  • Dr O'Donovan explains 10 things you can do at home to help manage the symptoms and signs of an anal fissure.
An anal fissure is a tear or open sore (ulcer) that develops in the lining of the large intestine, near the anus.
The most common symptoms of anal fissures are:
    1. a sharp pain when you poo, often followed by a deep burning pain that may last several hours
    2. bleeding when you poo - most people notice a small amount of bright red blood either in their poo or on the toilet paper
See your doctor if you think you have an anal fissure. Do not let embarrassment stop you seeking help. Anal fissures are a common problem GPs are used to dealing with. Most anal fissures get better without treatment, but your doctor will want to rule out other conditions with similar symptoms, such as piles (haemorrhoids).
They can also tell you about self-help measures and treatments that can relieve your symptoms and reduce the risk of fissures coming back.
    Anal fissure treatment options:
    Treatment Options for Anal Fissures
    1. Dietary Changes:
    - High-Fiber Diet: Increase intake of fruits, vegetables, and whole grains to soften stool.
    - Hydration: Drink plenty of water to prevent dehydration and hard stools.
    Medications:
    2. Stool Softeners: Over-the-counter products like docusate sodium to ease bowel movements.
    3. Fiber Supplements: Psyllium husk or methylcellulose to add bulk and softness to stool.
    4. Topical Anesthetics: Creams containing lidocaine to relieve pain.
    5. Nitroglycerin Ointment: Relaxes the anal sphincter to reduce pressure and improve blood flow.
    6. Calcium Channel Blockers: Topical diltiazem or nifedipine to relax the sphincter and improve healing.
    7. Warm Sitz Baths: Soaking the anal area in warm water for 10-20 minutes several times a day to promote healing and relieve discomfort.
    8. Botox Injections: Botulinum toxin injections into the anal sphincter to relax the muscle and promote healing.
    9. Surgical Interventions: Lateral Internal Sphincterotomy: A surgical procedure to cut a small portion of the anal sphincter to reduce pressure and allow healing. Anal Dilation: Gently stretching the anal sphincter under anesthesia to relieve pressure.
    Prevention Strategies for Anal Fissures
    1. Maintain Regular Bowel Movements:
    - Aim for soft, formed stools to reduce strain during bowel movements.
    2. High-Fiber Diet and Hydration: Continue a diet rich in fiber and maintain adequate fluid intake to prevent constipation.
3 .Proper Bathroom Habits: Avoid straining and prolonged sitting on the toilet. Respond promptly to the urge to have a bowel movement to prevent hard stools.
    4. Gentle Cleansing:
    - Use soft, unscented toilet paper or moistened wipes to clean the anal area gently.
    5. Regular Physical Activity: Exercise regularly to promote healthy bowel function.
    6. Avoid Irritating Substances: Avoid using harsh soaps, bubble baths, or other irritants in the anal area.

Im having success getting the bowl to start working within around 1 hour, after ive had 3 or so cups of coffee. But i suppose any hot drink will do the same. Maybe just plain hot water.. or tea etc.

One technic which is of utmost importance in treatment of anal fissure is gentle and slow dilatation of the anus and rectum with a KY jelly - lubricated dilatator . It reduces the tension of muscles there which are " angry " and agitated. Relieving the tension terminates in gradual healing of the fissure. Dilatation is better to be done three times daily . Lying on one side with the leg flexed at hip , helps the insertion of lubricated dilatator easy. One week is enough .
