldn't just say 'oh he's probably a goner' but perform the surgery anyway and that's the same principle.

    What is heartbreaking about this is that Dr Rajan does actually explain about how he had a patient who was 90 and performed CPR on them and how as a junior Dr it really impacted him, so for people to 'pile on' on without context isnt fair. He comes across as a great Dr and an absolute asset to the NHS and a great youtuber as well.
    Leave Tik Tok, sounds like you could do without the stress tbh and it might encourage others to leave behind things that both do not serve us and cause us stress.

You work hard at multiple things, so it's okay to take care of yourself and shed some toxic stuff. You have a massive, loyal following here on YT.       Definitely, but we also need some way of helping people learn how to differentiate between reputable research and unsubstantiated claims. One of the most popular catch cries of the anti-science anti-vax communities is "Do your research!", when their idea of "research" has nothing to do with scientifically proven facts, and everything to do with heavy confirmation bias and searching on the internet solely for anything that matches their preconceived notions.

      I think setting realistic expectations is not a bad thing.
      If it was my loved one or when I am at work in my ER…I know the low chances of getting them back and the low chance of long term survival even if we do get them back…but a 1 in 10-20 chance is nothing to sneeze at in either scenario. Sure it’s unlikely. But it’s worth a shot.
      Every time we get someone back after a 40 minute downtime and their pupils are fixed and dilated, it deflates me a bit. But sometimes the signs are more promising. I’ve even had a couple that return to full consciousness, obviously in hospital arrests.

      yeah, to my understanding, CPR isn't so much about resuscitating a person (great when it happens), but more of keeping the blood moving so hopefully residual oxygen in the blood supply can reach the brain holding off brain death before a defib machine gets there.

    Coincidentally, in the first video I saw of Dr. Rajan's, he pondered the legal weight of a patient's DNR tattoo. They got wheeled into the OR, and staff started getting his chest ready when they found he'd gotten "DO NOT RESUSCITATE" tattooed on his body. He hadn't filled out a DNR form or anything, and Dr. Rajan went through the hospital's whole process of determining whether to honor this tattoo as a revocation of consent to resuscitation.

      From what I understand, they don't honor DNR tattoos in the US. The idea is that the patient could have changed their mind at some point, but tattoos are permanent.

      I am just glad I don’t have to make this type of decision. I have a DNR with my GP that I keep updating depending on current health issues I am facing and my family knows about it. But having to make a life (or death I suppose) altering decision in seconds - rather them than me.

    I fully applaud what you said over here. I'm a teacher in my 30s, and its so sad and shocking seeing how little younger people today understand how something can be nuanced or biased. That there might be a bigger or larger context you've got to be aware of. Social media has removed all of that for them. Instead, it encourages en masse outrage or attacks rather than allowing us to see the whole picture. Good work as always!

      I know! I watched that clip and was like "why are people reacting so strongly?" Like yes Mike advocates for CPR but this doc wasn't saying "CPR is useless" he was just saying it has low success rate. But a 10 or 20% success rate doesn't make something useless!!!!! He was just adding nuance not calling it useless 😭 but I guess out of context it can seem like his point is not to use it? If you're not really thinking about it..
