I think that line where he says, “I’m like a fire that could burn this fuckin’ town to the ground, and there’ll be nothin’ left but ashes if you let me come down, so you better use me up,” is not primarily about his self-destructiveness, but about how strong he is.
The song, the story in it, and the visual, all convey that he is not a simple victim, he is an incredibly dangerous individual (yep, he was in the mafia in life) who is only barely contained by the “prison bars” of his addiction and his deal with Valentino.
If his clients go too far or make the mistake of not keeping him high, he can and will wreck them. Do they really want to try their luck…? Better use him up, bc if there’s even a little left, they’re courting disaster.
